Summary: As competency-based medical education has become prominent, little is known about the impact of competency-based time-variable training (CBTVT) on learners. This study explored how a CBTVT pilot program affected resident motivation, assessment, and feedback. The program had variable effects on residents' motivation and increased their engagement with and awareness of assessment. Participants became more aware of the quality of assessments and reported being more deliberate when assessing others. Future research should investigate interventions that promote mastery learning over performance.

Summary: The aim of this study was to determine whether regional abdominal wall nerve block is superior to epidural anesthesia after hepatectomy. The study found that regional abdominal wall nerve block was associated with shorter hospital stay without an increase in postoperative complications or mortality. This means that regional abdominal wall nerve block can be a viable alternative anesthesia adjunct to epidural anesthesia in patients undergoing hepatectomy.

Videolaryngoscopy showed to be a successful rescue device in most cases.

Summary: This study aimed to determine whether auto-titrating positive airway pressure (APAP) reduces postoperative delirium in older adults with newly diagnosed obstructive sleep apnea (OSA) undergoing elective hip or knee arthroplasty. The results showed no significant difference in the risk of postoperative delirium between the group receiving APAP treatment and the routine care group. Additionally, there were no significant differences in length of stay and perioperative complications between the two groups.

Summary: Ultrasound-guided identification of the cricothyroid membrane significantly reduced the recommended vertical incision size with similar success rates. However, there was an increased time when performing a Scalpel-Bougie-Tube cricothyroidotomy on an impalpable porcine larynx model by physicians in training. Ultrasonography should be considered in difficult airway management of impalpable neck.
