Dilated cardiomyopathy due to hypocalcaemic rickets: is it always a reversible condition?

Nutritional rickets is still occasionally found in high-income countries, especially in populations at risk, and induced hypocalcaemia is a rare but possible cause of dilated cardiomyopathy. Although rare, physicians need to consider nutritional rickets in the differential diagnosis of hypocalcaemia cardiac failure, especially in high-risk populations such as immigrants. Despite being a reversible condition, the prognosis depends on the severity and time of diagnosis. We report two cases of exclusively breastfed infants with congestive cardiac failure due to hypokinetic dilated cardiomyopathy who had completely different outcomes. This report supports the need for prevention of this deficiency and underlies the role of vitamin D supplementation.

Myocarditis and coronary aneurysms in a child with acute respiratory syndrome coronavirus 2

Summary: A 6-year-old African boy with multi-viral infection, including parvovirus B19 and severe acute respiratory syndrome coronavirus 2, presented with persistent fever, respiratory distress, and myocarditis leading to cardiogenic shock. Treatment with a combination of medications led to normalization of cardiac function and coronary diameters, with cardiac MRI showing mild myocardial interstitial edema indicative of cardiac stunning related to cytokine storm rather than direct viral injury.

Cancer survivors require personalized clinical attention due to the risk of long-term side effects of the treatments. Antineoplastic drugs, including anthracyclines and radiotherapy, can have adverse effects on the cardiovascular system. Nutritional changes during treatment and dietary habits in survivors can also impact their health. Nutrition may play a role in preventing cardiac damage, and practical advice can help improve the dietary habits of survivors and their families.

Summary: Multisystem inflammatory syndrome (MIS-C) is a rare condition associated with COVID-19 in children, characterized by severe systemic inflammation and nonspecific symptoms. The reduction in MIS-C cases and severity is likely influenced by factors such as vaccination, spread of different SARS-CoV-2 variants, and changes in human immunological response. Multiple factors, including immunological memory, mutations in S protein, and the immune response elicited by vaccination, play a role in this change.

Summary: This study investigated endothelial injury in Kawasaki Disease (KD), Multisystem Inflammatory Syndrome in Children (MIS-C), and acute COVID-19 (AC19). The results showed that AC19 and KD patients had higher levels of circulating endothelial cells (CECs) than MIS-C patients during the acute stage. Cellular clusters/syncytia were more common in KD patients. Further larger studies are needed to validate these findings.
