Summary: Revisional bariatric surgery is becoming more popular as long-term follow-up studies show a high failure rate of primary surgery. This study aimed to determine the best revisional procedure after failure of primary restrictive surgery. The results showed that biliopancreatic diversion with duodenal switch and single-anastomosis duodenoileal bypass were the most effective procedures in terms of weight loss.

Summary: This study evaluated the cost-effectiveness of medical therapy, sleeve gastrectomy (SG), and Roux-en-Y gastric bypass (RYGB) in patients with severe obesity and type 2 diabetes (T2D). The results showed that RYGB is the most cost-effective treatment strategy, resulting in the highest number of quality-adjusted life-years (QALYs) gained.

Summary: This study aimed to analyze the pathologies on preoperative EGD in patients qualified for bariatric surgeries. The results showed that patients with obesity had a higher prevalence of esophageal pathologies compared to the control group, while the control group was more likely to have duodenal pathologies. Additionally, patient sex was a significant predictor of duodenal pathologies.

Summary: This study examined the outcomes of patients diagnosed with malignancy before and after bariatric surgery in a French multisite cohort. The results showed that patients with a history of malignancy and those diagnosed with postoperative malignancy mainly had breast and gynecologic cancers. It was found that the weight loss outcomes one and two years after surgery were similar in both groups. Furthermore, the study suggests that a history of malignancy should not be considered an absolute contraindication for bariatric surgery.

Summary: Obesity is linked to worsened asthma outcomes, but bariatric surgery has shown to be effective in weight loss and asthma improvement. Compared to nonsurgical weight loss programs, bariatric surgery offers greater weight loss and more consistent improvement in asthma symptoms. However, more research is needed to study the effects of surgery on both mild and severe asthma cases.

Summary: This study aimed to evaluate the association between beta cell function and total body weight loss (TBWL%) 1 to 4 years after bariatric surgery in patients with morbid obesity. The results showed that baseline beta cell function was associated with long-term weight loss, especially after the first year post-surgery. This may be a helpful predictor of weight loss in clinical practice.

Summary: This case-control study analyzed the macrophage infiltration in subcutaneous adipose tissue after bariatric surgery or conservative treatment of obesity, and investigated whether these features could predict weight loss outcome. The study found a significant decline in crown-like structures (CLS) density after surgery, regardless of weight loss. However, surgery had no effect on the infiltration of single-cell macrophages, and the abundance of these macrophage populations before surgery did not predict weight loss or response to surgery.

Summary: Although bariatric surgery leads to significant long-term weight loss, weight gain after surgery affects a considerable percentage of patients. Additionally, some patients experience inadequate weight loss. The effectiveness of anti-obesity medications for post-operative weight gain has not been firmly established yet due to varying study populations and designs. Observational studies consistently show the benefits of medical weight management after bariatric surgery, particularly with liraglutide, topiramate, and phentermine/topiramate, while new anti-obesity medications are expected to be helpful for post-surgical weight optimization.

Summary: This study aimed to investigate the long-term consequences of laparoscopic gastric plication (LGP) in obese patients. The results showed a significant reduction in BMI at 2 years, but a plateau at 3 and 4 years, with a significant BMI increase at 6 years. Weight regain was observed in 35 patients at the 6-year mark, resulting in a high rate of revisions. Predictors for weight regain included disrupted plication fold, increased hunger, and non-adherence to regular exercise.
