Familial hemiplegic migraine in pediatric patients: A genetic, clinical, and follow-up study

Summary: The aim of this study was to describe a cohort of pediatric patients with genetically confirmed familial hemiplegic migraine (FHM). The knowledge of genotype-phenotype correlations may suggest prognostic factors associated with severe phenotypes. The study data show that most of our patients with early-onset FHM experienced infrequent and non-severe attacks, which improved over time.

Deciphering in silico the Role of Mutated NaV1.1 Sodium Channels in Enhancing Trigeminal Nociception in Familial Hemiplegic Migraine Type 3

Summary: Familial hemiplegic migraine type 3 (FHM3) is caused by gain-of-function mutations in the SCN1A gene, resulting in a dysfunction of Na(V)1.1 sodium channels. Computer modeling revealed that FHM3-mutated Na(V)1.1 channels contribute to abnormal nociceptive signaling in trigeminal pain mechanisms. Mutations leading to loss of Na(V)1.1 function reduce firing of trigeminal nerve fibers, while combined activation of P2X3 and 5-HT3 receptors in mutants results in prolonged and high-frequency spiking activity.

Epilepsy in patients with familial hemiplegic migraine

Summary: This study systematically reviewed the coexistence of epilepsy in patients with familial hemiplegic migraine, finding that mutations in all three or possibly four FHM genes can cause epilepsy, with the highest number of cases associated with epilepsy belonging to the ATP1A2 mutation. Drug-resistant forms of epilepsy are rare in these patients.

CACNA1A-p.Thr501Met mutation associated with familial hemiplegic migraine: a family report

Summary: Hemiplegic migraine is a rare form of migraine that can be sporadic or familial. A mutation in the CACNA1A gene, specifically the p.Thr501Met variant, was found in 12 out of 15 patients in a family, leading to familial hemiplegic migraine. The mutation can prevalently occur as hemiplegic migraine without significant cerebellar involvement.

Changes in Plasma Lipid Levels Following Cortical Spreading Depolarization in a Transgenic Mouse Model of Familial Hemiplegic Migraine

Summary: Metabolite levels in peripheral body fluids may be associated with attack features in migraine patients, indicating the potential of plasma metabolites as disease biomarkers. Lipid metabolic pathways were found to be affected by CSD, with transient increase in PGD2 and elevated levels of anti-inflammatory lipid mediators in wild-type mice after CSD. Monitoring peripheral changes in lipids could provide insights into central brain mechanisms related to migraine pathophysiology.

A novel CACNA1A R2201W variant in a woman with hemiplegic migraine

Summary: This article reports a patient with familial hemiplegic migraine type 1, whose clinical history is consistent with hemiplegic migraine, and genetic testing revealed a variant in the CACNA1A gene. MRI showed extensive leukoencephalopathy, suggestive of small vessel disease, progressing over the years. Exome sequencing identified a heterozygous variant c.6601C>T (p.Arg2201Trp) in the CACNA1A gene.

Novel Presentation of Hemiplegic Migraine in a Patient With Cockayne Syndrome

This report may expand the phenotype of Cockayne syndrome.

A case report of atypical hemiplegic migraine with nonheadache onset in a Chinese child

Gene detection is crucial for early diagnosis and treatment in similar clinical cases.

Target prediction and pathway analysis suggested their involvement in RCVS pathogenesis, which was confirmed in vitro. MiR-130a-3p was associated with disruption of the blood-brain barrier and increased permeability in a human BBB model.

Severe migraine and its control: A proposal for definitions and consequences for care

Summary: Many severe migraine patients do not receive appropriate treatment, while specialist headache centres often see patients who could be managed adequately in the community. The absence of standardized definitions for migraine severity and control, as well as a treatment algorithm for difficult-to-treat patients, may contribute to this issue. Proposed definitions and treatment algorithms have been suggested to address this gap and enhance the management of severe migraine.

Type of headache at onset and risk for complications in reversible cerebral vasoconstriction syndrome

Summary: Patients with reversible cerebral vasoconstriction syndrome (RCVS) who do not present with thunderclap headache (TCH) at onset have a higher risk for neurological complications, including cervical artery dissections, posterior reversible encephalopathy syndrome, seizures, and subarachnoid hemorrhage, compared to those with TCH. However, the functional outcome at 3 months is similar in both groups, with most patients having a good prognosis. Further multicentric studies are needed to confirm these findings.

Summary: This study evaluated the frequency, distribution, and clinical associations of the dilated appearance of cerebral cortical veins, termed cortical veins sign, on T2*-weighted gradient recalled-echo (T2*-GRE) in the acute setting of migraine with aura attack in adult patients. The results showed that the cortical veins sign was commonly observed in patients with migraine with aura and could be detected reliably. This imaging marker may assist clinicians in identifying the diagnosis of migraine with aura.

Effectiveness of onabotulinumtoxinA (BOTOX (R)) for the preventive treatment of chronic migraine: A meta-analysis on 10 years of real-world data
