Rapid lung MRI in children with pulmonary infections: Time to change our diagnostic algorithms

BackgroundTo determine the diagnostic utility of a new rapid MRI protocol, as compared with computed tomography (CT) for the detection of various pulmonary and mediastinal abnormalities in children with suspected pulmonary infections. MethodsSeventy-five children (age range of 5 to 15 years) with clinically suspected pulmonary infections were enrolled in this prospective study, which was approved by the institutional ethics committee. All patients underwent thoracic MRI (1.5T) and CT (64 detector) scan within 48 h of each other. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of MRI were evaluated with CT as a standard of reference. Inter-observer agreement was measured with the kappa coefficient. ResultsMRI with a new rapid MRI protocol demonstrated sensitivity, specificity, PPV, and NPV of 100% for detecting pulmonary consolidation, nodules (>3 mm), cyst/cavity, hyperinflation, pleural effusion, and lymph nodes. The kappa-test showed almost perfect agreement between MRI and multidetector CT (MDCT) in detecting thoracic abnormalities (k = 0.9). No statistically significant difference was observed between MRI and MDCT for detecting thoracic abnormalities by the McNemar test (P = 0.125). ConclusionRapid lung MRI was found to be comparable to MDCT for detecting thoracic abnormalities in pediatric patients with clinically suspected pulmonary infections. It has a great potential as the first line cross-sectional imaging modality of choice in this patient population. However, further studies will be helpful for confirmation of our findings. J. Magn. Summary: This study compares the sensitivity of P-ROSE (pulmonologist on-site evaluation) and WHOSE (cytologist off-site evaluation) for adequacy and diagnosis of cytology specimens obtained by EBUS-TBNA. The results show that WHOSE significantly improves the performance of P-ROSE. This suggests that off-site evaluation of slides by a cytologist using WhatsApp has a positive impact on on-site evaluation.

Isolated tracheobronchial mucormycosis is a rare disease characterized by cough, haemoptysis, and hoarseness of voice. Bronchoscopy plays an important role in early diagnosis, and treatment with antifungals and control of risk factors are crucial.

Summary: This systematic review and meta-analysis aimed to evaluate the clinical effectiveness and safety of omalizumab in patients with treatment-refractory allergic bronchopulmonary aspergillosis (ABPA). The results showed that omalizumab treatment significantly reduced exacerbations and oral corticosteroid (OCS) use, improved lung function and asthma control in ABPA patients, and was well-tolerated.

Summary: In this open-label trial, we compared the efficacy and safety of nebulized amphotericin B (NAB) in combination with intravenous liposomal amphotericin B to intravenous liposomal amphotericin B alone for the treatment of pulmonary mucormycosis (PM). The results showed that the addition of NAB did not significantly improve overall response at 6 weeks or reduce 90-day mortality. Further research is needed to explore alternative treatment options for PM.

Summary: This study aimed to estimate the prevalence of aspergillus sensitization (AS) and allergic bronchopulmonary aspergillosis (ABPA) in children with bronchial asthma through a systematic review of literature. The results showed a high prevalence of AS and ABPA in asthmatic children. Community-based studies from different ethnicities using a standardized methodology are needed to determine the true prevalence of AS and ABPA in pediatric asthma.

Role of serum procalcitonin in the diagnosis and monitoring of treatment response in treatment-naive subjects with chronic pulmonary aspergillosis

Summary: This study investigated the diagnostic performance of plasma procalcitonin (PCT) in chronic pulmonary aspergillosis (CPA). The results showed that the proportion of elevated PCT in CPA patients was very low, with a sensitivity of only 3.7% and a specificity of 100%. After treatment, the plasma PCT levels decreased in some patients, but 45% of the patients had an increase or no change in PCT levels.

Summary: This study assessed the diagnostic accuracy of cerebrospinal fluid cytochemistry in diagnosing meningitis in infants below 90 days of age and determined the certainty of evidence. The results showed that cerebrospinal fluid white blood cell count and protein have good diagnostic accuracy, while glucose has good specificity but poor sensitivity. The overall certainty of the evidence was moderate.

Summary: Mucormycosis in HIV infection is rare without additional risk factors or intravenous drug use. In this systematic review, 94 cases were classified into three groups: HIV with additional risk factors, intravenous drug users, and no other risk factor for mucormycosis. The most common presentations varied between the groups, with renal and cerebral mucormycosis being most common in intravenous drug users. The overall mortality rate was 53% and did not differ significantly among the groups.

Relationship between Aspergillus and asthma

Diagnosis involves careful assessment of clinical history, immunological tests, and imaging. Treatment plans for fungal asthma vary based on severity and may include inhaled corticosteroids, bronchodilators, and antifungal therapy.
