Caesarean birth by maternal request: a poorly understood phenomenon in low- and middle-income countries

Margo S. Harrison, Ana Garces, Lester Figueroa, Fabian Esamai, Sherri Bucher, Carl Bose, Shivaprasad Goudar, Richard Derman, Archana Patel, Patricia L. Hibberd, Elwyn Chomba, Miusaku Mwenechanya, Michael Hambidge, Nancy F. Krebs

Summary: The trend of caesarean birth by maternal request in low- and middle-income countries is unclear, but factors like age, education, multiple gestation, and hypertensive disease are associated with maternal request for caesarean birth. Women who undergo caesarean birth by maternal request are more likely to breastfeed within 1 hour of delivery, with no other adverse outcomes compared to those who undergo caesarean birth for medical reasons.

Seasonal trends and maternal characteristics as predictors of maternal undernutrition and low birthweight in Eastern Maharashtra, India

Lindsey M. Locks, Archana Patel, Elizabeth Katz, Elizabeth Simmons, Patricia Hibberd

Summary: Studies have shown that in rural and peri-urban areas in South Asia, there is minimal seasonal fluctuation in the nutritional status of women and infants. Maternal characteristics such as age, education, parity, and height are significantly associated with nutritional status. The prevalence of maternal anemia decreased significantly from 2014 to 2018.

Assessing the Respiratory Effects of Air Pollution from Biomass Cookstoves on Pregnant Women in Rural India

Raj Parikh, Sowmya R. Rao, Rakesh Kukde, George T. O'Connor, Archana Patel, Patricia L. Hibberd

Summary: The study evaluated respiratory health among pregnant women in rural India using biomass fuel, finding higher levels of FeNO, respiratory questionnaire scores, and dyspnea scale scores in women exposed to biomass fuel compared to those using LPG. These results suggest the adverse effects of biomass smoke exposure during pregnancy and highlight the potential utility of FeNO as a biomarker for inflammation in this population.

Trends in cesarean section rates in private and public facilities in rural eastern Maharashtra, India from 2010-2017

Elizabeth Simmons, Kevin Lane, Sowmya R. Rao, Kunal Kurhe, Archana Patel, Patricia L. Hibberd

Summary: Cesarean section rates have significantly increased in both public and private facilities in India from 2010 to 2017. The odds of having a CS at a private facility were 40% higher than at a public facility after adjusting for other known risk factors. Cesarean section rates have unequal spatial distributions at the subdistrict level.

Improving the experience of facility-based delivery for vulnerable women through obstetric care navigation: a qualitative evaluation

Kirsten Austad, Michel Juarez, Hannah Shryer, Patricia L. Hibberd, Mari-Lynn Drainoni, Peter Rohloff, Anita Chary

Summary: Global disparities in maternal mortality could be reduced by universal facility delivery. Obstetric care navigators (OCNs) can improve mothers' experiences in public hospitals and reduce limitations faced by providers. However, broader buy-in from hospital staff-especially nurses-appears critical to program success.
