African-Caribbean ethnicity is associated with an increased risk of both first and recurrent venous thromboembolism (VIE). The aim of this study was to evaluate thrombin generation in African-Caribbeans compared with whites in patients with deep vein thrombosis (DVT) and healthy volunteers. Thrombin generation was measured in a case-control study of 80 patients who had completed anticoagulation therapy for a first DVT (50 white and 30 African-Caribbean) and 66 controls. Peak thrombin with and without thrombomodulin was significantly higher in African-Caribbeans with DVT compared with whites with DVT (P<0.001 for both comparisons) and African-Caribbean controls (P<0.001, 0.003, respectively). Endogenous thrombin potential (ETP) with and without thrombomodulin was significantly higher in African-Caribbeans with DVT than whites with DVT (P <= 0.001 for both comparisons). Maximum velocity and ETP ratio were increased in African-Caribbeans with DVT compared with whites with DVT (P<0.001 and 0.030, respectively) and African-Caribbean controls (P<0.001 and 0.019, respectively). Within the control group, peak thrombin was significantly increased in African-Caribbeans compared with whites (P=0.017). ETP, peak thrombin with thrombomodulin and maximum velocity were also increased in African-Caribbeans compared with white controls (P=0.045 for all comparisons). African-Caribbeans with DVT had significantly higher factor VIII levels compared with whites with DVT and controls. African-Caribbean ethnicity confers a hypercoagulable state as measured by thrombin generation. This supports epidemiological findings of increased risk of first and recurrent VIE. Thrombin generation requires adjustment for ethnicity in studies undertaken in ethnically diverse populations. Preventing VTE following total hip and knee arthroplasty: Is prediction the future?

Banne Nemeth, Rob Nelissen, Roopen Arya, Suzanne Cannegieter

Summary: VTE is a common complication following THA and TKA, despite available guidelines for prophylaxis, approximately 1.5% of patients still develop symptomatic VTE. This paper discusses an individualized approach to optimize the balance between VTE and bleeding risks by stratifying patients according to their risk levels.

Summary: Recent reports have highlighted rare cases of CVST and thrombocytopenia following Vaxzevria vaccination. An immunological mechanism similar to HIT, but without prior heparin exposure, is suspected. Management of such cases involves PF4 antibody testing and treatment similar to HIT.

Summary: A national VTE prevention program was introduced in England, and the first national thrombosis survey was conducted. The survey revealed that 88% of patients received anticoagulant thromboprophylaxis when indicated, with a patient information provision rate of 31%. However, 13% of hospital-associated VTE cases were considered potentially preventable.

How do anticoagulants impact menstrual bleeding and quality of life? - The PERIOD study

Jignesh P. Patel, Obianuju Nzelu, Lara N. Roberts, Jemma Johns, Jackie Ross, Roopen Arya

Summary: This study reports on the occurrence of heavy menstrual bleeding in menstruating women after starting anticoagulant therapy, and its impact on their quality of life. The results showed that women in the anticoagulated group experienced an increase in the length of their menstrual cycle and significantly higher levels of bleeding, leading to a negative impact on their quality of life. Clinicians should be aware of this issue and take measures to minimize the impact of heavy menstrual bleeding on patients.

Summary: This study aimed to understand the lived experience of menstruating women commencing anticoagulants. Through semi-structured interviews, it was found that women experienced heavy menstrual bleeding during the medication period, which had a negative impact on their social life and mood. Therefore, measures should be taken to support and minimize the negative effects of menstrual bleeding on women's lives.

Summary: This study evaluated the use of hirudin anticoagulated blood compared to citrate in rotational thromboelastometry (ROTEM) in thrombocytopenic patients. The results showed higher platelet aggregation with hirudin compared to citrate, particularly in patients with severe thrombocytopenia. Therefore, the use of hirudin anticoagulation is important for platelet function tests, especially in patients with severe thrombocytopenia.

Summary: This study compares the impact of micronized progesterone (MP) or medroxyprogesterone acetate (MPA) in combination with transdermal estradiol (t-E-2) on traditional coagulation factors and thrombin generation parameters in postmenopausal women with premature ovarian insufficiency or early menopause. The results suggest that the thrombin generation parameters remain neutral after 3 months of hormone therapy, while the traditional coagulation factors show significant changes.

Rotational thromboelastometry in critical phase of dengue infection: Association with bleeding

Summary: This study examines the association between coagulation test parameters and bleeding risk in patients with dengue in the critical phase. The results suggest that TRAPTEM in ROTEM platelet is associated with bleeding manifestations, indicating a potential role for defective platelet aggregation in the pathogenesis of bleeding in the critical phase of dengue.
