A potential role of Escherichia coli pathobionts in the pathogenesis of pediatric inflammatory bowel disease

Through genomic analysis of mucosa-associated Escherichia coli strains, we found a close genetic association among isolates from pediatric inflammatory bowel disease (IBD) patients. A specific E. coli pathovar, adherent-invasive E. coli (AIEC), was found in Crohn's disease (CD) adult patients - this pathovar has enhanced adhesive and invasive properties, mainly due to the mannose-bonding FimH protein. We aimed to characterize 52 mucosa-associated E. coli strains isolated from pediatric IBD and non-IBD patients. Eleven E. coli strains, showing a strong similarity in fimH gene sequence to that of E. coli AIEC LF82, were characterized for fimH gene sequence, genomic profiling, adhesive and invasive ability, and phylogrouping. The results were compared with E. coli strains AIEC LF82 and MG1655. The 11 E. coli isolates showed 82.4% +/- 1.4% fimH sequence similarity and 80.6% +/- 1.3% genomic similarity to strain AIEC LF82. All these strains harbored V27A and S78N FimH mutations, as found in LF82. Nine of them belonged to the more virulent B2 and D phylogroups. Neuraminidase treatment, mimicking inflamed mucosa, enhanced adhesion of all 11 strains by 3.5-fold, but none showed invasion ability. It could be argued that the 11 selected strains could be a branch of an E. coli subpopulation (pathobionts), that could take advantage in an inflamed context because of a suitable genomic and (or) genetic backdrop.

Summary: The study identified an ileal immune profile associated with the prognosis of colon cancer and responses to chemotherapy, showing upregulation of the ileal immune transcriptome in poor-prognosis patients with proximal colon cancer. Conversely, oxaliplatin-based chemotherapy could restore a favorable, attenuated ileal immune fingerprint in responders. These results suggest that chemotherapy inversely shapes the immune profile of the ileum-tumor axis, influencing clinical outcome.

Summary: Limited experimental evidence suggests a link between nutrition and cancer immunosurveillance. This study demonstrates that ketogenic diet induces an anti-tumor effect through 3HB-mediated T cell-mediated cancer immunosurveillance. 3HB prevents immune checkpoint inhibition and promotes the expansion of specific T cells, while KD also leads to compositional changes in gut microbiota.

The study suggests that profiling the reproductive tract microbiome offers new perspectives on human reproduction.

Summary: The study showed that tumor-associated immune and genomic biomarkers of response to combined immune checkpoint blockade (CICB) in melanoma patients were similar to those of monotherapy, while toxicity from CICB was associated with a more diverse peripheral T-cell repertoire. Additionally, analysis of gut microbiota revealed a higher abundance of Bacteroides intestinalis in patients experiencing toxicity, along with an upregulation of the inflammatory factor IL-1 beta in colitis samples.

Summary: In addition to PD-L1 expression, biomarkers for predicting the response to immune checkpoint inhibitors (ICIs) in non-small-cell lung cancer (NSCLC) are needed. This study found that fecal Akkermansia muciniphila (Akk) was associated with clinical benefit of ICI treatment in NSCLC patients, independent of PD-L1 expression and other factors. The relative abundance of Akk in the gut microbiome may serve as a potential biomarker for patient stratification.

Split-dose versus day-before regimen of sodium picosulfate plus magnesium citrate for bowel cleansing before colonoscopy in children: Randomized controlled trial

Summary: This study aimed to assess the superiority of split-dose regimen of SPMC compared with SPMC day-before regimen for bowel cleansing before colonoscopy in children. The results showed that the rate of successful cleansing level was significantly higher in the split-dose group. The BBPS scores were significantly higher in the split-dose group for the whole colon, right colon, and transverse colon. Patient acceptability was better in the split-dose group and the percentage of children needing nasogastric tube placement was lower.

Gastrointestinal Involvement in Children with Systemic Lupus Erythematosus

Summary: Systemic lupus erythematosus (SLE) is a systemic autoimmune disorder. When it presents before the age of 18 years (childhood-onset systemic lupus erythematosus, cSLE), the disease course tends to be more severe with a higher rate of organ involvement and requires an early diagnosis. Gastrointestinal involvement in cSLE is rare and scarcely reported in the literature. This manuscript provides a narrative review of gastrointestinal manifestations in cSLE, focusing on hepatic, pancreatic, and intestinal involvement, based on a comprehensive literature search using the PubMed database.

Migraine, Allergy, and Histamine: Is There a Link?

Summary: The relationship between migraines and allergies is still not well understood, although there is an epidemiological link between them. Genetic and biological factors contribute to both migraines and allergic disorders. The histaminergic system, particularly the H3 and H4 receptors, may play a role in the pathophysiology of both conditions. Antihistamine drugs could potentially be helpful in treating migraines and allergic disorders. Further research is needed to fully understand the mechanistic link between these two common and debilitating conditions.
