Post-thrombotic syndrome (PTS) is the most common complication of deep vein thrombosis (DVT). Current preventative strategies are limited to the daily wear of graduated compression stockings (GCS). The aim of this study was to evaluate early predictors of PTS. One hundred and twenty-two consecutive patients with a first DVT were prospectively recruited from diagnosis and followed for up to 6months post-end of anticoagulation. D-dimer was measured in 107 participants at presentation and Villalta scale was evaluated in 70 participants at a median of 2weeks following diagnosis. PTS developed in 51 center dot 6% of participants. GCS were obtained by 78 center dot 1% of participants, with 33 center dot 7% reporting daily wear at the end of follow-up. Mean early Villalta scale was significantly higher in those with PTS (8 center dot 1 +/- 3 center dot 7) compared to those without (2 center dot 6 +/- 2 center dot 7, P<0 center dot 001). Median D-dimer was significantly higher in those with PTS [3260ng/ml, interquartile range (IQR) 8208000ng/ml] compared to those without (1540ng/ml, IQR 8102520ng/ml, P<0 center dot 001). The adjusted odds ratio for every one point increase in early Villalta scale was 1 center dot 78 [95% confidence interval (CI), 1 center dot 192 center dot 64; P=0 center dot 005] and for D-dimer >1910ng/ml it was 2 center dot 71 (95% CI, 1 center dot 057 center dot 03; P=0 center dot 04). These markers could enable targeted counselling regarding GCS for those at high risk of PTS.

Preventing VTE following total hip and knee arthroplasty: Is prediction the future?

Banne Nemeth, Rob Nelissen, Roopen Arya, Suzanne Cannegieter

Summary: VTE is a common complication following THA and TKA, despite available guidelines for prophylaxis, approximately 1.5% of patients still develop symptomatic VTE. This paper discusses an individualized approach to optimize the balance between VTE and bleeding risks by stratifying patients according to their risk levels.

Summary: While many patients who switched from VKAs to DOACs were adherent to therapy long-term, between 23 and 39% exhibited sub-optimal adherence. These patients can be identified through their modifiable health beliefs at the time of switching.

Prevention of hospital-associated venous thromboembolism - Insight from the Getting It Right First Time thrombosis survey in England

Summary: A national VTE prevention program was introduced in England, and the first national thrombosis survey was conducted. The survey revealed that 88% of patients received anticoagulant thromboprophylaxis when indicated, with a patient information provision rate of 31%. However, 13% of hospital-associated VTE cases were considered potentially preventable.

How do anticoagulants impact menstrual bleeding and quality of life? - The PERIOD study

Jignesh P. Patel, Obianuju Nzelu, Lara N. Roberts, Jemma Johns, Jackie Ross, Roopen Arya

Summary: This study reports on the occurrence of heavy menstrual bleeding in menstruating women after starting anticoagulant therapy, and its impact on their quality of life. The results showed that women in the anticoagulated group experienced an increase in the length of their menstrual cycle and significantly higher levels of bleeding, leading to a negative impact on their quality of life. Clinicians should be aware of this issue and take measures to minimize the impact of heavy menstrual bleeding on patients.

Summary: This study aimed to understand the lived experience of menstruating women commencing anticoagulants. Through semi-structured interviews, it was found that women experienced heavy menstrual bleeding during the medication period, which had a negative impact on their social life and mood. Therefore, measures should be taken to support and minimize the negative effects of menstrual bleeding on women's lives.

Hirudin versus citrate as an anticoagulant for ROTEM platelet whole blood impedance aggregometry in thrombocytopenic patients

Wasanthi Wickramasinghe, Bhawani Yasassri Alvitigala, Thisarika Perera, Panduka Karunanayake, Saroj Jayasinghe, Senaka Rajapakse, Praveen Weeratunga, Ananda Wijewickrama, Roopen Arya, Klaus Goerlinger, Lallindra Viranjan Gooneratne

Summary: This study evaluated the use of hirudin anticoagulated blood compared to citrate in rotational thromboelastometry (ROTEM) in thrombocytopenic patients. The results showed higher platelet aggregation with hirudin compared to citrate, particularly in patients with severe thrombocytopenia. Therefore, the use of hirudin anticoagulation is important for platelet function tests, especially in patients with severe thrombocytopenia.

Summary: This study compares the impact of micronized progesterone (MP) or medroxyprogesterone acetate (MPA) in combination with transdermal estradiol (t-E-2) on traditional coagulation factors and thrombin generation parameters in postmenopausal women with premature ovarian insufficiency or early menopause. The results suggest that the thrombin generation parameters remain neutral after 3 months of hormone therapy, while the traditional coagulation factors show significant changes.

Summary: This study examines the association between coagulation test parameters and bleeding risk in patients with dengue in the critical phase. The results suggest that TRAPTEM in ROTEM platelet is associated with bleeding manifestations, indicating a potential role for defective platelet aggregation in the pathogenesis of bleeding in the critical phase of dengue.

Summary: An observational study on the use of rivaroxaban for VTE treatment in the UK showed that nonadherence was mainly due to unintentional reasons such as forgetfulness, carelessness, or changes in routine. Older age was identified as the strongest predictor of good adherence, emphasizing the importance of timely identification of risk factors for nonadherence.
