Longitudinal Trends in 30-Day Mortality Between Multi-Site and Single-Site Surgeons

Summary: Background: Quality leaders are concerned about the potential negative outcomes of multi-hospital health systems due to fragmented surgical care. However, little empirical data exist on the outcomes of multi-site surgeons. Methods: National Medicare data from 2011 to 2016 were analyzed to compare multi-site and single-site surgeons in terms of mortality rates. Results: The number and percentage of multi-site surgeons decreased over time. Overall, multi-site surgeons had lower 30-day mortality rates than single-site surgeons. The mortality advantage of multi-site surgeons decreased over time, and by 2016, there was no significant difference in outcomes between the two groups. Conclusion: Surgeons operating at multiple hospitals can provide surgical care without increased mortality rates.

Systematic dissection, preservation, and multiomics in whole human and bovine hearts

Summary: Current cardiac biobanking practices are insufficient to capture the diverse array of phenotypes present in cardiovascular diseases (CVDs) and the spatial heterogeneity across cardiac tissue sites. We have developed a rigorous and systematic protocol for the dissection and preservation of human cardiac biospecimens to enhance the availability of whole organ tissue for multiple applications. When combined with longitudinal clinical phenotyping, our protocol will enable multiomics in hearts to deepen our understanding of CVDs.

The STAR-MAMA RCT: Bilingual Mobile Health Coaching for Postpartum Weight Loss

Summary: This study aimed to explore the effectiveness of a postpartum health coaching intervention for reducing postpartum weight gain in Latina populations. The results showed that the intervention did not significantly reduce postpartum weight gain. However, stratified analyses revealed that the intervention had more favorable effects among English speakers and those with higher perceived diabetes risk.

Association of Vascular Health Measures and Physical Function: A Prospective Analysis in the Framingham Heart Study

Summary: This study examined the association between vascular health measures and grip strength and gait speed. The cross-sectional analysis showed a positive association between resting brachial artery diameter and grip strength, as well as between mean arterial pressure (MAP) and grip strength. The longitudinal analysis revealed a significant association between brachial pulse pressure and slower gait speed, but no association with grip strength.

Summary: The study found that clonal hematopoiesis of indeterminate potential (CHIP) is associated with an increased risk of liver inflammation and chronic liver disease progression. Furthermore, experiments showed that mice transplanted with Tet2-deficient hematopoietic cells exhibited more severe liver inflammation and fibrosis. These findings suggest that CHIP contributes to liver inflammation and the development of chronic liver disease through an aberrant inflammatory response.

The Patient Safety Adoption Framework: A Practical Framework to Bridge the Know-Do Gap

Summary: This study aims to develop a framework to improve the adoption and implementation of patient safety initiatives. Through literature review and qualitative interviews, barriers and facilitators to adoption and implementation were identified, and a framework and guidance tool were developed. The framework, consisting of five domains and six subdomains, provides practical guidance for improvement teams. Testing confirmed the framework's acceptability, feasibility, and utility among implementers and subject-matter experts.

Summary: This study investigated the age and sex trajectories, regional differences, and global reference values of arterial stiffness. The results showed that the global age-standardized means of pulse wave velocity were 12.5 m/s for brachial-ankle PWV and 7.45 m/s for carotid-femoral PWV. Males had higher levels than females, and there were regional differences across different countries.

Patterns of tobacco product use and substance misuse among adolescents in the United States

Summary: Among adolescents in the US, sole use of e-cigarettes is the most common pattern, but concurrent use with other tobacco products is not uncommon and may be associated with high-risk behaviors. The study reveals that the prevalence of sole e-cigarette use, dual use, and poly use among youth is 23.2%, 4.2%, and 3.3% respectively, while nonuse of any tobacco product is reported by 62.9% of youth. Furthermore, youth who use tobacco products have higher prevalence of substance misuse compared to non-users.

Inflammatory biomarkers and MRI visible perivascular spaces: The Framingham Heart Study

Summary: This study investigated the relationship between inflammatory biomarkers and magnetic resonance imaging-visible perivascular spaces (PVS) in participants without stroke and dementia. The study found significant associations between certain biomarkers and PVS burden in the basal ganglia and centrum semiovale. The findings suggest that inflammation may play a role in the pathogenesis of cerebral small vessel disease and perivascular drainage dysfunction represented by PVS, with different biomarkers depending on PVS topography.

Summary: A retrospective cohort study was conducted to evaluate changes in the treatment initiation of patients newly diagnosed with atrial fibrillation (AF) after the onset of the COVID-19 pandemic. Data from Optum's de-identified Clinformatics (R) Data Mart Database were used to identify individuals with incident AF. The study found no significant changes in the initiation of oral anticoagulation (OAC), direct oral anticoagulants (DOACs), and rhythm control medications associated with the pandemic, but a significant decrease in the initiation of electrical cardioversion.
