The expression of glucocorticoid and mineralocorticoid receptors in pituitary tumors causing Cushing's disease and silent corticotroph tumors

Summary: Pituitary neuroendocrine corticotroph tumors commonly cause Cushing's disease (CD) due to increased ACTH secretion. However, some corticotroph tumors remain clinically non-functioning. Cortisol secretion is regulated by the hypothalamic-pituitary-adrenal axis, and glucocorticoids act through GR and MR receptors to reduce ACTH levels. This study aimed to determine the role of GR and MR expression in functioning and silent corticotroph tumors.

Postoperative Serum Cortisol and Cushing Disease Recurrence in Patients With Corticotroph Adenomas

Summary: This study retrospectively analyzed postoperative serum cortisol levels and recurrence rates in patients with Cushing's disease, and found that the lowest postoperative cortisol level is the most important predictor for recurrence and time-to-recurrence. Compared to the first postoperative cortisol level and cortisol halving time, a nadir < 2 μg/dL appears within the first 24 to 48 hours after surgery and has the strongest association with long-term remission.

A case of adrenal Cushing's syndrome and primary hyperparathyroidism due to an atypical parathyroid adenoma

Summary: This case report describes a 31-year-old woman with hirsutism, amenorrhea, and weight gain, who was diagnosed with ACTH-independent Cushing's syndrome and primary hyperparathyroidism due to a parathyroid adenoma. Successful resection of both adrenal and parathyroid lesions led to a good clinical outcome for the patient. Genetic testing revealed no pathogenic variants, and long-term follow-up is needed to monitor for recurrence of hypercalcemia and hypercortisolemia.

Summary: The 2017 World Health Organization classification of endocrine tumors defines pituitary adenomas based on their cell lineages, with T-PIT serving as a complementary tool for identifying silent corticotroph adenomas (SCAs). Compared to functioning corticotroph adenomas, SCAs have a more aggressive course and resistance to conventional treatments due to intrinsic histological features. Molecular mechanisms studies of SCA pathogenesis are crucial for diagnosis and management. Understanding clinical features in the context of pathophysiology is important for optimal management, involving surgical, radiation, and/or medical therapies.

Plasma sodium and potassium concentrations after hypophysectomy in dogs with corticotroph adenomas

Summary: This study aimed to describe electrolyte abnormalities, particularly hypernatremia, following transsphenoidal hypophysectomy in dogs with pituitary-dependent hypercortisolism. The results showed that hypernatremia was a common complication postoperatively, but did not seem to have a significant impact on postoperative outcome. Variations in plasma potassium concentrations were also noted, but did not influence postoperative outcome.

Summary: This study investigated the prevalence, presentation, and outcome of CTP-BADX/NS in Cushing's disease patients included in the ERCUSYN database. The results showed that 45% of BADX patients developed CTP-BADX/NS and only half of them were able to control tumor progression. Further studies are needed to establish effective strategies for prevention and treatment.

Clinical Study and Systematic Review of Pituitary Microadenomas vs. Macroadenomas in Cushing's Disease: Does Size Matter?

Summary: This study compared the clinical and biochemical characteristics of patients with corticotroph microadenomas and macroadenomas. It was found that macroadenomas had higher levels of ACTH but no association was found between tumor size and cortisol hypersecretion or clinical features of hypercortisolemia. Furthermore, patients with macroadenomas with sphenoid or cavernous sinus invasion had higher biochemical burden, highlighting that extent/location of the adenoma may be more important than size per se.

The prevalence of SCAs, especially ACTH-negative SCAs, was substantially underestimated and should be given enough attention due to the high aggressiveness of this subtype of refractory pituitary adenoma.

Summary: A new meal tolerance test (MTT) is useful in evaluating glucose intolerance and triglyceride excursions in morbidly obese patients. It shows high concordance with the oral glucose tolerance test (OGTT) in patients with normal glucose tolerance, but indicates a milder severity of glucose intolerance in patients with impaired glucose tolerance or diabetes mellitus. The MTT results also suggest an association between glucose disposal rates and increased levels of the gut hormone, glucose-dependent insulinotropic peptide (GIP). The MTT is clinically valuable in assessing abnormal glucose and triglyceride metabolism in morbidly obese patients.

Advanced proliferative diabetic retinopathy and macular edema in acromegaly: a case report and literature review

Summary: This article describes the multimodal management of a patient with proliferative diabetic retinopathy and diabetic macular edema associated with active acromegaly. The patient received various treatments including anti-vascular endothelial growth factor treatments, cataract surgeries, and retinal direct laser photocoagulation, as well as gradual glycemic control with basal insulin. The patient's acromegaly went into remission after a successful surgery, leading to improved visual acuity without worsening of diabetic retinopathy.

Summary: This study aimed to determine whether the diuretic effect of SGLT2 inhibitors could modify skin hydration status in patients with type 2 diabetes mellitus. The results showed that ipragliflozin treatment for 14 days did not significantly affect skin hydration status in well-controlled type 2 diabetes patients. There was a significant linear correlation between skin water content at pre-treatment and 14 days after treatment with each drug.
