Summary: This study compared the long-term therapeutic outcomes of drug-eluting bead transarterial chemoembolization (DEB-TACE) with radiofrequency ablation (RFA) for the initial treatment of a single small hepatocellular carcinoma (HCC). The results showed that RFA had superior local tumor progression and progression-free survival rates compared to DEB-TACE, but there was no significant difference in overall survival rates. Therefore, DEB-TACE may be considered as an efficient substitute for RFA in some patients with a single small HCC who are ineligible for RFA.

Summary: This study compared the occurrence of adverse events (AEs) among radiofrequency ablation (RFA), conventional transarterial chemoembolization (cTACE), and drug-eluting bead TACE (DEB-TACE) in treating HCC patients. It found that cTACE resulted in the highest incidence of AEs, with male gender, bronchial asthma, and disease duration being independent predictive factors for total AEs risk.

Summary: This study investigated the effectiveness and safety of combining sorafenib and drug-eluting bead transarterial chemoembolization (DEB-TACE) in the treatment of early intrahepatic stage-progressed advanced hepatocellular carcinoma (ISPA-HCC). The results showed that the combination therapy significantly improved tumor response rates, overall survival, and time to progression compared to sorafenib monotherapy. However, the DTS group had a higher incidence of adverse events during treatment.

Summary: The study discussed the effects of RFA followed by TACE and vice versa on objective response, overall survival, local recurrence, and tumor-free survival in the treatment of hepatic malignant tumors. Results showed that RFA/TACE demonstrated effective complete response, downstaging, disease-free survival, and overall survival.

Summary: This study compared the efficacy and safety of TACE-P and TACE-R in the treatment of patients with eiHCC. Using the propensity score matching method, it was found that the overall survival and progression-free survival were similar between the TACE-P group and the TACE-R group. Subgroup analysis showed that patients with a single tumor had worse tumor response when treated with TACE-P. There were no significant differences in adverse events between the two groups.

Summary: This study evaluated the impact of drug-eluting bead transarterial chemoembolization (DEB-TACE) and conventional transarterial chemoembolization (cTACE) on hepatic fibrosis in hepatocellular carcinoma (HCC) patients. The results showed that DEB-TACE attenuates the progression of hepatic fibrosis and has comparable tolerance to cTACE in treating intermediate- or advanced-stage HCC.

Summary: Unfortunately, some HCC patients are not eligible for curative treatments, making locoregional treatments like TACE crucial. Systemic therapy has advanced in the past decade, with combination atezolizumab and bevacizumab now the standard for advanced disease. Combination therapies with TACE, including systemic therapies, locoregional therapies, and immunotherapies, show promise for the management of intermediate stage HCC.

Summary: Recurrence of autoimmune hepatitis (AIH) is common after liver transplantation, and it is associated with younger age at transplantation, the use of mycophenolate mofetil post-transplant, sex mismatch, and high pre-transplant IgG levels. Recurrent AIH is associated with impaired graft and overall survival. Better characterization, prevention, and treatment strategies are needed for the management of recurrent AIH.

However, due to the heterogeneity of data, lack of prospective studies, absence of a standardized assessment method, and interpopulation variation, body composition assessment is not currently part of the standard pre-therapeutic workup.

Summary: This study aimed to investigate the association between severe vitamin D deficiency and disease progression, hepatobiliary malignancies, liver-related mortality, and the need for liver transplantation in primary sclerosing cholangitis (PSC). The results showed that severe vitamin D deficiency was related to an increased risk of hepatobiliary malignancies, and a severe deficiency at diagnosis and persistent deficiencies over time were independently associated with poor clinical liver outcomes.

Summary: This article summarizes the consensus recommendations from Canadian experts on the management of intermediate-stage hepatocellular carcinoma (HCC). Through iterative feedback cycles and online surveys, consensus statements were developed in the areas of multidisciplinary treatment, intra-arterial therapy, biologics, radiation therapy, surgical resection and transplantation, and percutaneous ablative therapy. These recommendations reflect an expansion in treatment options, increased therapeutic flexibility, and a shift towards strategies with curative intent.

Summary: The outcomes of liver transplantation have improved over the years, but long-term survival rates are still lower than the general population. The liver has unique immunological functions and can modulate the recipient's immune system to promote tolerance. Immunosuppressive drugs should be individualized to control alloreactivity while minimizing toxicities.
