Depletion of zebrafish essential and regulatory myosin light chains reduces cardiac function through distinct mechanisms

Aims Mutations in the essential myosin light chain (ELC) and regulatory myosin light chain (RLC) genes have been linked to sarcomeric hypertrophic cardiomyopathies in humans; however, the specific functions of the different myosin light chains during cardiogenesis in a vertebrate animal are not well understood. Methods and results Using zebrafish (Danio rerio) as a model organism, we have identified cmlc1 and cmlc2 as the main ELC and RLC orthologues, respectively, and have furthermore characterized their functions during cardiogenesis by morpholino technology. Depletion of either cmlc1 or cmlc2 using morpholino-modified antisense oligonucleotides leads to a disruption in sarcomere structure and compromises cardiac function as well, although through seemingly distinct mechanisms. While myosin still assembles into a novel rod-like structure in both morphants, the sarcomere length is longer in cmlc1 morphants than that in wild-type embryos, whereas it is shorter in cmlc2 morphants. In addition, cardiomyocyte size and number are increased upon depletion of cmlc1, resulting in a larger ventricular chamber volume; in contrast, depletion of cmlc2 leads to a reduction in cardiomyocyte size and number. Conclusion Our data have elucidated distinct roles for cmlc1 and cmlc2 during zebrafish cardiogenesis, suggesting that cardiomyopathies resulting from human mutations in ELCs vs. RLCs may have distinct pathological characteristics during disease progression.

Continuous transcutaneous carbon-dioxide monitoring to avoid hypercapnia in complex catheter ablations under conscious sedation

Summary: Continuous transcutaneous carbon-dioxide partial pressure (tpCO(2)) monitoring during conscious sedation in complex electrophysiological catheter ablation procedures is feasible and accurate, showing good correlation with arterial and venous blood gas analysis. This monitoring method may contribute to additional safety by detecting hypercapnia earlier and more likely than traditional half-hourly blood gas measurements.

Safety of conscious sedation in electroanatomical mapping procedures and cryoballoon pulmonary vein isolation

Summary: The study compared the respiratory and hemodynamic safety endpoints of cryoballoon PVI and EAM procedures, finding that despite longer procedure duration and deeper sedation requirement in EAM procedures, conscious sedation appears to be as safe as cryoballoon ablation procedures in terms of hemodynamic and respiratory safety endpoints.

Cryo-balloon catheter localization in X-Ray fluoroscopy using U-net

Catheter ablation for atrial fibrillation in HFpEF patients-A propensity-score-matched analysis

Summary: This study compared outcomes of HFpEF patients with AF who underwent CA versus received medical therapy, finding that CA group had significantly lower arrhythmia recurrence, reduced heart failure hospitalization and death, and improved clinical and echocardiographic parameters of HFpEF. Reassessment at follow-up showed resolution of HFpEF in a higher percentage of patients in CA group compared to medical therapy group.

Summary: Pulmonary vein isolation using cryoballoon technology is an effective treatment for atrial fibrillation. However, one of the most common complications is phrenic nerve injury. This study aimed to assess the incidence, characteristics, and recovery rate of phrenic nerve injury during cryoballoon-based pulmonary vein isolation. The results showed that the majority of patients experienced recovery within 12 months.

Trends in Transcatheter Edge-to-Edge Mitral Valve Repair Over a Decade: Data From the MiTra ULM Registry

Summary: This study aimed to investigate the potential changes in baseline characteristics of symptomatic severe mitral regurgitation patients who underwent transcatheter edge-to-edge mitral valve repair (M-TEER) over the past decade in a high-volume center. The results showed that patients undergoing M-TEER nowadays have lower surgical risk and are treated earlier before significant left ventricular (LV) remodeling occurs. The experience gained in the procedure has led to an increase in the number of patients with complex degenerative pathology being treated.

Summary: CB-based PVI is an effective treatment for AF, but the most common complication is PNI. Our study found a high rate of reconnection during repeat procedures. These findings can help identify the optimal dosing protocol in CB-based PVI procedures.

Summary: This study compares the characteristics and outcomes of functional mitral regurgitation (FMR) patients treated with transcatheter edge-to-edge repair (TEER) or surgical mitral valve repair (SMVr). The results show that despite higher age and comorbidity burden, TEER is associated with significantly lower in-hospital mortality and comparable functional and mitral regurgitation improvements compared to SMVr.

MRI-based training model for left atrial appendage closure

Summary: This study proposes an MRI-based training model for percutaneous closure of the left atrial appendage (LAA). Patient-specific silicone models of the LAA were generated using MRI data, allowing for assessment of the influence of the transseptal puncture (TSP) site on the access to patient-specific LAA shapes. The model demonstrated its usability through simulation of venous access and correct handling of the deployment catheter.

Summary: The study aimed to evaluate the procedural efficacy of a single 2-minute freeze for pulmonary vein isolation in atrial fibrillation patients. Results showed that using TTI-guided ablation protocol did not show significant differences in overall reconnection rates compared to a fixed ablation protocol, but the reconnection rate in the left inferior pulmonary veins was significantly lower in patients treated with a fixed protocol.

Substrate-based ablation of atypical atrial flutter in patients with atrial cardiomyopathy

Summary: The study evaluated the efficacy of a substrate-based ablation (SBA) strategy compared to a standard anatomically based ablation (ABA) strategy for atypical AFL ablation. The results showed that SBA was as effective as ABA in the acute termination of AFL, but had less recurrence of AFL in patients with atrial cardiomyopathy in the long term.
