Radiation-induced brain tumours after central nervous system irradiation in childhood: a review

Objects Radiation-induced cerebral tumours constitute a significant risk for subjects undergoing radiotherapy for the management of cerebral neoplasms. Age-related cerebral vulnerability could be a specific factor in the genesis of these complications. Methods The pertinent literature of both paediatric and adult series has been reviewed. Three personal cases were added. Results One hundred forty-two paediatric second brain tumours were evaluated. Out of them, 69 were malignant gliomas, 33 meningiomas, 8 sarcomatous lesions and 13 low-grade astrocytomas. The average latency period for the appearance of the second tumour was 8 years. Among the second tumours occurring in adults, meningioma is the most common. In this subgroup, the latency period ranged between 16 and 30 years. Conclusion Paediatric radiation-induced brain tumours differ from the adult counterpart for both the histological subtypes. These figures indicate a specific vulnerability of the infantile brain demonstrated by the most frequent occurrence of highly malignant lesions.

Pediatric Brain Tumors: Signatures from the Intact Proteome

Summary: The study aimed to explore the intact proteome of pediatric brain tumors of different WHO grades and localizations to understand the molecular mechanisms underlying the onset and progression of these pathologies. Proteomic analysis revealed statistically significant differences in protein/peptide levels between tumor histotypes, as well as the identification of naturally occurring fragments and post-translational modifications. This study contributes to elucidating the molecular profiles of the most common solid tumors affecting children.

Atypical Teratoid Rhabdoid Tumor: Proposal of a Diagnostic Pathway Based on Clinical Features and Neuroimaging Findings

Summary: This study aims to assess the main imaging and clinical features in adult- and pediatric-onset atypical teratoid rhabdoid tumor (ATRT) and build a predefined pathway for diagnosis. Clinical evaluation showed that ATRT was more common in children under 3 years old, with infratentorial localization reported more frequently in children under 24 months. Imaging findings revealed that ATRT had a preferential location near the ventricles and liquor spaces, with eccentric cysts as a hallmark feature. Histopathological analysis confirmed the diagnosis using specific immunohistochemical markers. The diagnosis of ATRT is more challenging in adults and relies solely on neuropathological examination.

Enhancing the Reliability of Intraoperative Ultrasound in Pediatric Space-Occupying Brain Lesions

Summary: Intraoperative ultrasound (IOUS) is a helpful tool for the resection of space-occupying brain lesions, although technical limits may affect its reliability. By using IOUS with careful assessment of technical limits and implementing strategies to enhance real-time imaging, accurate localization and evaluation of resection extent can be achieved. These strategies include the use of hyperechoic markers, contrast administration, and specific techniques to overcome the limitations.
