Summary: This study aimed to investigate the association between polypharmacy and all-cause mortality in the very old. The results showed that each additional medication prescribed was associated with a 3% increased risk of mortality. Amongst the very old, the risks and benefits of each additional medication prescribed should be carefully considered.

Summary: This study assessed the prevalence, genetic background, comorbidities, and features of cognitive decline in primary age-related tauopathy (PART) in an unselected elderly population. The findings revealed that PART is common among the elderly and exhibits distinct patterns of cognitive decline and genetic and neuropathological features compared to Alzheimer's disease.

Summary: This study investigated the clinical variables associated with handgrip strength and its change over a 4-year period in an oldest old cohort from the Leiden 85-plus Study. It found that body height, weight, and functional performance were positively associated with handgrip strength, and that men experienced a greater decline in handgrip strength compared to women.

Summary: This study investigated the association between plasma ANGPTL2 levels and all-cause and cancer mortality in a community-dwelling sample of younger-old adults. The results showed that elevated plasma ANGPTL2 levels were associated with a higher risk of all-cause and cancer mortality, especially in participants with frailties or with lifestyles of heavier drinking or current smoking.

Summary: The study found that complex multimorbidity is common among people aged 85 and above, but hyperpolypharmacy is not. Preventative medicines are commonly prescribed in all categories of polypharmacy, indicating diverse prescribing patterns. There are significant gender and selected socioeconomic differences in prescribing, suggesting the need for better support in managing preventative medications.

Summary: This study investigated the association between ideal cardiovascular health (CVH) scores and all-cause mortality among Chinese elderly. The results showed a linear relationship between CVH scores and the risk of all-cause mortality, with a 7.5% lower risk for each unit increase in CVH scores.

Summary: Elderly men with prostate cancer have a lower mortality rate compared to younger men, as they have a higher risk of dying from other causes. For low-grade cancer patients, the annual risk of dying from cancer increases continuously and peaks in men aged 85 and older.

Summary: Limited evidence exists on the predictive validity of absolute cardiovascular disease (CVD) risk in predicting disability and mortality in old age. This study of over 7000 community-dwelling individuals aged 65 and older found that higher levels of absolute CVD risk were associated with increased risks of disability and mortality, suggesting that estimated CVD risk levels may serve as surrogate markers for these outcomes even in old age.

Summary: This study examines the relationship between sickness benefits and life expectancy at age 65 in fifteen affluent countries from 1960 to 2015. The results suggest that countries with higher benefit coverage experience larger increases in life expectancy, with mixed effects of income replacement depending on the population coverage. The institutional interplay between coverage and income replacement supports the beneficial effects of universal programs on population health.

The study found that the perceived working mechanisms of psychosomatic therapy include the continuous alternation of psychosocial conversations and body-oriented exercises to create awareness of the interaction between body and mind, and the therapeutic alliance and finding common ground between patient and therapist.

Summary: This study found that patients with knee osteoarthritis (KOA) in the Netherlands are only moderately satisfied with the quality of care received, and there are substantial gaps between perceived quality of care for OA and internationally accepted standards. Most respondents expressed a desire for change in the care for KOA, with recommendations focusing on the need for tailored care, more education, and increased empathy and support from healthcare providers.

The study found that besides individual context, factors such as history of cardiovascular disease, complexity of health problems, side effects, and life expectancy also influence the appropriateness of CPM.

Summary: This study compared the efficacy of intramuscular (IM) glucocorticoid injection and intra-articular (IA) glucocorticoid injection in reducing knee pain for patients with knee osteoarthritis. The results showed that at 4 weeks, IM glucocorticoid injection was less effective in reducing pain compared to IA injection, but at 8 and 24 weeks, IM injection was noninferior to IA injection. There were no significant differences in all secondary outcomes between the two injection methods.

Summary: This study investigated the associations between systolic blood pressure (SBP) and health outcomes in very old individuals, taking into account factors such as blood pressure-lowering treatment, history of cardiovascular diseases, cognitive and physical fitness. The results showed that higher SBP was associated with lower all-cause mortality, but this association was not evident in fit octogenarians. SBP was not consistently associated with cognitive and functional decline.

Summary: The loss of skeletal muscle function, strength, and mass is common in older adults, and it has significant socioeconomic impact. Subclinical hypothyroidism, which is prevalent in older adults, has been associated with reduced muscle strength. However, there hasn't been a randomized placebo-controlled trial to investigate the effect of treating subclinical hypothyroidism on muscle function and mass.

Summary: Suboptimal diet is a major risk factor in cardiovascular disease. Various stakeholders, including governments, individuals, educational institutes, healthcare facilities and the industry, should work together to improve dietary habits. The authors suggest explicitly including healthcare facilities as a setting for policy suggestions in the guidelines for cardiovascular disease prevention.

Summary: This meta-analysis of data from 21 cohorts across six continents found that women have a higher risk of developing dementia than men, especially in low- and lower-middle-income economies. Longer education and former alcohol use are stronger risk factors for dementia in men than in women, while other risk factors have similar effects in both genders.
