Summary: This study investigated the efficacy of a unified HSCT conditioning protocol consisting of low-dose Cy, fludarabine, and ATG in children with severe aplastic anaemia. The data reviewed from Saudi institutions showed a successful engraftment in all 32 patients who received the HSCT. The 3-year event-free survival rate was 93% and the overall survival rate was 95%.

The findings showed no significant difference in haematological indices and disease status before and after vaccination. Summary: The advances in the management of aplastic anaemia (AA) have been successful, with better understanding of the mechanisms underlying the reduction of hematopoietic stem cells (HSCs) and improvements in supportive care. Efforts to increase immunosuppression potency and stimulate HSCs have not achieved better outcomes, but the use of thrombopoietin receptor agonists (Tpo-RA) has been effective in stimulating early HSCs despite high endogenous Tpo levels.

Summary: Aplastic anaemia (AA) is a rare immune-related adverse event (irAE) seen in patients after immune checkpoint inhibitors (ICIs) administration, with uncertain incidence and outcomes. In a study of 52,303 ICI-treated patients, 77 (0.15%) cases of AA were identified, with a median onset of 126 days (interquartile range, 58-363 days). Systemic glucocorticoids were the most common treatment for AA, and 41.6% of patients were able to resume ICI within a year. Patients diagnosed with AA had a significant decline in overall survival (OS) within the first 120 days, with a worse OS compared to matched patients without AA (hazard ratio 1.72, 95% confidence interval 1.19-2.50; p = 0.003).

Summary: This systematic review and meta-analysis compared the therapeutic effects and prognosis of peripheral blood stem cell transplantation (PBSCT) and traditional bone marrow transplantation (BMT) for aplastic anemia (AA). The results showed that before 2010, there was no significant advantage of PBSCT over BMT in terms of 5-year overall survival, transplant-related mortality, and graft failure rate, but it had a higher risk of chronic and acute graft-versus-host disease (GVHD). After 2010, PBSCT and BMT showed similar 3-year overall survival, GVHD risks, transplant-related mortality, and graft failure rate. Therefore, PBSCT is more suitable for HSCT of AA due to its convenience and pain relief.

Summary: The development of autoimmune diseases following COVID-19 infections or vaccinations has been reported, but the exact relationship between vaccines and autoimmune diseases remains unclear. This case suggests that measuring vaccine antibody titers before and after allogeneic HSCT may provide clues to the pathogenesis of vaccine-related autoimmune diseases. Further evaluations are needed to assess the associations between vaccines and autoimmune diseases such as aplastic anemia.

Summary: Severe aplastic anemia (SAA) is a marrow failure disorder with high morbidity and mortality. This study found that reduced-intensity conditioning HLA-haploidentical bone marrow transplantation (BMT) with posttransplantation cyclophosphamide (PTCy)-based graft-versus-host (GVHD) prophylaxis resulted in 100% overall survival and minimal GVHD in patients with SAA. This approach not only avoids the adverse effects of immunosuppressive therapy, but also expands access to BMT across all populations.

Summary: The study demonstrated the efficacy of haploidentical bone marrow transplantation (haplo-BMT) for patients with severe aplastic anemia II (SAA-II), achieving a 3-year overall survival and disease-free survival rate of 86.4% +/- 0.73%. This indicates that haplo-BMT could be considered as an alternative treatment strategy for SAA-II patients lacking HLA-matched donors.
