A multicentre study of antifungal strategies and outcome of Candida spp. peritonitis in intensive-care units

Information on the species causing Candida peritonitis, their in vitro susceptibility, antifungal strategies in this setting and patient outcome is still scarce. AmarCand was a prospective, non-interventional study in 271 adult intensive-care unit (ICU) patients with proven invasive Candida infection who received systemic antifungal therapy (France, 2005-2006). Of these ICU patients, 93 (median age 65 years, simplified acute physiology score II 52) had Candida peritonitis, including 73 nosocomial peritonitis, 53 concomitant bacterial peritoneal infections and 26 candidaemias. Candida species were C. albicans (n = 63/108 isolates, 58%), C. glabrata (n = 22, 20%), C. krusei (n = 9), C. kefyr (n = 5), C. parapsilosis (n = 3), C. tropicalis (n = 3), C. ciferii (n = 2) and C. lusitaniae (n = 1). Of tested isolates, 28% were fluconazole-resistant or susceptible dose-dependent (C. albicans 3/32, C. glabrata 9/14, C. krusei 4/4). Empiric antifungal treatment was started 1 day (median) after peritonitis diagnosis, with fluconazole (n = 72 patients), caspofungin (n = 12), voriconazole (n = 3), amphotericin B (n = 2), or a combination (n = 4). Following susceptibility testing, empiric antifungal treatment was judged inadequate in 9/45 (20%) patients and modified in 30 patients (fluconazole was replaced by caspofungin (n = 14) or voriconazole (n = 4)). Mortality in ICU was 38% (35/93) and was not influenced by type of Candida species, fluconazole susceptibility, time to treatment, candidaemia, nosocomial acquisition, or concomitant bacterial infection. No specific factors for death were identified. In summary, a high proportion of fluconazole-resistant or susceptible dose-dependent strains was cultured. These results confirm the high mortality rates of Candida peritonitis and plead for additional investigation in this population. Antifungal treatment for severe cases of Candida peritonitis in ICU patients remains the standard care.

It is more prevalent in older patients, those receiving invasive ventilation, and those receiving tocilizumab treatment. CAPA is an independent predictor of ICU mortality.

Summary: This study collected retrospectively records of leishmaniasis cases diagnosed in 15 centers between 2014 and 2019, and analyzed data on country of infection, reason for traveling, infecting species, age, and sex. The results showed that visceral leishmaniasis was mainly acquired in Europe, while cutaneous leishmaniasis was primarily imported from outside Europe. The geographic species distribution largely confirmed known epidemiology, with some exceptions. The study highlights the importance of pooling species typing data from multiple centers to monitor changing epidemiology.

Aspergillus Lateral Flow Assay with Digital Reader for the Diagnosis of COVID-19-Associated Pulmonary Aspergillosis (CAPA): a Multicenter Study

Summary: This multicenter study evaluated the performance of the IMMY Aspergillus Galactomannan Lateral Flow Assay (LFA) for the diagnosis of pulmonary aspergillosis in patients with COVID-19-related acute respiratory failure, showing good sensitivity when used from respiratory samples but limited sensitivity in serum, possibly due to weak invasiveness during CAPA. Further mycological investigations are recommended for isolated results slightly above the cutoff.

Summary: This study retrospectively analyzed 119 cases of cerebral aspergillosis and identified two distinct disease patterns, hematogenous dissemination and contiguous dissemination, based on the route of spread. Hematogenous dissemination was associated with impaired consciousness, multiple abscesses, and microangiopathy, while contiguous dissemination was associated with cranial nerve palsy and evidence of meningitis. Hematogenous dissemination, hematological malignancy, and detection of serum galactomannan were associated with higher six-week mortality.

Summary: This study analyzed the clinical characteristics of cryptococcosis in kidney transplant recipients in France. The incidence of cryptococcal meningitis (CM) was relatively high, and the prognosis was poor. Hyponatremia and neurological symptoms were associated with CM.

Summary: This study conducted a systematic review and individual patient-level image analysis of CT findings of COVID-19-associated pulmonary aspergillosis (CAPA), revealing that CAPA can frequently manifest as COVID-19 pneumonia without common CT abnormalities of pulmonary aspergillosis. If abnormalities exist on CT images, CAPA may frequently accompany bronchial abnormalities.

Summary: This article describes and evaluates the treatment outcomes of cutaneous leishmaniasis (CL) among travelers and migrants in Europe caused by Leishmania major and Leishmania tropica. The study found that L. major infections were mostly found in individuals visiting friends and relatives, while L. tropica infections were mainly identified in migrants. Pentavalent antimony had a cure rate of 73% (L. major) and 78% (L. tropica) among patients.

Summary: The study aimed to determine the epidemiological cut-off values (ECVs) of ten antifungal agents in a wide range of yeasts and Aspergillus spp. using gradient concentration strips. Retrospective collection of minimum inhibitory concentrations for various antifungal agents was done from 35 French microbiology laboratories between 2002 and 2020. The calculated ECVs can aid in interpreting minimum inhibitory concentrations and identifying non-wild type isolates.

Summary: The study used data from the Mycosands survey to investigate the factors affecting the distribution of yeasts and molds along European shores. Environmental factors such as climate, soil properties, and water conditions were analyzed using species distribution modelling. The results showed that yeasts and molds have different distributions along European shores, with yeasts being more tolerant of low temperatures and molds showing affinity with specific soil compositions and water conditions.
