Circulating AMH Reflects Ovarian Morphology by Magnetic Resonance Imaging and 3D Ultrasound in 121 Healthy Girls

Context: In adult women, Anti-Mullerian hormone (AMH) is produced by small growing follicles, and circulating levels of AMH reflect the number of antral follicles as well as primordial follicles. Whether AMH reflects follicle numbers in healthy girls remains to be elucidated. Objective: This study aimed to evaluate whether serum levels of AMH reflects ovarian morphology in healthy girls. Design and Setting: This was a population-based cohort study involving the general community. Participants: Included in the study were 121 healthy girls 9.8-14.7 years of age. Main Outcome Measures: Clinical examination, including pubertal breast stage (Tanner's classification B1-5), ovarian volume, as well as the number and size of antral follicles were assessed by two independent modalities: magnetic resonance imaging (MRI), Ellipsoid volume, follicles >= 2 mm; and Transabdominal ultrasound, Ellipsoid and 3D volume, follicles >= 1 mm. Circulating levels of AMH, inhibin B, estradiol, FSH, and LH were assessed by immunoassays; T and androstenedione were assessed by liquid chromatography-tandem mass spectrometry. Results: AMH reflected the number of small (MRI 2-3 mm) and medium (4-6 mm) follicles (Pearson's Rho [r] = 0.531 and r = 0.512, P < .001) but not large follicles (>= 7 mm) (r = 0.109, P = .323). In multiple regression analysis, small and medium follicles (MRI <= 6 mm) remained the main contributors to circulating AMH beta, 0.501; P = .001) whereas the correlation between AMH and estradiol was negative (beta, -0.318; P = .005). In early puberty (B1-B3), the number of AMH-producing follicles (2-6 mm) correlated positively with pubertal stages (r = 0.453, P = .001), whereas AMH levels were unaffected (- 0.183, P = .118). Conclusions: Similarly to adult women, small and medium antral follicles (<= 6 mm) were the main contributors to circulating levels of AMH in girls.

The results showed that high daily glucocorticoid doses per weight were associated with HPA axis suppression in infants. Monitoring is necessary for infants receiving glucocorticoids after ocular surgery.

The seminal plasma microbiome of men with testicular germ cell tumours described by small RNA sequencing

Summary: The ratio between luteinizing hormone (LH) and follicle-stimulating hormone (FSH) has been found to be a reliable marker for sex determination in healthy infants. However, its role in patients with differences of sex development (DSD) remains unclear. This retrospective study aimed to describe LH/FSH in infants with DSD and found that the ratio sometimes corresponds to the assigned sex, but not in all cases.

Individuals with numerical and structural variations of sex chromosomes: interdisciplinary management with focus on fertility potential

Summary: Diagnosis and management of individuals with differences of sex development (DSD) due to numerical or structural variations of sex chromosomes (NSVSC) is challenging. Girls with Turner syndrome (45X) may have varying phenotypic features. Individuals with 45,X/46,XY chromosomal mosaicism may have Turner syndrome-like features and short stature. Newborn screening for sex chromosome variations could be possible but requires further analysis. Comprehensive healthcare and fertility assessment are important for individuals with NSVSC.

Summary: Congenital adrenal hyperplasia (CAH) is a recessive condition that affects the adrenal glands. Adult males with CAH, despite hormone replacement therapy, exhibit reduced levels of Leydig and Sertoli cell-derived hormones, including insulin-like factor 3 (INSL3) and testosterone. This study suggests that circulating INSL3 may serve as a biomarker to monitor Leydig cell function in CAH patients, particularly those with testicular adrenal rest tumors.

Summary: Adult patients with Klinefelter syndrome (KS) exhibit variable phenotypes and a high risk of metabolic disorders. Most adults require testosterone replacement therapy (TRT), but its use during puberty is debated. This study found that patients with KS have unfavorable body composition and impaired bone mineral status already during childhood and adolescence. Further research is needed to evaluate the effects of TRT during puberty on these parameters.

Cotinine concentrations in maternal serum and amniotic fluid during pregnancy and risk of testicular germ cell cancer in the offspring: A prospective nested case-control study

Summary: Maternal smoking during pregnancy does not appear to be significantly associated with the risk of testicular germ cell cancer (TGCC) in offspring, based on a study using cotinine measurements as a biomarker for tobacco exposure. The study found comparable cotinine levels in maternal serum and amniotic fluid, but no significant association between active tobacco use and TGCC risk. However, different risks were observed for different types of TGCC, although they were not statistically significant.

However, dimension reduction of clinical and hormonal profiles by PCA improved the diagnostic value, which in the future may support the diagnostic process as a supplement to the GnRH test in evaluation of pubertal disorders.

Effects of vitamin D on sex steroids, luteinizing hormone, and testosterone to luteinizing hormone ratio in 307 infertile men

Summary: This study aimed to investigate whether vitamin D supplementation has an impact on sex steroid production in infertile men with vitamin D insufficiency. The results showed that vitamin D supplementation did not alter sex steroid production in infertile men overall, but in men with vitamin D insufficiency, it significantly increased testosterone/luteinizing hormone ratio.
