The Korean Neonatal Network: An Overview

Currently, in the Republic of Korea, despite the very-low-birth rate, the birth rate and number of preterm infants are markedly increasing. Neonatal deaths and major complications mostly occur in premature infants, especially very-low-birth-weight infants (VLBWIs). VLBWIs weigh less than 1,500 g at birth and require intensive treatment in a neonatal intensive care unit (NICU). The operation of the Korean Neonatal Network (KNN) officially started on April 15, 2013, by the Korean Society of Neonatology with support from the Korea Centers for Disease Control and Prevention. The KNN is a national multicenter neonatal network based on a prospective web-based registry for VLBWIs. About 2,000 VLBWIs from 60 participating hospital NICUs are registered annually in the KNN. The KNN has built unique systems such as a web-based real-time data display on the web site and a site-visit monitoring system for data quality surveillance. The KNN should be maintained and developed further in order to generate appropriate, population-based, data-driven, health-care policies; facilitate active multicenter neonatal research, including quality improvement of neonatal care; and ultimately lead to improvement in the prognosis of high-risk newborns and subsequent reduction in health-care costs through the development of evidence-based neonatal medicine in Korea.

Mesenchymal Stem Cells and Formyl Peptide Receptor 2 Activity in Hyperoxia-Induced Lung Injury in Newborn Mice

Summary: This study investigates the role of Formyl peptide receptor (FPR) 2 in neonatal hyperoxia-induced lung injury and the potential of mesenchymal stem cells (MSCs) to attenuate this injury by regulating FPR2 activity. The findings suggest that MSCs can reduce FPR2 activity and protect against hyperoxic lung injury.

Early Discontinuation of Levothyroxine Treatment Is Safe and Feasible in Extremely Low Birth Weight Infants with Delayed Hyperthyrotropinemia

Summary: This study investigated the feasibility of early discontinuation of levothyroxine replacement therapy (LRT) for delayed hyperthyrotropinemia in extremely low birth weight infants. The results showed no significant differences in short-term and long-term outcomes between the short-term and long-term LRT groups, but the mortality rate was significantly higher in the no LRT required group compared to the long-term LRT group.

Summary: This study examined the associations between serum folate and homocysteine concentrations and the risk of all-cause, CVD, and cancer mortality in Korean adults. The findings suggest that higher serum homocysteine and lower serum folate concentrations are associated with an increased risk of all-cause, CVD, and cancer-related mortality.

Neonatal Intensive Care Quality Level-Dependent Variations in the Survival Rate of Infants with a Birth Weight of 500 g or Less in Korea: A Nationwide Cohort Study

Summary: Recent evidence suggests that the survival rate of periviable infants with birth weight (BW) 5.500 g could be improved with better care practices in the neonatal intensive care unit (NICU). This study aimed to investigate the care quality level of NICU-dependent variations in the survival rate of infants with BW <= 500 g. The results showed that providing better NICU quality care practices, including better cardiorespiratory management starting from delivery room resuscitation, can improve the survival rate of infants with BW <= 500 g.

Comprehensive Evaluation of the NeoBase 2 Non- derivatized MSMS Assay and Exploration of Analytes With Significantly Different Concentrations Between Term and Preterm Neonates

Summary: The NeoBase 2 assay was comprehensively evaluated and found to be able to distinguish between transient tyrosinemia of the newborn (TTN) and tyrosinemia type 1 (TYR 1) using succinylacetone (SUAC). Different cutoffs were established for analytes in preterm neonates. Simultaneous measurement of tyrosine (Tyr) and SUAC successfully differentiated between TYR 1 and TTN.

Summary: Congenital CMV infection is a common cause of hearing loss and neurodevelopmental impairment in newborns. A robust LAMP assay was developed for large-scale screening of newborns, providing higher sensitivity and shorter detection time compared to qPCR. The colorimetric LAMP assay can also be used for point-of-care testing in low-resource environments.

Changes to Blood-Sampling Protocol to Reduce the Sampling Amount in Neonatal Intensive Care Units: A Quality Improvement Project

Substantial Lipid Increases During Menopausal Transition in Korean Middle-Aged Women

Summary: This study aimed to investigate lipid changes in Korean middle-aged women during the perimenopausal period. The results showed that lipid levels significantly increased from 3 to 5 years before menopause to 1 year after menopause in all women, regardless of their premenopausal body mass index. However, lipid changes slowed down after 1 year postmenopause, indicating the importance of early intervention strategies for cardiovascular disease prevention in women.

The Multifaceted Clinical Characteristics of Congenital Cytomegalovirus Infection: From Pregnancy to Long-Term Outcomes

Summary: This study aimed to investigate the clinical characteristics of congenital CMV infection from diagnosis to treatment using a multidisciplinary approach. The results showed that prenatal findings have limited ability to detect congenital CMV in Korea. Developmental delay and hearing loss were common long-term outcomes, emphasizing the need for early antiviral treatment and specific diagnostic strategies to reduce the risks of neurologic impairment.

Weight Changes and Unhealthy Weight Control Behaviors Are Associated With Dysmenorrhea in Young Women

Summary: This study aimed to assess the prevalence of dysmenorrhea in young Korean women and investigate the impact of weight changes or unhealthy weight control behaviors on dysmenorrhea. The findings indicate that weight changes (≥3 kg) or unhealthy weight control behaviors are common among young women and may have a negative effect on dysmenorrhea. Therefore, it is important to pay attention to excessive weight changes and unhealthy weight control behaviors to improve dysmenorrhea in young women.

Mesenchymal Stromal Cells Primed by Toll-like Receptors 3 and 4 Enhanced Anti-Inflammatory Effects against LPS-Induced Macrophages via Extracellular Vesicles

Summary: TLR3 and TLR4 priming of hUCB-MSCs can induce an anti-inflammatory phenotype, potentially mediated by extracellular vesicles (EVs). In an LPS-induced lung injury model, TLR3 and TLR4-primed hUCB-MSCs were more effective than naive MSCs in reducing inflammatory cytokines and promoting M2 polarization of macrophages. Moreover, EVs derived from TLR-primed MSCs exhibited comparable anti-inflammatory and immunosuppressive effects to MSC treatment alone.

Major Congenital Anomalies in Korean Livebirths in 2013-2014: Based on the National Health Insurance Database

Summary: This study investigated the prevalence of 67 major congenital anomalies in Korea and compared it with the European data. The results showed that the prevalence of spina bifida, atrial septal defect, congenital megacolon, hip dislocation and/or dysplasia, and skeletal dysplasia in Korea was more than five times higher than in Europe. However, the prevalence of aortic atresia/interrupted aortic arch and gastroschisis in Korea was less than one-fifth of that in Europe. Preterm births accounted for 15.7% of the cases, with more than 40% of infants with anencephaly, annular pancreas, and gastroschisis being born prematurely. Neonatal intensive care unit admission rate was 29.2%, and surgical operation rate was 25.6%. The mortality rate was 1.7%, with 78.2% of the deaths occurring within the first year of life. ASD and ventricular septal defect were the most common anomalies, while trisomy 18 and hypoplastic left heart syndrome were the most fatal anomalies. All infants with aortic atresia/interrupted aortic arch and conjoined twins received surgery.
