Predicting late recurrence in surgically treated patients with Cushing's disease

Objective Cushing's disease (CD) has an uncertain prognosis because patients achieving remission after transsphenoidal pituitary neurosurgery (TSS) may relapse. We aimed to identify factors predicting relapse, focusing on desmopressin (DDAVP) and corticotropin-releasing hormone (CRH) tests after surgery. Materials and methods Fifty-seven patients with CD (mean age 36 years) after TSS experienced remission (24 cases), late relapse (LR) (15 cases), or persistent disease (18 cases). Results The median time to relapse was 40 months. ACTH levels increased after both DDAVP and CRH stimulation, with a significantly higher response in the late recurrence group, showing this to be an indicator of increased risk of relapse. In the logistic regression model, a rise in ACTH >9 pg/ml after DDAVP and >36.7 pg/ml after CRH showed a sensitivity of 93% and 73%, respectively, a specificity of 82% and 76% in LR group. The area under the curve was 0.91 for DDAVP, 0.80 for CRH and 0.95 for DDAVP+CRH test, i.e. the combined tests performed better than each test alone, but not to a statistically significant degree. A response to both tests resulted in a positive predictive value (PPV) of 100%, while no response to either test in a negative predictive value (NPV) of 100%. Conclusions ACTH hyper-responsiveness to DDAVP stimulation proved a valuable indicator of relapsing patients with high sensitivity and specificity; in selected cases when a clear high increment of ACTH level is not evident, the CRH test might be used as additional tool to confirm the risk of future relapses.

Summary: This study investigated the diversity of approaches utilized for each diagnostic step in different expert centers regarding primary aldosteronism (PA). The results showed significant variations in blood sampling conditions, assay methods, screening and confirmatory tests among centers. This diversity may lead to inconsistent diagnosis of PA and limit comparison of evidence between centers. Standardization of the diagnostic process and revisiting the concept of mild PA are necessary.

Dynamic Testing for Differential Diagnosis of ACTH-Dependent Cushing Syndrome: A Systematic Review and Meta-analysis

Paradoxical GH increase after oral glucose load in subjects with and without acromegaly

Summary: This study aims to evaluate the growth hormone (GH) response in patients with and without acromegaly. It was found that a paradoxical increase in GH levels was observed after glucose intake. The study also revealed a higher prevalence of GH response in acromegalic patients, which was associated with an increased occurrence of glucose metabolism abnormalities.

Second-line tests in the differential diagnosis of neoplastic and non-neoplastic hypercortisolism: a systematic review and meta-analysis

Summary: The study aimed to provide an overview of available dynamic tests for discriminating between neoplastic and non-neoplastic hypercortisolism and to quantify their diagnostic performance. The combined Dex-CRH test showed the highest sensitivity, while the CRH test demonstrated excellent specificity. Further research is needed, particularly in mild Cushing's Disease and well-characterized non-neoplastic hypercortisolism patients.

Long-Term Outcomes after Surgery for Pheochromocytoma and Sympathetic Paraganglioma

Summary: Primary hyperthyroidism is an endocrine disorder characterized by excessive thyroid hormone synthesis and secretion. Treatment options include medical therapy, radioactive iodine, total thyroidectomy, and in severe cases, therapeutic plasma exchange. Two cases of thyrotoxicosis, one caused by amiodarone treatment and the other by methimazole-induced agranulocytosis, were described as examples of therapeutic plasma exchange applications in clinical practice.

Summary: This study aims to investigate the prevalence of comorbidity in adults with 21-hydroxylase CAH and the current practice for assessing comorbidity. A questionnaire was sent to 46 expert centres managing adults with CAH, collecting information on therapy and surveillance practice for osteoporosis/osteopaenia, hyperlipidaemia, type 2 diabetes/hyperinsulinaemia, hypertension, CV disease, and obesity. The results indicate that cardiometabolic and bone morbidities are not uncommon in adults with CAH. Standardized screening and collection of data are needed to explore optimal therapy.
