Correlates and determinants of nuclear epidermal growth factor receptor content in an oropharyngeal cancer tissue microarray

Background: We have previously reported nuclear localization of epidermal growth factor receptor (EGFR) protein in oropharyngeal cancer tissue. Nuclear EGFR levels were inversely correlated with survival and response to radiotherapy. Here, we sought to identify the determinants and correlates of nuclear EGFR content. Methods: We analyzed an oropharyngeal cancer tissue microarray for the expression of the key molecules of the EGFR signaling cascade using an automated image analysis technique (AQUA) scored on a scale of 0 to 255, which permits protein quantitation and subcellular localization. Patients with oropharyngeal squamous cell cancer treated with radiotherapy or surgery and radiotherapy were eligible. Data were analyzed using Spearman correlations and multiple linear regression with robust SEs. Results: Of the 95 tumors included in this study, 72 (75%) had sufficient tissue for analysis of nuclear EGFR. Nuclear EGFR levels were associated with membranous/cytoplasmic EGFR levels (p = 0.82, P < 0.001), nuclear extracellular signal-regulated kinase-2 (P = 0.30, P = 0.01), and nuclear proliferating cell nuclear antigen (PCNA; p = 0.36, P = 0.003). Nuclear phosphorylated-Akt, cyclin D1, phosphatase and tensin homolog (mutated in multiple cancers 1) (PTEN), p53, and proliferation marker Ki-67 levels did not correlate with nuclear EGFR level. In multivariable analysis, only PCNA retained its significant association (P = 0.01). Conclusions: These results are consistent with preclinical data showing that EGFR may function as a tyrosine kinase in the nucleus, phosphorylating and stabilizing PCNA. The nuclear activity of EGFR may constitute a novel therapeutic target.

Summary: This study evaluated the concordance and interrater reliability of scoring HER2 IHC in breast cancer biopsies by 18 breast cancer-specialized pathologists. The results showed poor concordance within the intermediate categories and for the IHC 0 cases. The use of a 3-category system partially reduced the discordance. The study also revealed the optimal number of raters for determining HER2 IHC scores of 0 and 3+.

Timing of Unplanned Reoperation After Lower Extremity Free Flap Reconstruction What Are the Roles of Procedure Indication and Defect Etiology?

Summary: This study aimed to understand the temporal pattern and risk factors associated with lower extremity free flap failure. It found a reoperation rate of 14.5% within the first 30 postoperative days, with patients with vascular indications having the highest rate. The rate of reoperation was highest during the first 2 days postoperative and significantly decreased afterwards. African American race, malignant tumors, prosthetic/implant complications, and wound/infectious indications were significant predictors for reoperation.

Eliminating children's tobacco smoke exposure: a pathway to bioverified abstinence among low-income maternal smokers in the Babies Living Safe and Smokefree (BLiSS) trial

Summary: This study aimed to examine the effectiveness of the BLiSS multilevel intervention in promoting smoking cessation among low-income maternal smokers and its impact on smoking relapse rates over a 12-month period. The results showed that mothers who received the intervention were more likely to eliminate their children's tobacco smoke exposure within three months, which was significantly associated with long-term smoking abstinence.

Benefits versus drawbacks of delaying surgery due to additional consultations in older patients with breast cancer

Summary: Additional evaluations and second opinions before breast cancer surgery may improve care, but could cause detrimental delays. This study investigates the timing of surgical delays associated with survival benefits and potential harm. It found that quick new patient visits have a protective association with breast cancer mortality, but substantial delays may increase mortality in older patients. Similarly, delays in medical oncologist and surgeon visits can also have negative impacts on outcomes.

Summary: This study examined the relationship between neighborhood social composition, gentrification, and acculturation stress among first-generation Chinese immigrants. The findings showed that Chinese immigrants living in neighborhoods with a higher proportion of Chinese immigrants and higher wealth experienced lower acculturation stress, while gentrification had no impact.

Patient-Caregiver Portal System in Palliative Oncology: Assessment of Usability and Perceived Benefit
