GATA2 deficiency and MDS/AML: Experimental strategies for disease modelling and future therapeutic prospects

Summary: The importance of predisposition to leukemia, especially GATA2 deficiency, with a high risk of developing myelodysplastic syndrome and acute myeloid leukemia, is increasingly recognized. Allogenic hematopoietic stem cell transplantation is the only curative option, but chances of survival decrease with progression of immunodeficiency and disease evolution. Variability in penetrance and expressivity within families carrying GATA2 mutations suggests the involvement of cooperating extrinsic events in triggering the disease. Lack of predictive tools and understanding of intrafamilial heterogeneity highlights the need for further research in this area.

Summary: GATA2 deficiency is a genetic disorder affecting hematopoiesis, lymphatics, and immunity, with prominent pulmonary manifestations including chronic infections, pulmonary alveolar proteinosis, and pulmonary arterial hypertension. Stem cell transplantation can reverse myelodysplasia and immune deficiency, as well as improve pulmonary manifestations in most patients.

Summary: The correct maintenance and differentiation of hematopoietic stem cells (HSC) in bone marrow, regulated by GATA2, is crucial for the functioning of the human blood system. GATA2 deficiency syndrome, caused by mutations in GATA2, leads to various defects and diseases, yet the underlying mechanisms remain to be fully understood. This review summarizes recent advances in understanding the role of GATA2 in determining the fate of hematopoietic cells and discusses the challenges of modeling GATA2 deficiency syndrome.

The Clinical Spectrum, Diagnosis, and Management of GATA2 Deficiency

Summary: A predisposition to myeloid neoplasms, such as GATA2 deficiency, has been recognized as a clinical entity by the World Health Organization. The only curative treatment for this syndrome is allogeneic hematopoietic stem cell transplantation, which should be performed before irreversible organ damage occurs. Patients with GATA2 deficiency should be referred to a multidisciplinary team for evaluation and consultations with appropriate specialists.

Elderly Gata2 haploinsufficient mice exhibit different characteristics compared to young mice, suggesting a potential impact on the evolution of malignancies in syndromes with GATA2 haploinsufficiency.

Summary: A 3-year-old girl presented with cervical and mediastinal lymphadenopathy due to Mycobacterium fortuitum infection. Analysis revealed profound mononuclear cell deficiency and a de novo 187-kb duplication of the entire GATA2 locus with a maternally inherited copy number variation deletion of 25 kb. This case highlights the importance of evaluating allele count in suspected GATA2-related syndromes.

Summary: In pediatric MDS, patients with GATA2 mutations who underwent HSCT had a 5-year overall survival of 75% and disease-free survival of 70%. Non-relapse mortality and relapse were equal contributors to treatment failure. Organ toxicities and infection rates did not show significant increase, suggesting early consideration of HSCT in young individuals with GATA2 deficiency.

Summary: In this study, a cohort of 31 Italian patients with molecular diagnosis of GATA2 deficiency was described. Case Report: Missing zinc finger domains: hemophagocytic lymphohistiocytosis in a GATA2 deficiency patient triggered by non-tuberculous mycobacteriosis

Summary: Haploinsufficiency of GATA2, also known as GATA2 deficiency, can result in a wide range of clinical manifestations. In this study, we presented a case of a 28-year-old male with a GATA2 variant who also suffered from hemophagocytic lymphohistiocytosis (HLH), ultimately diagnosed as HLH triggered by Mycobacterium avium bloodstream infection due to primary immunodeficiency. We also reviewed GATA2 deficiency patients with HLH and found that GATA2 variants causing loss of zinc finger domains were associated with HLH, with erythema nodosum possibly being an accompanying symptom.
