The diagnosis of nongenital Neisseria gonorrhoeae infection by the Becton Dickinson ProbeTec ET Chlamydia trachomatis and N. gonorrhoeae Amplified DNA assay had low positive predictive value among an Australian community-based sample of homosexual men. Only 30.4% of oropharyngeal samples and 73.7% of anorectal samples were positive for N. gonorrhoeae by the porA assay. The accuracy of nucleic acid amplification tests in this context is compromised without supplemental testing.

How to have sex in a pandemic: the development of strategies to prevent COVID-19 transmission in sexual encounters among gay and bisexual men in Australia

Dean Murphy, Jeanne Ellard, Lisa Maher, Peter Saxton, Martin Holt, Bridget Haire, Andrew Grulich, Benjamin Bavinton, Steven Philpot, Adam Bourne, Daniel Storer, Fengyi Jin, Mohamed Hammoud, Garrett Prestage

Summary: Although previous studies have reported a decrease in sexual partner numbers among gay and bisexual-identifying men during the early period of the COVID-19 pandemic, few studies have explored risk reduction strategies. This study analyzed free-text responses from an online survey conducted from April to July 2020 to understand how men sought to minimize the risk of COVID-19 during sexual encounters. The findings highlight partner selection, risk assessment, and in situ practices as important strategies employed by men to reduce COVID-19 transmission risk. The study also discusses the parallels between these strategies and early responses to HIV.

Summary: Attribution algorithms have limited accuracy in defining HPV genotype risk in anal disease for populations with a high rate of multiple HPV infections, particularly in GBM. The proportional algorithm performed the best among the five algorithms tested.

Second Primary Cancers in People With HIV/AIDS: A National Data Linkage Study of Incidence and Risk Factors

Summary: A national population-based data linkage study in Australia revealed the incidence and risk factors of second primary cancer (SPC) in people living with HIV (PLWHIV) between 1982 and 2012. The study found that the risk of SPC remains high in PLWHIV, with a temporal increase observed in non-infection-related SPCs while the incidence of infection-related SPCs decreased.

Infectious syphilis in women and heterosexual men in major Australian cities: sentinel surveillance data, 2011-2019

Hellard, Basil Donovan, Rebecca J. Guy

Summary: This study examined changes in the positive infectious syphilis test rate among women and heterosexual men in major Australian cities, and identified social, biomedical, and behavioural determinants of health that influenced the test rate. The results showed an increase in the positive test rate in 2019 compared to 2011. The study recommends routine syphilis screening for individuals attending reproductive health or alcohol and drug services.

Renal impairment associated with tenofovir disoproxil fumarate for antiretroviral therapy and HIV pre-exposure prophylaxis: An observational cohort study

Summary: This study analyzed data from 52 sexual health clinics in Australia and found that the use of TDF for pre-exposure prophylaxis (PrEP) was associated with lower rates of renal impairment than its use for HIV infection. Factors such as age and baseline kidney function were predictors of renal impairment.

Interim position statement on doxycycline post-exposure prophylaxis (Doxy-PEP) for the prevention of bacterial sexually transmissible infections in Australia and Aotearoa New Zealand - the Australasian Society for HIV, Viral Hepatitis and Sexual Health Medicine (ASHM)

Vincent J. Cornelisse, Jason J. Ong, Nathan Ryder, Catriona Ooi, Arthur Wong, Penny Kenchington, Massimo Giola, Basil Donovan, Judith A. Dean, Jean-Michel Molina, Nicholas A. Medland

Summary: Recent studies show the effectiveness of using doxycycline (Doxy-PEP) to prevent bacterial STIs among gay, bisexual, and other men who have experienced multiple STIs. However, potential adverse outcomes such as antimicrobial resistance and disruption of the microbiome still need further investigation. This position statement aims to provide clinicians in Australia and New Zealand with evidence-based information for discussions with patients planning to use Doxy-PEP.

Agreements, Behaviour, and Change: Sex Outside the Relationship in Male HIV-negative Partners in HIV Serodiscordant Relationships in Australia, Brazil, and Thailand

James Gray, Garrett Prestage, Fengyi Jin, Nittaya K. Phanuphak, Ruth K. Friedman, Christopher K. Fairley, Anthony Kelleher, David J. Templeton, Iryna Zablotska-Manos, Jennifer Hoy, Anna McNulty, David Baker, Graham Brown, Andrew Grulich, Benjamin Bavinton, Opposites Attract Study Group

Summary: Male HIV-serodiscordant couples have different relationship agreements regarding sex outside the relationship. These agreements are largely stable over time, with a majority of HIV-negative men reporting the same agreement across follow-up. Breaking an agreement is predicted by condomless anal intercourse within the relationship and PrEP use in the last three months. Focus needs to be placed on sex occurring outside the relationship and the agreements that facilitate this when considering HIV transmission risk for HIV-negative men in serodiscordant relationships.

Requesting HIV Results Be Conveyed in-Person: Perspectives of Clinicians and People Recently Diagnosed with HIV

Nathanael Wells, Dean Murphy, Jeanne Ellard, Chris Howard, Phillip Keen, Christopher Fairley, Basil Donovan, Garrett Prestage

Summary: In Australia, it is recommended that HIV-positive test results be delivered in person whenever possible. However, HIV-negative and other STI results are routinely given over the phone or through text messages. Requesting individuals to receive positive HIV test results in person may deviate from standard healthcare delivery and be seen as indicating a positive HIV diagnosis.

Rapid molecular detection of CMY-2, and CTX-M group 1 and 9 variants via recombinase polymerase amplification

Nicole G. Ertl, Adam D. Irwin, Joanne Macdonald, Michelle J. Bauer, Claire Y. T. Wang, Patrick N. A. Harris, Claire Heney, Hosam M. Zowawi, David M. Whiley

Summary: In this study, rapid diagnostic assays using recombinase polymerase amplification with lateral flow detection (RPA-LF) were developed and tested for detecting the beta-lactamase genes CTX-M and CMY-2. The assays showed high sensitivity (82.1% - 100%) and specificity (100%), with a short turnaround time (15-20 min). The results suggest that these RPA-LF assays have the potential to be used in clinical and low-resource settings for rapid antimicrobial resistance detection.

Sexually transmissible infections (STI) and HIV testing and diagnosis among Aboriginal and non-Aboriginal adolescents in contact with the Australian justice system: a cross-sectional study

Patricia Taflan, Paul L. Simpson, Mandy Wilson, Jocelyn Jones, Basil Donovan, Janaki Amin, Sally Nathan, Tony Butler

Summary: The associations between factors related to sexually transmissible infections (STI) and HIV testing and diagnosis among justice-involved adolescents, particularly Aboriginal or Torres Strait Islander peoples, are unclear. This study found that for Aboriginal participants, being male and having fewer sexual partners were associated with never having had an STI/HIV test. For non-Aboriginal participants, being male, single, attending school, not seeking sexual health information, and having lower STI/HIV knowledge scores were associated with never having had an STI/HIV test. Factors associated with STI diagnosis were non-heterosexual sexual orientation, transactional sex, and seeking sexual health information.
