A cadaveric study of the structural anatomy of the sternoclavicular joint

Pathologies of the sternoclavicular (SC) joint are infrequent and effective management is often hindered by a limited understanding of the anatomy. In this study, we did macroscopic evaluations of the ligaments, the intra-articular disc, and the articulating surfaces of 25 SC joints. After removal of the joint capsule, the articulating surfaces of the sternal end of clavicle and the sternum were evaluated and the intra-articular disc was macroscopically examined. The anterior SC ligament covered the intra-articular disc, which divided the joint into a clavicular and a sternal part. A thin capsule, relatively lateral and medial from the anterior SC ligament, covered the two intra-articular parts. This means that the anterior SC ligament can be used as a landmark to enter into clavicular or sternal part of the SC joint. Posteriorly, there was a thick capsule without soft-spot or clear posterior SC ligament. Only the antero-inferior surface of the sternal end of every clavicle was covered by cartilage. Of the intra-articular discs 56% were incomplete. All of these incomplete discs displayed a central hole with signs of degeneration and fraying. This was associated with increased cartilage degeneration at the clavicular side. Three-dimensional evaluation of scapular morphology in primary glenohumeral arthritis, rotator cuff arthropathy, and asymptomatic shoulders

Summary: This study evaluated and compared the coracoacromial complex in pathologic shoulders (glenohumeral osteoarthritis [GHOA] and cuff tear arthropathy [CTA]) and nonpathologic shoulders. The results showed significant differences in acromial overhang, acromion-glenoid angle, and acromial height among the different shoulder groups.In conclusion, the acromial part of the complex was turned more posteriorly in both pathologic groups, and the coracoacromial complex was more cranial to the glenoid center in the GHOA group.

Is bone grafting always necessary in revision reverse total shoulder arthroplasty with uncontained glenoid bone defects?

Summary: This study indicates that in patients with uncontained glenoid bone defects undergoing revision to an RTSA, the use of the CPPSP technique can achieve similar clinical outcomes compared to using bone grafts. While the CPPSP technique results in a significant increase in strength, it may lead to different complications that need to be carefully considered.

Summary: Titanium based implants are often associated with infection, making it crucial to find effective strategies to prevent infections. This study investigated nanocomposite coatings with copper nanoparticles deposited on titanium discs for antibacterial activity and biocompatibility. Coatings with higher numbers of copper nanoparticles showed excellent antibacterial activity without harming osteoblast cells, indicating a promising solution for preventing infections in implant surgery.

Does Immobilization Post Injection Reduce Contrast Extravasation in MR Arthrography of the Shoulder?

Flexor tendon repair using a reinforced tubular, medicated electrospun construct

Summary: A reinforced tubular, medicated electrospun construct was developed for deep flexor tendon repair, showing similar strength and anti-inflammatory and anti-adhesion effects as a modified Kessler suture. Different surgical techniques demonstrated varied adhesion patterns postoperatively, but all showed good to excellent repair outcomes histologically, suggesting successful combination of mechanical and biological repair strategies for flexor tendon repair.

Biomechanical Evaluation of a Tubular Braided Construct for Primary Deep Flexor Tendon Surgery

Summary: This ex vivo study demonstrates that the use of a tubular braided construct for repair allows for immediate rehabilitation and improves range of motion after flexor tendon repair. The tubular braided construct withstands higher loads in both static and cyclic tests compared to the control group, providing greater strength.

Scapular morphology of great apes and humans: A three-dimensional computed tomography-based comparative study

Summary: In this study, the three-dimensional morphology of the scapula in humans and great apes was quantified and compared using an image-based methodology originally developed for orthopaedic practice. The results showed significant differences in scapular morphology between humans and great apes, with chimpanzees showing a morphology more similar to humans. These differences may be related to differences in mobility and loading regime of the shoulder.

Acromioclavicular reconstruction techniques after acromioclavicular joint injuries: A systematic review of biomechanical studies

Summary: This systematic review evaluated the biomechanical role of acromioclavicular capsule/ligament reconstruction techniques after an acromioclavicular joint injury. The results showed that reconstruction of the acromioclavicular capsule/ligaments can increase anteroposterior and rotational stability of the shoulder joint, but an optimal reconstruction technique that mimics the native joint is not yet available.

Demographic heterogeneity in valgus angulation of the proximal ulnar canal in relation to the flexion-extension axis

Summary: This study used 3-dimensional computed tomography (CT) reconstructions of the elbow joint of healthy volunteers to determine the demographic variability of the ulna and the relationship between two angles in the frontal plane. The study found a wide range of valgus angulation of the proximal ulnar canal, and the available elbow implant designs do not cover the variability in the population.

Hyaluronic acid as an alternative treatment option for degenerative rotator cuff tears

Summary: This research confirms that hyaluronic acid injections can be used for non-operative treatment of degenerative rotator cuff tears in older people, improving pain and shoulder function. Most patients do not require additional treatment and surgical intervention is rare.

Radial head fractures: a quantitative analysis

Summary: This study evaluated the location of radial head fracture fragments and the influence of Mason type on the size of the fracture fragment. The results showed that the fracture fragments were mainly located in the anteromedial quadrant and that the average fracture fragment size was significantly larger in Mason type II compared to type I fractures.
