Summary: Time to blood culture positivity (TTP) is an indirect measure of bacterial concentration in blood. A short TTP has been linked to infective endocarditis (IE) and poor prognosis in Staphylococcus aureus bacteremia. Factors associated with IE were investigated, and TTP remained associated with IE when adjusted for age, gender, comorbidity, and nosocomial acquisition, with no association between TTP and mortality.

The study found that the linezolid minimum inhibitory concentration and concentration-time curve/MIC were not associated with mortality, while the linezolid dose per body weight, body height, and the interaction between them were independent risks for mortality. In conclusion, the clinical outcomes of linezolid treatment for vancomycin-resistant Enterococcus bloodstream infection are associated with the dose and body height.

Summary: The aim of this study was to investigate the modulation of the inflammatory response after out-of-hospital cardiac arrest (OHCA) by blocking IL-6-mediated signalling with tocilizumab and relate the induced changes to clinical status, myocardial, and brain injury. The results showed that tocilizumab treatment significantly affected IL-5, IL-6, IL-17, neutrophil, and monocyte counts, as well as the Vasoactive-Inotropic Score (VIS). Tocilizumab increased IL-5 and IL-6 levels, while lowering IL-17 levels. Neutrophils and monocytes were lower at 24 and 48 hours, and VIS was lower at 24 hours in the tocilizumab group compared to the placebo group.

Summary: Neutrophil gelatinase-associated lipocalin (NGAL) is an inflammatory biomarker associated with acute kidney injury (AKI). This study aimed to evaluate the prognostic value of NGAL in predicting AKI and mortality in patients with ST-elevation myocardial infarction (STEMI). The results showed that higher levels of NGAL on admission were independently associated with a higher risk of severe AKI and 30-day all-cause mortality. Assessment of NGAL should be delayed for optimal prognostic purposes.

Summary: This study explored the potential of valsartan to modify disease development in subclinical HCM and characterized the short-term phenotypic progression. The results showed that valsartan did not have a significant effect on cardiac remodeling and the progression of subclinical HCM was minimal. Nearly one-fifth of the patients developed clinically overt HCM, and the transition to disease was associated with baseline interventricular septum thickness and left atrial volume index. These findings emphasize the importance of longitudinal follow-up of sarcomere variant carriers and the need to improve understanding of factors that drive disease penetrance and progression.
