Summary: This study evaluated changes in carotid plaque calcification over a 2-year follow-up in patients with symptomatic carotid artery disease. The results showed both increases and decreases in the volume of extra- and intracranial carotid artery calcification, which were significantly correlated with baseline calcification volume. Factors such as diabetes and use of antihypertensive drugs were also associated with changes in calcification.

Summary: This study demonstrates that circulating IL-6 levels predict carotid plaque severity, vulnerability, and progression. A cutoff of 2.0 pg/mL for IL-6 levels could facilitate the selection of individuals that would benefit from anti-IL-6 drugs for stroke prevention.

Summary: This study found an association between persistent asthma and carotid plaque burden, as well as higher levels of inflammatory biomarkers in participants with persistent asthma. The association between carotid plaque and asthma subtype was not attenuated after adjusting for baseline inflammatory biomarkers, indicating that the increased risk of atherosclerotic cardiovascular disease among those with persistent asthma may be multifactorial.

Summary: This study found that calcification and inflammation coexist in human carotid plaques, with different degrees of inflammation associated with different levels of calcification. By combining both imaging modalities and considering plaque phenotype, high-risk unstable plaques with little or no calcification could be accurately identified for diagnostic benefit.

Summary: The study suggests that innate immunity is linked to larger carotid atherosclerotic plaques, while adaptive immunity seems to be associated with smaller plaques and a lower frequency of intraplaque hemorrhage. This imbalance in innate and adaptive immunity may play a role in the vulnerability of carotid atherosclerotic plaques.

Summary: This study identified novel correlations between non-invasive imaging biomarkers of inflammation-related plaque metabolism and morphological MRI markers of plaque instability. If replicated, these findings may support the use of combined MRI and PET to detect vulnerable plaque in future clinical practice and randomized trials.

Summary: This study aimed to determine criteria for carotid atherosclerotic plaque instability using advanced ultrasound technology, immunohistochemical analysis, and EPR. The results showed that plaque area, volume index, and specific black area were significant predictors of unstable plaques, with increased inflammatory and neovasculogenesis markers in such plaques. EPR data analysis indicated that divalent manganese could serve as a marker for plaque instability. Overall, additional ultrasound criteria, along with histological studies, can be essential in identifying patients at risk of ischemic stroke.

Summary: Coronary artery disease (CAD) is the leading cause of death in individuals >20 years old in the Western world. It plays a major role in sudden cardiac death (SCD), accounting for 50-75% of cases. Plaque rupture, plaque erosion, and calcified nodules are the main forms of coronary thrombosis in SCD. The study found that nearly 50% of erosion cases had no calcification, while almost 50% of stable plaques showed severe calcification. Identifying individuals at risk of atherosclerosis can help reduce the incidence of SCD, but noninvasive tools can only detect a portion of complicating coronary lesions.

Summary: The study enrolled 317 patients with severe carotid atherosclerosis stenosis and found that there was no significant difference in plaque characteristics between smokers and non-smokers, but smoking was positively correlated with plaque calcification. Ultrasonography was efficient in diagnosing plaque morphology and calcification, with promising clinical significance for current smoking patients.

Summary: This study identifies diabetes mellitus, chronic kidney disease, lipid-rich plaque, and macrophages as independent predictors for rapid progression of coronary calcification. The rapid progression of calcification is associated with a reduction of inflammatory features, suggesting a relationship between baseline vascular inflammation and the progression of calcification.

Summary: Carotid atherosclerosis is a major cause of ischemic strokes. Early identification of vulnerable carotid plaques is crucial for determining high-risk patients for stroke, and the potential role of serum biomarkers in this identification process is discussed in this review.
