Plasmid-Mediated Ciprofloxacin Resistance Imparts a Selective Advantage on Escherichia coli ST131

Summary: The plasmid-associated resistance gene, AAC (6')-Ib-cr, provides a selective advantage for the dissemination of Escherichia coli ST131, a highly antibiotic resistant clone. This gene, which inactivates the fluoroquinolone antibiotic ciprofloxacin, is present in over 70% of strains from the rapidly growing subgroup of multidrug resistant ST131. The presence of this gene gives ST131 an edge even in the presence of chromosomal FQ-resistance mutations.

The effects of different doses of inhaled bacteriophage therapy for Pseudomonas aeruginosa pulmonary infections in mice

Summary: This study investigated the dose-response effect of Pseudomonas-targeting phage PEV31 in a mouse lung infection model. The results showed that phage treatment significantly reduced pulmonary bacterial load, regardless of the delivered phage dose. The study provides important insights for the dose selection of inhaled phage therapy for P. aeruginosa respiratory infections in humans.

Polymyxin causes cell envelope remodelling and stress responses in mcr-1-harbouring Escherichia coli

Summary: This study investigated the concentration-dependent transcriptomic responses of mcr-harbouring E. coli to polymyxin treatment. The results revealed that cell envelope remodelling, pEtN modification, iron acquisition, and oxidative stress protective mechanisms play a key role in the survival of mcr-carrying E. coli treated with polymyxin.

A polytherapy based approach to combat antimicrobial resistance using cubosomes

Summary: The use of nano therapies to treat antibiotic resistance is gaining interest due to the depleted antimicrobial drug pipeline and the increasing prevalence of Gram-negative 'superbugs'. This study examines the antimicrobial activity of polymyxin-loaded cubosomes and explores a polytherapy treatment using cubosomes in combination with polymyxin. The findings show that polytherapy treatment significantly increases antimicrobial activity compared to using polymyxin-loaded cubosomes or polymyxin and cubosomes alone.

Comparative Proteomics of Outer Membrane Vesicles from Polymyxin-Susceptible and Extremely Drug-Resistant Klebsiella pneumoniae

Summary: Outer membrane vesicles (OMVs) can help bacteria to fight antibiotics by spreading resistance genes and acting as protective armor. Proteomic analysis revealed that OMVs shield K. pneumoniae and act as decoys against polymyxin attack by reducing the export of resistance-related proteins. Comparative proteomics of OMVs from polymyxin-susceptible and resistant strains identified perturbed pathways and highlighted the potential roles of OMVs in adhesion, virulence, and antibiotic resistance. These findings provide new insights into the importance of OMVs in the bacterial response to polymyxins.

Therapeutic Potential of Intravenous Phage as Standalone Therapy for Recurrent Drug-Resistant Urinary Tract Infections

Summary: Recurrent urinary tract infections (rUTI) are common and problematic in kidney transplant recipients (KTR). This case report presents a successful treatment of rUTI caused by ESBL Klebsiella pneumoniae in a KTR using 4 weeks of adjunctive intravenous bacteriophage therapy alone, without concomitant antibiotics. The patient remained infection-free during one year of follow-up.

Duration of antibiotic treatment and timing of oral switching for bloodstream infections: a survey on the practices of infectious diseases and intensive care physicians

Summary: This study investigated the current practice of antibiotic treatment duration and timing of IV to oral switching for common bacteraemic conditions among Turkish infectious disease and ICU physicians. The results showed that the most commonly recommended duration of antibiotic treatment for bacteraemia was 14 days, followed by 10 and 7 days. Most physicians recommended switching to oral antibiotics after 7 days of IV treatment. Prolonged treatment durations may be due to a presumption that resistant bacterial infections require longer therapy.

Summary: An outbreak of vanB-type VREfm ST78 occurred in a large tertiary Australian hospital. Whole-genome sequencing revealed a complex transmission route involving unknown environmental reservoirs. This genomic analysis provided detailed information on the local epidemiology of the outbreak strain and facilitated better targeted control measures for VREfm.
