Molecular epidemiology of extended-spectrum β-lactamase-producing Klebsiella pneumoniae strains in a university hospital in Tunis, Tunisia, 1999-2005

During a period of 6 years and 5 months (January 1999 to May 2005), 103 extended-spectrum beta-lactamase (ESBL)-producing Klebsiella pneumoniae isolates, each from an individual patient or site, were collected at Mongi Slim University Hospital Centre, Tunis, Tunisia. The objectives of our work were the characterization of the bla genes encoding ESBLs, the investigation of clonal diversity of strains, and identification of the transmission modes of the resistance genes. We carried out detection by PCR and sequencing of the bla(SHV), bla(CTX-M) and bla(TEM) genes, transferability studies, plasmid replicon typing, and analysis by multilocus sequence typing (MLST) on selected isolates. Forty-seven isolates were found to be producers of CTX-M-type ESBLs, of which 43 were CTX-M-15, two CTX-M-14 and two CTX-M-27. Fifty-eight isolates were producers of SHV-12, and three were producers of SHV-2a. More than one ESBL was detected in seven isolates, as five produced both CTX-M-15 and SHV-12, and two produced both CTX-M-27 and SHV-12. By a PCR-based replicon typing method, the plasmids carrying the bla(SHV-2a) or bla(CTX-M-15) genes were assigned to IncFII or, more rarely, to IncL/M types. Of 12 plasmids carrying the bla(SHV-12) gene, only one could be typed: it was positive for the HI2 replicon. The MLST results showed large genetic background diversity in the SHV-12-producing isolates and dissemination of specific clones of the CTX-M-15-producing isolates within the same ward and among wards, and suggested endemicity with horizontal dissemination of the bla(CTX-M-15) and the bla(SHV-12) genes.

Genomic evolution of the globally disseminated multidrug-resistant Klebsiella pneumoniae clonal group 147

The CG147 genomes exhibit a variety of acquired resistance genes and mutations, with significant diversity in molecular characteristics among different phylogenetic clades within the ST147 group.

Summary: The impact of COVID-19 pandemic on bloodstream infections due to Streptococcus pneumoniae and Streptococcus pyogenes was assessed in 25 university hospitals in Paris. The incidence rates of BSIs, which had been stable in 2018 and 2019, decreased for the two pathogens during the two lockdown periods in 2020. Containment policies such as social distancing, masking, and hand hygiene strengthening in community and hospital settings are likely to reduce BSIs caused by these pathogens.

Summary: The French National Reference Centre for Escherichia coli, Shigella and Salmonella (FNRC-ESS) identified two human clusters of Salmonella infections in France. The outbreaks were caused by Salmonella enterica serotype Bovismorbificans, ST142, HC5_243255 in September-November 2020, and S. enterica serotype 4,12:i:-, ST34, HC5_198125 in October-December 2020. These outbreaks were traced back to the consumption of dried pork sausages from the same manufacturer. The French authorities issued recalls and withdrawals of affected products, and notifications were sent to European food safety systems.

Rapid emergence of extensively drug-resistant Shigella sonnei in France

Summary: There has been a rapid emergence of extensively drug-resistant (XDR) Shigella sonnei, including an international epidemic strain. A laboratory surveillance study in France from 2005 to 2021 revealed a dramatic increase in the proportion of isolates simultaneously resistant to ciprofloxacin, third-generation cephalosporins, and azithromycin since 2015. Genomic analysis identified 13 different clusters within the CIP-resistant sublineage 3.6.1, which was selected in South Asia 15 years ago.

Nanopore-only assemblies for genomic surveillance of the global priority drug- resistant pathogen, Klebsiella pneumoniae

Summary: Oxford Nanopore Technologies (ONT) sequencing has potential for genomic epidemiology and public health investigations of bacterial pathogens, but low base-call accuracy limits its reliability for critical tasks. In this study, we tested the accuracy of ONT-only assemblies for genotyping and cluster detection. We found that the SUP basecalling method performed the best, accurately identifying capsule type, multi-locus sequence type, and acquired antimicrobial resistance genes. However, cluster detection was challenging with ONT data.

Phages against Noncapsulated Klebsiella pneumoniae: Broader Host range, Slower Resistance

Genomic Epidemiology of Corynebacterium diphtheriae in New Caledonia

Summary: The study analyzed the clinical and microbiological features of Corynebacterium diphtheriae isolates in New Caledonia. The majority of isolates were from cutaneous sources and were associated with Staphylococcus aureus and/or Streptococcus pyogenes. Toxigenic isolates were observed in patients returning from Vanuatu. The isolates showed susceptibility to commonly used antibiotics. The study highlights the importance of diphtheria surveillance and improving immunization in neighboring islands.

Corynebacteria of the diphtheriae Species Complex in Companion Animals: Clinical and Microbiological Characterization of 64 Cases from France

Summary: We report on the occurrence and clinical and microbiological characteristics of infections caused by members of the CdSC in companion animals. This is the first study based on a systematic analysis of a very large animal cohort (18,308 samples) and provides data on the frequency of CdSC isolates in various types of clinical samples from animals. The findings suggest that CdSC members can cause infections in animals such as rhinitis, dermatitis, non-healing wounds, and otitis.

Summary: Lake Tanganyika basin in Africa is a hotspot for cholera. Vibrio cholerae O1 isolates collected from the Democratic Republic of the Congo side of the lake during 2001-2020 belonged to 2 out of the 5 clades of the AFR10 sublineage. One clade became dominant after acquiring a parC mutation that reduced susceptibility to ciprofloxacin.
