Vascular walls change their dimension and mechanical properties in response to injury such as balloon angioplasty and endovascular stent implantation. Placement of bare metal stents induces neointimal proliferation/restenosis which progresses through different phases of repair with time involving a cascade of cellular reactions. These phases just like wound healing comprise distinct steps consisting of thrombosis, inflammation, proliferation, and migration followed by remodelling. It is noteworthy that animals show a rapid progression of healing after stent deployment compared with man. During stenting, endothelial cells are partially to completely destroyed or crushed along with medial wall injury and stretching promoting activation of platelets, and thrombus formation accompanied by inflammatory reaction. Macrophages and platelets play a central role through the release of cytokines and growth factors that induce vascular smooth muscle cell accumulation within the intima. Smooth muscle cells undergo complex phenotypic changes including migration and proliferation from the media towards the intima, and transition from a contractile to a synthetic phenotype; the molecular mechanisms responsible for this change are highlighted in this review. Since studies in animals and man show that smooth muscle cells play a dominant role in restenosis, drugs like rapamycin and paclitaxel have been coated on stent with polymers to allow local slow release of drugs, which have resulted in dramatic reduction of restenosis that was once the Achilles' heel of interventional cardiologists.

Summary: Endothelial cells play a crucial role in cardiovascular health, and dysfunction can be caused by cardiovascular risk factors but restored once stimuli are removed. There are various methods to measure endothelial function, with each having its pros and cons. It is recommended to adopt a consensus methodology like flow-mediated dilatation and establish reference values for different populations. Future studies should explore the integration of endothelial function measurements with patient-specific epigenetic data and other biomarkers.

Summary: The study shows that the GLP-1 receptor agonist liraglutide can prevent the accumulation of C16 and C24 ceramides/sphingomyelins species, alleviate inflammation, and inhibit the initiation of fibrosis in the liver of MCD-diet-fed mice. Additionally, liraglutide influences the composition of gut microbiota induced by the MCD-diet.

Summary: The advent of single-cell biology has opened up new possibilities for understanding human biological processes and diagnosing, monitoring, and treating diseases, including cardiovascular disease (CVD). By analyzing CVD samples at the single-cell level, new cell communities that play a role in disease development can be identified, leading to new therapeutic strategies. This review focuses on the single-cell analysis of atherosclerotic plaques, highlighting the current understanding of cellular subpopulations and their heterogeneity and plasticity in relation to clinically relevant features. The clinical impact of single-cell technologies in CVD patient care is emphasized, calling for multidisciplinary and international collaborative efforts.

Atorvastatin-loaded spray-dried PLGA microparticles for local prevention of intimal hyperplasia: Drug release rate optimization and activity on synthetic vascular smooth muscle cells

Summary: This study aimed to encapsulate a high payload of atorvastatin (ATV) to prevent intimal hyperplasia (IH) in vascular bypass grafts and evaluate its impact on synthetic smooth muscle cells (SMCs) involved in IH progression. Poly(lactic-co-glycolic acid) microparticles (MPs) were formulated with different drug loadings and tested for ATV release, sterilization, and effects on SMCs. The results showed sustained ATV release from 10% and 20% ATV-loaded MPs for 90 days. The ATV-loaded MPs reversed the synthetic phenotype of SMCs and attenuated their migratory and proliferative capacities.
