False-positive screening results in the Finnish prostate cancer screening trial

BACKGROUND: There is evidence that prostate cancer (PC) screening with prostate-specific antigen (PSA) serum test decreases PC mortality, but screening has adverse effects, such as a high false-positive (FP) rate. We investigated the proportion of FPs in a population-based randomised screening trial in Finland. METHODS: Finland is the largest centre in the European Randomized Study of Screening for Prostate Cancer. We have completed three screening rounds with a 4-year screening interval (mean follow-up time 9.2 years) using a PSA cutoff level of 4.0 ng ml(-1); in addition, men with PSA 3.0-3.9 and a positive auxiliary test were referred. An FP result was defined as a positive screening result without cancer in biopsy within 1 year from the screening test. RESULTS: The proportion of FP screening results varied from 3.3 to 12.1% per round. Of the screened men, 12.5% had at least one FP during three rounds. The risk of next-round PC following an FP result was 12.3-19.7 vs 1.4-3.7% following a screen-negative result (depending on the screening round), risk ratio 3.6-9.9. More than half of the men with one FP result had another one at a subsequent screen. Men with an FP result were 1.5 to 2.0 times more likely to not participate in subsequent rounds compared with men with a normal screening result (21.6-29.6 vs 14.0-16.7%). CONCLUSION: An FP result is a common adverse effect of PC screening and affects at least every eighth man screened repeatedly, even when using a relatively high cutoff level. False-positive men constitute a special group that receives unnecessary interventions but may harbour missed cancers. New strategies are needed for risk stratification in PC screening to minimise the proportion of FP men. Suicide and other causes of death among Chernobyl cleanup workers from Estonia, 1986-2020: an update

Summary: Mortality among Estonian cleanup workers in the Chernobyl area from 1986 to 2020 was studied. The all-cause standardized mortality ratio (SMR) was 1.04 and all-cancer mortality was elevated (SMR 1.16), with radiation-related cancers in excess (SMR 1.20). However, the excesses could be attributed to tobacco and alcohol consumption. Longer stays in the Chernobyl area, non-Estonian ethnicity, and lower education were associated with increased mortality risks. Suicide mortality was also increased, particularly among men with lower education.

Lung cancer incidence attributable to residential radon exposure in Finland

Summary: This study estimated the number of lung cancer cases attributable to residential radon in Finland in 2017, considering factors such as age, sex, dwelling type, and smoking status. The results showed that at least 3% and at most 8% of all lung cancers were attributed to residential radon. Among smokers, the majority of radon-related cases were due to the joint effect of radon and smoking. Reducing radon exposure to specified action levels could significantly decrease the number of radon-attributable cases.

Residential mobility and childhood inflammatory bowel disease: a nationwide case-control study

Summary: The purpose of the study was to examine the association between residential mobility and childhood inflammatory bowel disease (IBD) risk. The findings suggest a slightly decreased risk of childhood IBD associated with residential mobility, especially when moving to rural areas and distances less than 50 km.

Cancer incidence among Chernobyl cleanup workers from Estonia: A 34-year follow-up

Summary: From 1986 to 1991, 4831 men from Estonia were sent to clean up radioactively contaminated areas near Chernobyl. The study compared their cancer incidence rates to that of the male population of Estonia. It was found that there was an excess of radiation-related cancers combined, but not when smoking- and alcohol-related cancers were excluded. Less educated workers had a higher risk of all cancers and smoking-related cancers. An elevated risk of alcohol-related cancers was evident 15 to 24 years after return from the Chernobyl area.

To improve validity, the hormonal section of the questionnaire needs additional explanation, and questions 8-10 should include a not-applicable option for sexually inactive patients.

Impact of Timing of Surgery and Adjuvant Treatment on Survival of Adult IDH-wild-type Glioblastoma: A Single-center Study of 392 Patients

The results showed that the time interval from referral to surgery within 4-10 weeks was not associated with decreased survivals in IDH-wt glioblastomas, while a delay exceeding 30 days from surgery to adjuvant treatment may decrease long-term survival.

Summary: This study found that men using testosterone replacement therapy (TRT) were not at increased risk for prostate cancer (PCa) and did not experience increased mortality rate specifically from PCa or cardiovascular disease (CVD) compared to non-users. In fact, TRT users had a lower PCa mortality rate and slightly lower CVD and all-cause mortality.
