Summary: Providing surgical margin information during breast cancer surgery is crucial for success, with no tumor ideally left within 1-2 mm from the surgical specimen surface. A novel multimodal optical imaging instrument combining confocal microscopy and optical coherence tomography has been developed to assess potential positive margins. The preliminary evaluation shows high correlation between optical image analysis and histological findings.

Summary: This study investigates the association between magnetic resonance imaging (MRI) and surgical management in patients with early-stage breast cancer. The results show that patients who received preoperative MRI had a doubled rate of mastectomy as the first surgery compared to those without MRI. Additionally, patients who received MRI were typically younger and had denser breasts.

Summary: This study aimed to evaluate surgical margins in breast-conserving surgery using metabolomics analysis. The results showed that a 1 mm negative margin is sufficient for surgery, and identified six metabolites as potential biomarkers for a negative margin.

Summary: This study aimed to assess predictors of acute postoperative complications for breast cancer patients and develop a predictive model that calculates complication probability using patient risk factors. The models built showed good performance in predicting various types of complications with high accuracy and calibration measures. The risk calculator developed has the potential to help stratify patients with higher complication risks and improve decision-making process expectations.

Summary: The decision of surgical management in breast cancer patients treated with neoadjuvant chemotherapy (NAC) is significantly influenced by tumor biology. This study found that breast conservation is a safe alternative to mastectomy in NAC setting, with improved overall survival and comparable disease-free survival and locoregional control.

Summary: The presence of AH at the surgical margins of BCS in patients who received NAC does not appear to increase the risk of ipsilateral breast cancer. Therefore, there is no need for surgeons to routinely perform additional re-excision. However, selective re-excision should be considered in certain cases, particularly in patients with HER2 overexpression.

Summary: The debate over optimal margins for DCIS in breast surgery continues, with a significant difference in positive margins and reoperation rates between breast-conserving surgery and mastectomy. Older patients have a lower reoperation rate after positive side margins in breast-conserving surgery. Individual assessment rather than strict adherence to guidelines influenced the decision on reoperation.

Summary: The short-term outcomes of rectum-preservation for locally advanced rectal cancer patients achieving major clinical response or complete clinical response after neoadjuvant therapy were reported in the study. Transanal local excision for patients achieving cCR or mCR is safe, with an increased cCR rate seen when there is a 12-week interval from chemoradiotherapy completion to LE. Performing LE after achieving cCR reduces the risk of ypT >.

Summary: Pancreatic ductal adenocarcinoma (PDAC) has a poor clinical outlook, and a better understanding of the tumor immune microenvironment is needed. In this study, T cells were found to be the predominant immune cell subset within PDAC tumors. The presence of exhausted TRM cells expressing PD-1 and TIGIT suggests that dual checkpoint receptor blockade could be a promising avenue for future immunotherapy.

The majority of the analyzed PDX models rely on oxidative phosphorylation metabolism, either alone or in combination with glucose metabolism. This metabolic classification of existing collections of PDX by the mini panel will be useful for pre-clinical testing of metabolism-targeting drugs.

Summary: The present study aimed to evaluate the worry content, depression, anxiety, and post-traumatic stress symptoms of breast cancer patients and define the role of worry trait and intolerance of uncertainty. The results showed that levels of worry trait and intolerance of uncertainty were associated with maintaining high levels of depression, anxiety, and post-traumatic disorders. Therefore, early assessment of these factors is critical in identifying patients at a higher psychopathological risk.

Summary: Pancreatic ductal adenocarcinoma has poor clinical outcome and limited response to immunotherapy. Stromal fibroblasts play a dominant role in the tumor microenvironment and exhibit high transcriptional heterogeneity influenced by tumor proximity. Targeting tumor-proximal fibroblast subsets, along with HIF-1 alpha inhibition and immune stimulation, may offer a multimodal therapeutic approach for this disease.
