Summary: Total aortic arch replacement using the frozen elephant trunk (FET) technique carries significantly higher perioperative morbidity and mortality risks in septuagenarians compared to younger patients. Long-term survival is notably compromised in older patients. Thorough patient selection and optimization of perioperative management are recommended to improve outcomes.

Summary: Early postoperative intraluminal thrombosis is a frequent complication after FET surgery. Smaller stent graft sizes, larger or partially covered aneurysms, and major bleeding are associated with early thrombosis. Slight FET oversizing, prompt thoracic endovascular aortic repair completion, and early reintervention for major bleeding may prevent early thrombosis.

Summary: The study evaluated the outcomes of aortic arch replacement using the frozen elephant trunk technique in patients who had previous proximal and/or distal open or endovascular thoracic aortic repair. The results showed low mortality and morbidity rates, but postoperative stroke remained a concern. After successful procedures, the approach provided an ideal platform for subsequent surgical or endovascular downstream aortic interventions that are often necessary.

Summary: This study investigated the clinical outcomes of elephant trunk stent fenestration in ATAAD patients, showing that this technique can reduce surgical difficulty and lead to a high postoperative survival rate.

Summary: The aim of this study is to report and identify risk factors for distal aortic failure following aortic arch replacement via the frozen elephant trunk (FET) procedure. The study found that acute aortic dissection, preoperative maximum descending aortic diameter, and preoperative maximum aortic diameter at the level of the diaphragm are risk factors for distal aortic failure.

Summary: The aim of this study was to compare the outcomes of conventional hemi-arch repair (CET) and frozen elephant trunk (FET) techniques in terms of mortality rates, re-intervention rates, and volumetric changes in aortas. The results showed that FET led to a significant increase in true lumen volume, while false lumen volume remained stable. In contrast, after the CET procedure, significant enlargement of the false lumen was observed, leading to a higher re-intervention rate.

Summary: En bloc arch reconstruction with FET technique for acute type A aortic dissection appeared to be feasible and effective based on the early clinical follow-up results, with a hospital mortality rate of 9.8% and various postoperative complications observed. Long-term follow-up studies with a larger sample size are needed to further evaluate the efficacy of this approach.

Summary: This study developed a simple and effective nomogram model to predict the 30-day mortality rate in acute type A aortic dissection patients undergoing total aortic arch replacement with frozen elephant trunk technique. By collecting clinical data, the study identified certain predictors that independently predicted the 30-day mortality rate, and the nomogram demonstrated good calibration and discrimination.

Summary: MiTAR is a simplified approach to TAR that reduces surgical trauma while achieving aortic reshaping effects. Compared to TAR, MiTAR patients had shorter times of cardiopulmonary bypass, aortic clamp, and hypothermic circulation arrest, as well as significantly lower volumes of intraoperative transfusions. The 30-day mortality rate was lower in MiTAR patients than in those receiving TAR.

Summary: The use of aortic balloon occlusion (ABO) technique in total aortic arch replacement (TAR) surgery with frozen elephant trunk (FET) can reduce circulatory arrest (CA) time and lower postoperative complication rates, resulting in improved recovery outcomes for patients with higher operative risks.

Summary: This article describes a case of an octogenarian patient with ATAAD who underwent total arch replacement combined with SET implantation. The conclusion is that acceptable outcomes can be achieved by performing emergent surgery on ATAAD octogenarians without serious preoperative complications.

Summary: The aortic balloon occlusion (ABO) technique significantly shortens the circulatory arrest time in total arch replacement (TAR) and frozen elephant trunk (FET) procedures for acute type A aortic dissection. Clinical data suggest a protective effect on liver and kidney function, but further large-sample studies are needed for thorough evaluation.

Summary: This article reports a case of a patient with right aortic arch and aberrant left subclavian artery who underwent urgent surgery for type A aortic dissection. Surgical management included the use of the frozen elephant trunk technique and left subclavian artery reimplantation. The patient experienced complications postoperatively and required reoperation.

Summary: The treatment of aortic arch aneurysms and dissections is highly complex and carries significant risks. This meta-analysis compared two surgical techniques, the elephant trunk (ET) technique and the frozen elephant trunk (FET) technique. The results showed that ET had higher early mortality but lower incidence of spinal cord injury compared to FET. However, recent studies did not find significant differences between ET and FET in terms of mortality or spinal cord injury.
