Glycated albumin to glycated hemoglobin ratio and mortality in diabetic patients on dialysis: a new association

Summary: The GA/A1c ratio is significantly associated with 3-year mortality, especially infectious and cardiovascular mortality, in dialysis patients with diabetes. This ratio may be a promising new clinical indicator of survival in these patients, independent of their current glycemic control and nutritional markers.

Factors associated with glycemic control and diabetes complications in a group of Saudi patients with type 2 diabetes

Summary: This study assessed the prevalence and factors related to suboptimal glycemic control and complications in a group of patients with type 2 diabetes. The results showed that longer disease duration was significantly associated with poor glycemic control, while age was independently associated with macrovascular complications. Hyperlipidemia was significantly associated with neuropathy and retinopathy. Lower physical activity was independently associated with neuropathy.

Summary: This study found that there was no significant difference in overall survival between lung cancer patients with diabetes and those without diabetes, but a glycated hemoglobin A1C level higher than 6.6% was associated with an increased risk of overall mortality. Proper glycemic control with HbA1c <= 6.6% was shown to improve overall survival for patients with advanced NSCLC.

Impact of stress hyperglycemia ratio, derived from glycated albumin or hemoglobin A1c, on mortality among ST-segment elevation myocardial infarction patients

Summary: This study found that SHR1 and SHR2, derived from HbA1c or GA, serve as robust and independent prognostic markers for STEMI patients undergoing PCI. Increases in both SHR1 and SHR2 were associated with higher risks of in-hospital death and all-cause mortality. Additionally, incorporating SHR1 or SHR2 into the base model significantly improved the discrimination and risk reclassification for in-hospital and all-cause mortality. The effects of SHR1 and SHR2 were more pronounced in patients with hypercholesteremia.

Poor quality of sleep in Mexican patients with type 2 diabetes and its association with lack of glycemic control

Summary: This study aimed to determine the association between sleep quality and lack of glycemic control in type 2 diabetes patients. The results showed a high prevalence of poor sleep quality and lack of glycemic control in the study population. Poor sleep quality was found to significantly double the risk of lack of glycemic control, even in the presence of other risk factors such as napping, diabetes duration, insulin use, and high diastolic blood pressure.

Summary: HbA1c shows promise as a biomarker for screening GDM during the first trimester of pregnancy, with a cut-off value of 5.33%, sensitivity of 71.9%, and specificity of 82.8%. Pregnant women with ft-HbA1c levels above the prediabetes limit are at increased risk of developing GDM.

Alleviating symptoms in patients undergoing long-term hemodialysis: a focus on chronic kidney disease-associated pruritus

Summary: Despite the limited increase in life expectancy for patients with end-stage kidney disease after the breakthrough of kidney replacement therapy, the quality of life and relief of symptoms have become increasingly important to patients. Currently, most dialysis-associated symptoms and adverse effects do not have approved treatments, and the few available treatments may add further adverse effects. This article demonstrates how understanding the pathophysiology of a particular symptom (chronic kidney disease-associated pruritus) in dialysis led to the development and regulatory approval of a treatment for that symptom. This approach can be applied to other dialysis-associated symptoms, improving the remaining years of patients' lives.

Proteinuria is a serious issue for renal disease patients, and nutritional strategies need to be explored to control proteinuria and inflammation, which is of great significance for the treatment of autoimmune kidney diseases such as glomerulonephritis.

However, providers perceive payor policy and out-of-state licensing as major obstacles to the growth of telemedicine.

System-Level Strategies to Improve Home Dialysis

Summary: Advocacy and policy change play a crucial role in improving the quality of care and support for patients on home dialysis. However, only a minority of patients in the United States choose home dialysis due to various factors, such as limited infrastructure, lack of patient education and training staff, patient-specific barriers, and insufficient physician expertise. This article discusses the trends in home dialysis use, the definition and measurement of high-quality care for home dialysis patients, and compares policy and advocacy efforts in the United States with those in other countries. Future directions for quality improvement and advocacy efforts are also discussed.
