Characterisation of the Insulinotropic Activity of an Aqueous Extract of Gymnema Sylvestre in Mouse beta-Cells and Human Islets of Langerhans

Leaves of the Gymnema sylvestre (GS) plant have been used to treat diabetes mellitus for millennia, but the previously documented insulin secretagogue effects of GS extracts in vitro may be non-physiological through damage to the beta-cells. We have now examined the effects of a novel GS extract (termed OSA) on insulin secretion from the MIN6 beta-cell line and isolated human islets of Langerhans. Insulin secretion from MIN6 cells was stimulated by OSA in a concentration-dependent manner, with low concentrations (0.06-0.25mg/ml) having no deleterious effects on MIN6 cell viability, while higher concentrations (>= 0.5mg/ml) caused increased Trypan blue uptake. OSA increased beta-cell Ca2+ levels, an effect that was mediated by Ca2+ influx through voltage-operated calcium channels. OSA also reversibly stimulated insulin secretion from isolated human islets and its insulin secretagogue effects in MIN6 cells and human islets were partially dependent on the presence of extracellular Ca2+. These data indicate that low concentrations of the GS isolate OSA stimulate insulin secretion in vitro, at least in part as a consequence of Ca2+ influx, without compromising beta-cell viability. Identification of the component of the OSA extract that stimulates regulated insulin exocytosis, and further investigation of its mode(s) of action, may provide promising lead targets for Type 2 diabetes therapy. Summary: This study evaluated the role of serum progesterone sulfates in the etiology of gestational diabetes mellitus (GDM). The results showed that concentrations of epiallopregnanolone sulfate (PM5S) were reduced in GDM and in women with higher fasting plasma glucose. PM5S increased glucose-stimulated insulin secretion (GSIS) and this effect was mediated by TRPM3. These findings highlight the importance of progesterone sulfates in the pathogenesis of GDM.

Summary: This study compared the expression of GPCR mRNA in human islets obtained from donors with normal weight range and those classified as obese. The altered expression of GPCRs in obesity may have an impact on islet function, and the study also considered the possible effects of adipokines on GPCRs showing altered expression in obesity.

Summary: Members of the aGPCR subfamily, such as GPR56, play important roles in metabolic processes and may serve as potential targets for type 2 diabetes therapy. This study revealed that GPR56 undergoes constitutive internalization in cells and can respond to its ligands when necessary, leading to protection against cell apoptosis. Activation of GPR56 by P7 was shown to increase intracellular calcium levels and insulin secretion in a GPR56-dependent manner, suggesting a potential therapeutic target for maintaining cell mass in type 2 diabetes.

Summary: Under conditions of obesity, the expression of non-olfactory GPCR mRNAs in human islets showed significant alterations, with upregulation of G alpha s- and G alpha q-coupled receptors. This opens up possibilities for exploring novel therapeutic candidates for regulating glucose homeostasis through potentiation of insulin secretion and/or beta-cell mass.

Additionally, high-physiologic triglyceride levels can increase insulin secretion.
