Summary: This study aimed to evaluate the sensitivity and false negative rates of standard and age-adjusted D-dimer cut-offs for isolated distal DVT (IDDVT) in outpatients. The results showed that small calf thrombi are associated with low D-dimer levels, and age-adjusted D-dimer may be below the cut-off more frequently in subjects with IDDVT than the standard cut-off D-dimer, although this may exclude IDDVT that require treatment.

It is emerging as a potential therapeutic target for venous thrombosis, pulmonary thromboembolism, and cancer metastasis due to its involvement in venous thrombus formation and thromboinflammation. Circulating soluble GPVI is also being explored as a biomarker for thrombosis-related complications.

Summary: D-dimer testing in cancer patients has great diagnostic and prognostic value for venous thromboembolism. However, its effectiveness is reduced due to lower specificity in this population. Standardizing D-dimer assays, developing modified pretest probability models, and adjusting cut-off values could significantly improve the accuracy and effectiveness of VTE diagnosis in cancer patients.

Summary: In recent years, innovations in the field of venous thromboembolism have led to the discovery of new therapeutic targets through biomarkers and metabolomics. Various studies have characterized the metabolic changes in this condition, including alterations in pathways related to carnitine, tryptophan, purine, and fatty acids metabolism. The recent COVID-19 pandemic has also provided new evidence of hypercoagulability, with factors such as altered von Willebrand factor activity, renin-angiotensin-aldosterone system, and immune dysregulation playing a role. This literature review is the first to bring together the latest evidence on biomarkers, metabolomics, and COVID-19 in venous thromboembolism, as well as current therapeutic protocols.

Therefore, the use of hirudin anticoagulation is important for platelet function tests, especially in patients with severe thrombocytopenia.
