Summary: This animal study found that an 8-week drug holiday prior to dental extractions can partially recover osteoclastic activity in osteoporosis rats, but does not alleviate the development of MRONJ-like lesion or exacerbate the osteoporotic condition in the femur. Longer-term drug holiday, or combination of drug holiday and other prophylaxes, could be explored in future studies for prevention of MRONJ in patients with osteoporosis.

Additionally, supplementing with geranylgeraniol (GGOH) can restore Rac1 homeostasis and rescue macrophage efferocytosis, potentially providing a new treatment approach for BRONJ.

Summary: This study investigated the effects of sex and treatment on the establishment of ONJ and bone biomechanical alterations in a ZA-related osteonecrosis mouse model. The results showed that ZA-treated groups had lower bone density and females had lower bone density compared to males. The ZA treatment decreased flexural modulus and yield stress but increased stiffness due to higher bone volume. Additionally, ZA increased crystallinity in bone and dental structure. In conclusion, ZA modifies the mineral matrix in bone and dental structure, improving bone mechanical properties despite the occurrence of osteonecrosis.

Summary: This study aimed to investigate the relationship between PPARγ and PPARGC1A polymorphisms and the development of BRONJ in female osteoporosis patients undergoing bisphosphonate treatment. The results showed that PPARGC1A rs2946385 and rs10020457 polymorphisms were significantly associated with BRONJ. Patients with the combined genotypes of GG in both PPARγ rs1151999 and PPARGC1A rs2946385 had a higher risk of BRONJ. Old age (≥70 years) and long duration of bisphosphonate use (≥60 months) also increased the risk of BRONJ.

This study compares the effects of hindlimb unloading and aging on bone microarchitecture and gene expression in mice, finding that while both induce similar changes, aging has a greater impact on the transcriptome and tissue level.

Summary: The increased risk of fractures observed in adults with type 1 diabetes (T1D) cannot be solely explained by modest decreases in areal bone mineral density (BMD). Accumulation of advanced glycation endproducts (AGEs) in bone has been suggested as a possible cause for the increased bone fragility in diabetes. Although the evidence linking AGEs and fractures in individuals with T1D is limited, recent data show that AGEs, as measured by skin intrinsic fluorescence, are a risk factor for lower BMD in T1D. Further research is needed to determine if there is a causal relationship between fractures and AGEs in T1D. If confirmed, this could lead to interventions that can reduce AGE accumulation and ultimately reduce fractures in T1D patients.
