Homogeneity in immune features between colorectal liver metastases better identifies patients with good prognosis compared to pathological response to preoperative chemotherapy

Summary: The density of tumor-infiltrating lymphocytes, the expression of class I major histocompatibility complex (MHC-I), and the pathological response to preoperative chemotherapy were shown to be associated with oncological outcomes in colorectal cancer liver metastases (CRLM) after complete resection. However, the significance of heterogeneity in these features for multiple CRLMs is still under investigation. Our study found that patients with immune homogeneous CRLMs had longer time to recurrence and better disease-specific survival compared to those with heterogeneous CRLMs. Homogeneity of response to preoperative chemotherapy was not associated with oncological outcomes. The heterogeneity of CD3 and MHC-I expression was independent of response to chemotherapy and other clinicopathological variables.

Summary: Liver metastasis of mismatch repair-proficient (pMMR) CRCs is unresponsive to immune checkpoint blockade (ICB) therapy, while the same tumors are sensitive to ICB therapy when grown subcutaneously. Dendritic cells play a crucial role in increasing the efficacy of immuno-therapies against pMMR CRC liver metastases, as evidenced by increased infiltration and improved survival in mouse models. Compared to subcutaneous tumors, orthotopic liver metastases of pMMR CRCs show a paucity of activated T cells and dendritic cells, indicating the importance of utilizing orthotopic tumor models in guiding human clinical trials.

Summary: Neutralizing the effects of IL-10 in human CRLM has shown to enhance T-cell-mediated carcinoma cell death, increase the proportion of CD8(+) T cells, and increase the HLA-DR expression of macrophages. Blocking IL-10 signaling also rescues CAR-T cell function from myeloid cell-mediated immunosuppression and augments CEA-specific CAR-T cell activation and cytotoxicity.

Summary: Our study utilized single-cell transcriptome analysis to reveal that the TME of treatment-naive tumors is characterized by the higher abundance of less-activated B cells and higher heterogeneity of TAMs. In contrast, tumors treated with PC showed activation of B cells, lower diversity of TAMs with immature and less activated phenotype, lower abundance of dysfunctional T cells and ECM-remodeling cancer-associated fibroblasts, and an accumulation of myofibroblasts. This provides a foundation for future research on cellular mechanisms underlying liver metastasis of CRC and its response to PC.

Summary: This study aimed to investigate the association between selected preoperative inflammatory markers and outcomes of liver resection for colorectal liver metastases (CRLM). It was found that Glasgow prognostic score (GPS), modified Glasgow prognostic score (mGPS), and C-reactive protein to albumin ratio (CAR) had no impact on postoperative complications, but CAR was a better predictor of overall survival, especially after open liver resections.

The purpose of this paper was to highlight the novelty and updates in the field of multiple minimally invasive liver resections and discuss the feasibility and outcomes of laparoscopic concomitant multiple liver resections for colorectal liver metastases.

Immune Contexture of MMR-Proficient Primary Colorectal Cancer and Matched Liver and Lung Metastases

Summary: Metastasis is the main cause of cancer mortality, with the liver and lungs being the most common sites for colorectal cancer (CRC) metastases. High immune cell infiltration and PD-L1 positivity were found to almost double in metastases compared to primary tumors, and high ICS in the least immune-infiltrated metastases of pMMR CRC patients was confirmed to have prognostic value. The differences in immune contexture between primary tumors and metastases may have implications for treatment strategies for advanced CRC patients.
