Background: The increasing prevalence of diabetes is associated with increased health care use and costs. Innovations to improve the quality of care, manage the increasing demand for health care and control the growth of health care costs are needed. The aim of this study is to evaluate the care process and costs of managed, protocolized and usual care for type 2 diabetes patients from a societal perspective. Methods: In two distinct regions of the Netherlands, both managed and protocolized diabetes care were implemented. Managed care was characterized by centralized organization, coordination, responsibility and centralized annual assessment. Protocolized care had a partly centralized organizational structure. Usual care was characterized by a decentralized organizational structure. Using a quasi-experimental control group pretest-posttest design, the care process (guideline adherence) and costs were compared between managed (n = 253), protocolized (n = 197), and usual care (n = 333). We made a distinction between direct health care costs, direct non-health care costs and indirect costs. Multivariate regression models were used to estimate differences in costs adjusted for confounding factors. Because of the skewed distribution of the costs, bootstrapping methods (5000 replications) with a bias-corrected and accelerated approach were used to estimate 95% confidence intervals (CI) around the differences in costs. Results: Compared to usual and protocolized care, in managed care more patients were treated according to diabetes guidelines. Secondary health care use was higher in patients under usual care compared to managed and protocolized care. Compared to usual care, direct costs were significantly lower in managed care (is an element of-1.181 (95% CI: -2.597 to -334)) while indirect costs were higher (is an element of 758 (95% CI: -353 to 2.701), although not significant. Direct, indirect and total costs were lower in protocolized care compared to usual care (though not significantly). Conclusions: Compared to usual care, managed care was significantly associated with better process in terms of diabetes care, fewer secondary care consultations and lower health care costs. The same trends were seen for protocolized care, however they were not statistically significant.

The handling of missing data in trial-based economic evaluations: should data be multiply imputed prior to longitudinal linear mixed-model analyses?

Angela Jornada Ben, Johanna M. van Dongen, Mohamed El Alili, Martijn W. Heymans, Jos W. R. Twisk, Janet L. MacNeil-Vroomen, Maartje de Wit, Susan E. M. van Dijk, Teddy Oosterhuis, Judith E. Bosmans

Summary: This study aimed to assess whether multiple imputation (MI) is required prior to using longitudinal linear mixed-models (LLM) when analyzing longitudinal cost and effect data. The results showed that LLM, MI-LLM, and MI-SUR outperformed other strategies in terms of costs and effects. MI-LLM and MI-SUR performed better than LLM for costs at 10% and 25% missing data. However, at 50% missing data, all strategies resulted in relatively high errors for costs.

A. N. Varga, A. E. Guevara Morel, J. M. van Dongen, J. Lokkerbol, M. Lindeboom, M. W. van Tulder, L. M. Bouter, J. E. Bosmans

Summary: This paper discusses the collaboration challenges faced by epidemiologists and economists in health intervention research due to differences in terminology, and illustrates these differences with examples. The study aims to improve the quality of multidisciplinary collaboration between the two fields and assist researchers in selecting the most suitable analytical techniques for their research problems.

Summary: This article presents a protocol for a randomized controlled trial (RCT) aimed at assessing the (cost-) effectiveness of adding Cognitive Behavioral Therapy for Insomnia (CBTi) to usual care for depression. The study hypothesizes that the addition of CBTi to depression treatment will result in significant improvements in treatment outcomes, quality of life, and a decrease in healthcare and societal costs.

Summary: The aim of this article is to review the methods available for dealing with confounding in analyzing the effect of health care treatments with single-point exposure in observational data. The results show that there are significant differences in performance between different methods, and the performance of a specific method is highly dependent on the estimator used.

To what extent does the use of crosswalks instead of EQ-5D value sets impact reimbursement decisions?: a simulation study

Summary: Inconsistent results have been found on the impact of using crosswalks versus EQ-5D value sets on reimbursement decisions. This simulation study further investigates the issue and finds that the use of crosswalks can significantly influence the cost-utility outcomes, potentially affecting reimbursement decisions.

Summary: This study aimed to estimate the ability of using electronic medical record data to identify frailty in older patients. The Charlson Comorbidity Index (CCI) was found to be the only measure with acceptable performance, while the other measures showed poorer performance in older age groups.

Individual psychotherapy for cluster-C personality disorders: protocol of a pragmatic RCT comparing short-term psychodynamic supportive psychotherapy, affect phobia therapy and schema therapy (I-FORCE)

Martine Daniels, Henricus L. L. Van, Birre van den Heuvel, Jack J. M. Dekker, Jaap Peen, Judith Bosmans, Arnoud Arntz, Marcus J. H. Huibers

Summary: This study aims to compare the differential (cost)-effectiveness of three individual psychotherapies (short-term psychodynamic supportive psychotherapy, affect phobia therapy, and schema therapy) for Cluster-C personality disorders. The study population consists of 264 patients aged 18-65 years seeking treatment at a Dutch mental health care institute. The primary outcome measure is the change in the severity of the PD, and secondary outcome measures include personality functioning, psychiatric symptoms, and quality of life. This study is important for improving the quality of care for this vulnerable group of patients.

Mapping Oswestry Disability Index Responses to EQ-5D-3L Utility Values: Are Cost-Utility Results Valid?

Summary: The study aimed to develop and validate approaches for mapping Oswestry Disability Index responses to 3-level version of EQ-5D utility values and evaluate their impact on cost-utility results compared with published regression models. Three response mapping approaches were developed and their performance was assessed. The results suggest that the developed approaches are not valid for estimating individual patients' utility values and may significantly affect cost-utility results.

Summary: Trial-based economic evaluations face methodological challenges such as missing data, correlated costs and effects, baseline imbalances, and skewed costs and effects. Inappropriate methods can bias results and hinder healthcare decision-making. This tutorial provides step-by-step guidance on combining appropriate statistical methods to address these challenges using an R script, with theoretical background and illustration using a simulated trial-based economic evaluation.
