Summary: This case report presents the first reported case of surgery in a pregnant woman treated with dual antiplatelet therapy without discontinuation of ticagrelor. Despite insufficient platelet inhibition of ticagrelor and moderate inhibition of aspirin, a caesarean section was performed for the patient due to the need for emergency surgery, with local hemostatic measures to prevent excessive bleeding.

Summary: The aim of this study was to identify factors associated with in-hospital adverse events in stable patients with STEMI who received pPCI. The results showed that only 1 in 20 stable STEMI patients developed an adverse event after receiving pPCI. Moderate to severe chronic kidney disease was identified as a predictor of future adverse events.

Summary: The study aimed to compare mortality in nonagenarians undergoing primary percutaneous coronary intervention (PPCI) versus medical management for myocardial infarction. The findings suggest that nonagenarians who underwent PPCI had lower mortality rates compared to those managed medically, indicating PPCI may be beneficial in this age group.

Summary: The study found that NSTE-ACS patients with MVD undergoing MV-PCI and CO-PCI had similar mortality rates at long-term follow-up, but the MV-PCI group showed improved event-free survival in terms of fewer coronary reinterventions.

Summary: Studies have shown a clear association between coronary chronic total occlusion (CTO) and adverse clinical outcomes, especially in patients with acute myocardial infarction (AMI). However, the prognostic impact of CTO-percutaneous coronary intervention (PCI) during AMI remains controversial due to the higher risk of complications compared to non-occlusive coronary lesions.

Summary: An analysis of National Health Insurance data in Korea showed that pre-existing depression does not have a significant impact on mortality in patients with established coronary artery disease undergoing PCI. However, younger patients with angina and depression may have a higher risk of all-cause mortality.

Summary: This article reports a case series of seven patients with cardio-cerebral infarction who underwent primary percutaneous coronary intervention. All patients presented with stroke-related symptoms and thrombus aspiration was the main reperfusion strategy. However, only two patients were diagnosed with coronary embolism, and the rest required stenting for severe atherosclerotic lesions. Overall, cardio-cerebral infarction is a rare but fatal condition that requires further investigation for optimal treatment strategies.

Summary: The study found a weak correlation between LVEDP and LUS in ST-segment-elevation myocardial infarction patients undergoing primary percutaneous coronary intervention. In multivariable analysis, LUS was significantly associated with in-hospital major adverse cardiovascular events, new cardiogenic shock, and in-hospital mortality, while LVEDP showed no significant association with these outcomes.

Summary: This retrospective study aimed to establish a clinical prognostic nomogram for predicting in-hospital mortality after PCI in patients with STEMI. Factors such as age, body mass index, systolic pressure on admission, and various laboratory values were found to be independently associated with in-hospital mortality. The nomogram showed good discrimination and prediction accuracy, suggesting its potential usefulness in clinical practice.

Summary: This study reveals that there is an inverse relationship between the volume of primary, elective, and total PCI performed in hospitals and in-hospital mortality in patients with acute MI undergoing primary PCI. Additionally, a lower ratio of primary to total PCI volume is associated with higher in-hospital mortality, suggesting it as an indicator of quality of care for acute MI.

Summary: In this study, 400 patients with STEMI were analyzed to compare the 2-year clinical outcomes of BP-DES and DP-DES in PPCI. The results showed that BP-DES were associated with lower incidence of MACE and ST within 2 years of follow-up. Subgroup analysis revealed that BP-DES were also linked to lower cardiac deaths compared to DP-DES.

Summary: The study found that 95.5% of patients undergoing their first PCI had at least one risk factor for coronary artery disease. Nearly half of the patients had three or more risk factors. The average age difference between men and women narrowed as the number of risk factors increased, and smoking and obesity were associated with earlier age of diagnosis.

Summary: This study compares the outcomes of women and men with acute myocardial infarction (AMI) who underwent percutaneous coronary intervention (PCI) or coronary artery bypass graft surgery (CABG). It found that women had worse in-hospital and long-term outcomes, especially in the PCI group. Women who underwent PCI were more likely to be admitted for heart failure during follow-up.

Summary: The study aimed to assess whether the increase in troponin I (TnI) levels after exercise stress test (EST) could help identify patients with obstructive coronary artery disease (CAD) and myocardial ischemia in those suspected of having stable angina. Results showed that TnI levels increased after EST, but this increase was largely independent of the results of coronary angiography and EST, regardless of the presence of obstructive CAD or not, and whether the EST results were positive or negative.

Summary: This study found that in type 2 diabetes patients, diastolic dysfunction is associated with impairment of endothelium-independent dilatation, but not with endothelium-dependent dilatation; cardiac autonomic dysfunction may be present in patients with more severe diastolic dysfunction. Therefore, further investigation is needed to explore the role of cardiac autonomic dysfunction in diastolic dysfunction in larger populations.

Summary: Numerous studies have shown that abnormalities in coronary microcirculation may be the cause of acute and chronic cardiac ischemic syndromes. The microvascular origin of myocardial ischemia and angina is considered in clinical practice when patients have normal or near-normal coronary arteries during angiography. This article reviews the diagnostic approach for patients suspected of having coronary microvascular dysfunction as the cause of ischemic syndromes and proposes a classification for chronic and acute microvascular coronary ischemic syndrome, including myocardial infarction with normal coronary arteries.

Summary: Atrial fibrillation is a common cardiac arrhythmia that often leads to ischemic strokes. Oral anticoagulation is the standard strategy for stroke prevention, but some patients cannot tolerate this therapy due to an increased risk of bleeding. A significant number of atrial fibrillation patients also have moderate-severe mitral regurgitation, making them unsuitable candidates for cardiac surgery. Therefore, new therapeutic approaches such as Left Atrial Appendage Occlusion and MitraClip procedure are gaining interest. However, the selection criteria for patients who may benefit from these combined percutaneous therapies, as well as their long-term risks and benefits, have yet to be determined. Future studies are needed to evaluate the efficacy and safety of combined MitraClip and LAA occlusion.

Summary: Heart failure with preserved ejection fraction (HFpEF) is a common clinical syndrome, and its incidence is increasing due to an ageing population and the increasing incidence of various diseases. The complexity of HFpEF has made it difficult to identify specific therapies, although some antidiabetic drugs have shown positive effects. There is limited evidence regarding the potential benefits of combined therapy with SGLT2-Is and GLP-1 RAs in HFpEF patients.

Summary: Severe tricuspid valve regurgitation is associated with adverse outcomes, but surgery has high mortality and morbidity rates. Transcatheter tricuspid valve interventions (TTVI) offer promise in high-risk patients. Tricuspid transcatheter edge-to-edge repair is a frequently used TTVI option.

Summary: This retrospective study examined cardiovascular risk factors and specific coronary findings in high-risk immigrant groups from Eastern Europe, the Middle East and North Africa, and South Asia. The results showed variations in cardiovascular disease among different ethnicities, with South Asians being particularly high-risk and other groups having lower cardiovascular risk factors. This study supports the role of genetic influence in these different ethnic communities.
