Clinical case review: A method to improve identification of true clinical and radiographic pneumonia in children meeting the World Health Organization definition for pneumonia

Background: The World Health Organization's (WHO) case definition for childhood pneumonia, composed of simple clinical signs of cough, difficult breathing and fast breathing, is widely used in resource poor settings to guide management of acute respiratory infections. The definition is also commonly used as an entry criteria or endpoint in different intervention and disease burden studies. Methods: A group of paediatricians conducted a retrospective review of clinical and laboratory data including C-reactive protein concentration and chest radiograph findings among Filipino children hospitalised in the Bohol Regional Hospital who were enrolled in a pneumococcal vaccine efficacy study and had an episode of respiratory disease fulfilling the WHO case definition for clinical pneumonia. Our aim was to evaluate which disease entities the WHO definition actually captures and what is the probable aetiology of respiratory infections among these episodes diagnosed in this population. Results: Among the 12,194 children enrolled to the vaccine study we recorded 1,195 disease episodes leading to hospitalisation which fulfilled the WHO criteria for pneumonia. In total, 34% of these episodes showed radiographic evidence of pneumonia and 11% were classified as definitive or probable bacterial pneumonia. Over 95% of episodes of WHO-defined severe pneumonia (with chest indrawing) had an acute lower respiratory infection as final diagnosis whereas 34% of those with non-severe clinical pneumonia had gastroenteritis or other non-respiratory infection as main cause of hospitalisation. Conclusion: The WHO definition for severe pneumonia shows high specificity for acute lower respiratory infection and provides a tool to compare the total burden of lower respiratory infections in different settings.

Summary: Limited studies have been conducted on the Integrated Management of Childhood Illness (IMCI) program in the Philippines, with a focus on the perspectives of frontline health care workers. Challenges faced by HCWs implementing IMCI in the Philippines include inadequate financial resources, training, leadership, access to relevant documents, and professional hierarchies, despite the perceived holistic benefits of the program to the community. More effective implementation processes are needed to improve acceptance in primary health care facilities.

Summary: Rabies is an acute fetal encephalitis caused by the rabies virus, which is incurable after symptom onset but can be prevented by post-exposure prophylaxis during the long incubation period. Apolipoprotein A1 (ApoA1) was identified as a serum biomarker for preclinical diagnosis, with its levels increasing during the incubation period and on the day of symptom onset. Additional biomarker candidates are needed for effective preclinical diagnosis of rabies.

Summary: The study evaluated the diagnostic accuracy of lateral flow devices (LFD) with the straw sampling method compared to the direct fluorescent antibody test (dFAT) in animal rabies diagnosis, showing that LFD with straw sampling method had relatively high sensitivity and 100% specificity. This method can be a strong tool to overcome limited animal surveillance in remote areas.

Time to Revise the WHO Categories for Severe Rabies Virus Exposures-Category IV?

Summary: Rabies is a devastating disease that needs appropriate and timely prevention and treatment. The current categorization for rabies exposure needs revision to include a special Category IV for severe bites. Multiple factors that contribute to the severity of the disease have not been considered in current recommendations. The introduction of Category IV will optimize patient management and improve care for high-risk patients, highlighting the urgency of this neglected disease at a global level.

Summary: Shorter rabies pre-exposure prophylaxis (PrEP) regimens offer improved convenience and feasibility over classic 3-week regimens, especially for regions with limited access to vaccines or travelers to rabies-endemic areas. In this multicenter trial, a 1-week PrEP regimen was found to be effective in generating robust immune responses across age groups, with no safety concerns.

Implications of the antiviral drug favipiravir on rabies immunoglobulin for post-exposure prophylaxis of rabies in mice model with category III-like exposures

Summary: This study assessed the potential of the antiviral drug favipiravir as an alternative treatment for rabies. The results showed that using favipiravir with the rabies vaccine provided complete protection against death caused by the virus, and the virus-neutralizing antibody titer was significantly higher compared to the treatment with the vaccine and immunoglobulin. A clinical trial of this approach is needed, especially in countries with limited availability of immunoglobulin.

Molecular Analysis of Rabies Virus Using RNA Extracted from Used Lateral Flow Devices

Summary: The study aims to develop an assay to detect the rabies virus genome in field samples using a portable real-time reverse transcription-polymerase chain reaction (RT-PCR) instrument. The developed PCR1100 assay showed high sensitivity and specificity, even detecting low copy numbers of the virus. This assay has the potential to effectively improve surveillance and confirmation of rabies in remote areas.
