Binding and inhibition of drug transport proteins by heparin A potential drug transporter modulator capable of reducing multidrug resistance in human cancer cells

A major problem in cancer treatment is the development of resistance to chemotherapeutic agents, multidrug resistance (MDR), associated with increased activity of transmembrane drug transporter proteins which impair cytotoxic treatment by rapidly removing the drugs from the targeted cells. Previously, it has been shown that heparin treatment of cancer patients undergoing chemotherapy increases survival. In order to determine whether heparin is capable reducing MDR and increasing the potency of chemotherapeutic drugs, the cytoxicity of a number of agents toward four cancer cell lines (a human enriched breast cancer stem cell line, two human breast cancer cell lines, MCF-7 and MDA-MB-231, and a human lung cancer cell line A549) was tested in the presence or absence of heparin. Results demonstrated that heparin increased the cytotoxicity of a range of chemotherapeutic agents. This effect was associated with the ability of heparin to bind to several of the drug transport proteins of the ABC and non ABC transporter systems. Among the ABC system, heparin treatment caused significant inhibition of the ATPase activity of ABCG2 and ABCC1, and of the efflux function observed as enhanced intracellular accumulation of specific substrates. Doxorubicin cytoxicity, which was enhanced by heparin treatment of MCF-7 cells, was found to be under the control of one of the major non-ABC transporter proteins, lung resistance protein (LRP). LRP was also shown to be a heparin-binding protein. These findings indicate that heparin has a potential role in the clinic as a drug transporter modulator to reduce multidrug resistance in cancer patients.

Evaluation of COVID-19 fear and quality of life in patients with haematopoietic stem cell transplantation during the COVID-19 pandemic

Summary: The objective of this study was to determine the fear level of COVID-19 in hemopoietic stem cell transplant patients and its relationship with quality of life. The Fear of COVID-19 Scale (FCV-19S) was used to evaluate the fear situation, while the European Organisation for Quality of Life Research and Treatment Core Questionnaire (EORTC QLQ-C30) was used to assess the quality of life. The results showed that the fear level was higher in urban residents compared to rural residents, and the strongest positive correlation between fear level and life quality was found in emotional function. Furthermore, the FCV-19S was proven to be a quick and valid tool for determining the fear level in vulnerable patient groups such as HSCT patients.

Summary: This study retrospectively evaluated 40 patients who were admitted to the ICU during their post-transplant period. The analysis included baseline patient characteristics, reasons for ICU admissions, laboratory and clinical findings, supportive treatment in ICU, and short-term survival. The results showed a 8.8% ICU admission rate and a 75% mortality rate among the ICU patients. Factors such as mechanical ventilation, vasopressor use, heart rate, and elevated INR were significantly associated with survival in the ICU.

Summary: Through analyzing the data of 71 patients with relapsed/refractory diffuse large B-cell lymphoma (DLBCL) treated with Polatuzumab vedotin (Pola) with bendamustine and rituximab (BR) from March 2018 to April 2021, it was found that Pola-BR is a promising option with an overall response rate of 47.9% and a complete response rate of 32.4% after a median of 3 cycles. The median overall survival was 5 months with manageable hematological toxicities of grade 3-4 neutropenia and thrombocytopenia.

Summary: The safety and efficacy of mesenchymal stromal cell (MSC) therapy for steroid-refractory late acute graft versus host disease (late aGvHD) was evaluated and compared to acute GvHD (aGvHD) in a multicenter study. It was found that patients with late aGvHD had a higher response rate, lower non-relapse mortality (NRM), and improved survival probability at the 2-year follow-up, indicating the benefits of MSC treatment for late aGvHD.
