Locoregional radiological treatment for hepatocellular carcinoma; Which, when and how?

Hepatocellular carcinoma (HCC) is one of the most frequent and deadliest cancers worldwide. Liver transplantation, surgical resection or local ablation offer the best survival advantages but most patients either present when the tumor is in an advanced stage or the degree of underlying liver disease precludes these options. Several therapies have been proposed for these patients with proven survival benefits. These therapies comprise the locoregional treatment for HCC, and include percutaneous ethanol injection (PEI), radiofrequency ablation (RFA), transarterial chemoembolization (TACE), transarterial radioembolization (TARE), and drug-eluting bead (DEB). PEI and RFA are considered curative treatments for early stage HCC; whereas TACE is a standard of care for intermediate stages. Additionally, evaluation of response to locoregional treatment in HCC is important, as objective response may become a surrogate marker for improved survival. Currently, there are several criteria for response assessment, including the World Health Organization (WHO), the Response Evaluation Criteria in Solid Tumors (RECIST), the European Association for the Study of the Liver Criteria (EASL), and the modified RECIST (mRECIST); however, there has been poor correlation between the clinical benefit provided by locoregional interventional therapies and conventional methods of response assessment. The aim of our study was to review and analyze the current evidence for radiological interventions in HCC, and to propose evidence based recommendations to improve the management of these patients. Summary: Recurrence of autoimmune hepatitis (AIH) is common after liver transplantation, and it is associated with younger age at transplantation, the use of mycophenolate mofetil post-transplant, sex mismatch, and high pre-transplant IgG levels. Recurrent AIH is associated with impaired graft and overall survival. Better characterization, prevention, and treatment strategies are needed for the management of recurrent AIH.

The study also highlights the need for further clinical trials to define the role of SIRT in managing HCC among Asian populations.

Summary: In the largest ever study of LSM in patients with PBC, LSM assessed by VCTE improves prediction of survival beyond biochemical response, established prognostic scores, and age categories, independent of time. LSM can reasonably be regarded as a useful prognostic marker and a potential surrogate endpoint in PBC clinical trials.

Summary: Recent advances have shown that body mass index is an inaccurate tool for assessing nutritional status, while body composition may serve as a non-invasive prognostic tool for patients with hepatocellular carcinoma. However, due to the heterogeneity of data, lack of prospective studies, absence of a standardized assessment method, and interpopulation variation, body composition assessment is not currently part of the standard pre-therapeutic workup.

Prognostic Significance of Severe Vitamin D Deficiency in Patients with Primary Sclerosing Cholangitis

Summary: This study aimed to investigate the association between severe vitamin D deficiency and disease progression, hepatobiliary malignancies, liver-related mortality, and the need for liver transplantation in primary sclerosing cholangitis (PSC). The results showed that severe vitamin D deficiency was related to an increased risk of hepatobiliary malignancies, and a severe deficiency at diagnosis and persistent deficiencies over time were independently associated with poor clinical liver outcomes.

Through iterative feedback cycles and online surveys, consensus statements were developed in the areas of multidisciplinary treatment, intra-arterial therapy, biologics, radiation therapy, surgical resection and transplantation, and percutaneous ablative therapy. These recommendations reflect an expansion in treatment options, increased therapeutic flexibility, and a shift towards strategies with curative intent.
