E/e' Ratio Is a Strong Prognostic Predictor of Mortality in Patients With Non-Valvular Atrial Fibrillation With Preserved Left Ventricular Systolic Function

Background: The purpose of this study was to investigate the prognostic values of the E/e' ratio and other echocardiographic and clinical parameters in patients with non-valvular atrial fibrillation (AF) with preserved left ventricular (LV) systolic function. Methods and Results: A total of 488 patients (322 men, age: 66 +/- 11 years) with non-valvular AF with preserved LV systolic function (LV ejection fraction >50%) were included. The E and e' velocities were measured in 5 consecutive heart beats and averaged. Mean follow-up duration after enrollment was 17.7 +/- 5.3 months. All-cause deaths occurred in 45 patients (cardiovascular deaths: n=29). There were significant differences in age (65.6 +/- 11.3 vs. 71.5 +/- 9.1, P<0.001) and hemoglobin concentration (13.6 +/- 2.9 vs. 11.5 +/- 3.4g/dl, P<0.0001) between the deceased group and the survivors. E/e' ratio in the deceased group was significantly higher than that in-the survivors (17.67 +/- 3.39 vs. 10.8 +/- 3.30, P<0.001). Survival analysis showed that a high E/e' ratio (>15.0) represents a poorer prognosis (P<0.001 by Log-Rank test) than an E/e' ratio of 15 and below. Multivariate analysis identified 2 significant variables that were predictive of all-cause deaths: hemoglobin (hazard ratio (HR)=0.806, 95% confidence interval (CI)=0.733-0.886, P<0.0001), and E/e' >15 (HR=3.064, 95%CI=1.38-6.804, P=0.006). Conclusions: E/e' ratio is a useful independent prognostic parameter for predicting mortality in patients with AF with preserved LV systolic function. (Circ J 2011; 75: 2350-2356)

Summary: This study demonstrated that peripheral venous pressure (PVP) could reliably estimate pulmonary artery (PA) pressures during exercise in patients with heart failure with preserved ejection fraction. Compared with the traditional method of estimating right atrial pressure (RAP), the use of PVP improved the ability to identify exercise-induced pulmonary hypertension. This finding has important implications for the diagnosis and treatment of heart failure.

Determinants of Exercise Capacity in Patients With Hypertrophic Cardiomyopathy

Summary: The present study aimed to identify factors that may affect exercise capacity in patients with hypertrophic cardiomyopathy (HCM). The results showed that age, female sex, increased NT-proBNP level, and E/e' ratio on echocardiography were significantly associated with decreased exercise capacity. Monitoring hemodynamic changes and increased filling pressure on echocardiography is important for improved outcomes in this population.

Real-world practice patterns and characteristics of adverse events with selexipag in Korean patients with pulmonary arterial hypertension

Summary: This study aimed to investigate the real-world practice patterns and adverse event (AE) characteristics of selexipag in patients with pulmonary arterial hypertension (PAH). The results showed that AE incidence was higher during the titration phase compared to the maintenance phase, but there was no association between AE incidence and dose distribution. The most common AEs were diarrhea, headache, nausea/vomiting, and myalgia, and they were not dose-dependent in terms of frequency or severity. The recovery rates ranged from 65.0% to 76.9% with a median time of 15-70 days.

Temporal trends in incidence, prevalence, and death of aortic stenosis in Korea: a nationwide population-based study

Summary: This study aims to determine the temporal trends of incidence, prevalence, and death of aortic stenosis (AS) in an East Asian population. The results showed that the proportion of AS patients aged 60 years and above was 80%. The age-standardized incidence rates of AS remained stable, prevalence rates increased, and death rates decreased over the course of a decade. The most common cause of death in AS patients was disease of the circulatory system. Continued efforts are needed to increase awareness of AS-related symptoms and potential complications in the elderly population.

Safety and effectiveness of ambrisentan in real clinical practice in pulmonary arterial hypertension: Results from the Korean post-marketing surveillance

Age and concomitant drug use may affect the occurrence of adverse events.

Identification of two novel COL3A1 variants in patients with vascular Ehlers-Danlos syndrome

Summary: In this study, two patients with vEDS who were not diagnosed by conventional Sanger sequencing were reported. Targeted panel or whole-genome sequencing was performed, and cDNA sequencing was used for diagnosis. The results demonstrated that cDNA sequencing was a useful auxiliary method, especially considering the limited sensitivity of the splicing prediction tool.

Major Clinical Issues in Hypertrophic Cardiomyopathy

Summary: Hypertrophic cardiomyopathy (HCM) is a common inheritable cardiomyopathy with a high disease burden in Korea. Sudden cardiac death, atrial fibrillation and thromboembolic risk, dynamic left ventricular outflow tract obstruction, and heart failure progression are important issues in HCM. Implantable cardioverter-defibrillators and anticoagulation are effective strategies for preventing sudden cardiac death and reducing thrombotic risk. Symptomatic dynamic LVOT obstruction can be improved with negative inotropes and septal reduction therapy. Myocardial fibrosis plays a role in the progression of heart failure in HCM patients.
