Pulmonary Artery Stents: Long-Term Follow-Up

Objectives: Determine the long-term outcomes of branch pulmonary artery (PA) stents. Background: PA stents in congenital heart disease effectively relieve stenoses in the short-term. Published long-term data are limited. Methods: Patients enrolled in an FDA IDE protocol from 1989-92 were included. Clinical follow-up and catheterization data were evaluated. Patients were included if >5 year follow-up data was available or if mortality occurred following the initial procedure. Results: There were five deaths: four due to progression of their underlying heart disease, and one from a complication during a follow-up catheterization. Clinical data for 43 surviving patients demonstrated 39 patients (91%) are in NYHA class I or II. Seven patients underwent surgical intervention during the follow-up period (five RV-PA conduit, two Fontan revisions), but none addressed PA stenosis. Final repeat catheterizations were performed in 36 patients (55 stents) 7.2 +/- 4.3 years post stent insertion with 1.2 +/- 0.9 further procedures with stent dilations. In this subgroup, the minimum vessel diameter increased from 4.7 +/- 1.8 to 13.4 +/- 2.4 mm (P < 0.001), and the pressure gradient improved from 41 +/- 25 to 9 +/- 11 mm Hg (P < 0.001). Higher initial gradient and smaller balloons were associated with a final stent diameter of <14 mm (P = 0.030 and 0.046). Jailed vessels occurred in 49% of stents with abnormal angiographic flow in 18/55. Six repeat catheterizations resulted in complications, including the one procedural death. Conclusion: Stents implants for PA stenoses provide effective improvement in vessel caliber in the long-term. Although repeat interventions are necessary, this procedure reduces RV pressure and provides an important alternative to surgery for residual PA obstruction. (C) 2010 Wiley-Liss, Inc.

Intentional bioprosthetic tricuspid valve fracture to facilitate transcatheter valve-in-valve deployment

Summary: Transcatheter valve-in-valve implantation can reduce the need for surgical reinterventions in some patients with congenital heart disease, but may lead to patient-prosthesis mismatch in others. Fracturing the bioprosthetic valve ring to optimize hemodynamics during implantation of a transcatheter valve could be an effective strategy.

Percutaneous Common Carotid Artery Access for Cardiac Interventions in Infants Does Not Acutely Change Cerebral Perfusion

Summary: This study investigated the effects of pediatric cardiac interventions via percutaneous common carotid artery (CCA) access on cerebral perfusion using cerebral near-infrared spectroscopy (NIRS). The results showed that no significant acute change in cerebral perfusion was observed with CCA access during the procedure, indicating that percutaneous CCA access did not decrease cerebral perfusion on the side of access.

Summary: Recurrent laryngeal nerve injury is a rare complication of left pulmonary artery stent placement or patent ductus arteriosus device closure. However, the incidence is highest in patients undergoing both procedures simultaneously. Further research is needed to determine contributing factors that may reduce the risk of recurrent laryngeal nerve injury.

Shortening of Palmaz Genesis XD stents by longitudinal compression in pediatric patients with pulmonary vein stenosis: Bench-testing and case series

Summary: The Palmaz Genesis XD stents are ideal for stenting vessels in pediatric patients due to their ability to be re-dilated to large diameters, but their length limits their utility for pulmonary vein stenting in small children. A stent shortening technique by longitudinally compressing them prior to deployment may enhance their applicability in pediatric pulmonary vein stenosis.

Summary: In this case report, the authors successfully managed a case of CHAOS using repeated fetoscopy. Selective Valve Removal for Melody Valve Endocarditis: Practice Variations in a Multicenter Experience

Summary: Management guidelines for Melody transcatheter pulmonary valve (TPV) infective endocarditis (IE) are currently lacking. A multicenter retrospective analysis of patients with Melody TPV from 10/2010 to 3/2019 found that 44% of patients with Melody IE were successfully treated medically without valve explantation or recurrence. The degree of valve stenosis at the time of IE diagnosis was strongly associated with the need for valve explantation, and explantation rates varied significantly among different institutions.

Summary: This study compared the hemodynamic stability after closing patent ductus arteriosus (PDA) in extremely premature infants using surgical ligation (SL) or cardiac catheter-based closure (CCC). The SL group had smaller and less mature infants with higher respiratory support. However, there was no significant difference in postoperative hemodynamic decompensation between the two groups.

Our evolution in the treatment of hepatic artery and portal vein thrombosis in pediatric liver transplantation: Success with catheter-directed therapies

Summary: A retrospective cohort analysis of 27 pediatric liver transplant recipients who experienced hepatic artery thrombosis (n=13), portal vein thrombosis (n=9), or both (n=5) showed a 100% success rate in catheter-directed recanalization in re-establishing blood flow to the graft. Compared to surgical recanalization or anticoagulation alone, catheter-directed recanalization not only increased patency but also decreased mortality in these patients.
