Patient experience of follow-up after surgery for kidney cancer: a focus group study

Summary: The objective of this study was to explore the patient experience of follow-up care after kidney cancer surgery and develop recommendations for best practice. Two focus groups were conducted with 14 participants who had experience with kidney cancer follow-up care after surgery. Thematic analysis identified six themes describing the patient experience, including feelings of abandonment, uncertainty about the plan, anxiety about appointments, variation in care, a need for information, and a need for emotional support. Based on the results, a set of recommendations was developed to address these challenges.

Early detection of kidney cancer using urinary proteins: a truly non-invasive strategy

Evaluation of feasibility, sample preparation, biomarker validation, and novel technologies may maximize the potential for a truly non-invasive biomarker in early RCC diagnostics.

Validation and public health modelling of risk prediction models for kidney cancer using the UK Biobank

Summary: The post hoc analysis of the SURTIME trial data showed that patients in the deferred nephrectomy arm had a higher percentage of receiving sunitinib compared to the immediate nephrectomy arm, and they also had a more profound reduction of target lesions. Starting with systemic therapy may impair the administration, onset, and duration of sunitinib in comparison to the deferred approach, potentially leading to early and more profound disease control.

Every decade counts: a narrative review of functional recovery after partial nephrectomy

Summary: This narrative review provides an overview of the major advances and controversies regarding ischaemia and functional recovery after partial nephrectomy (PN). The duration of ischaemia has been found to strongly correlate with the development of acute kidney injury (AKI) and chronic kidney disease after clamped PN. Further research has shown that the volume of parenchymal tissue saved is the main determinant of functional outcomes. Cold ischaemia has been confirmed to be highly protective, and limited warm ischaemia is also safe. The reconstructive phase of PN appears to be most important for functional recovery.

Risk models for recurrence and survival after kidney cancer: a systematic review

Summary: This study systematically compared the performance of different prognostic models in predicting survival or recurrence in patients with localized renal cell cancer who underwent surgery. The study found that several models had good discriminative ability, and there was no single "best" model. When choosing a model, both the comparative performance and the availability of factors included in the model should be considered.

Nephrometry scores: a validation of three systems for peri-operative outcomes in retroperitoneal robot-assisted partial nephrectomy

Summary: This study validates the RENAL, PADUA and SPARE scoring systems to predict key intra-operative outcomes in retroperitoneal RAPN. Only PADUA and SPARE were able to predict achievement of the trifecta. As a simplified version of the PADUA scoring system with comparable outcomes, we recommend using the SPARE system.

Opportunities for discussion are crucial in reducing psychological harm.

Percutaneous microwave ablation of renal masses in a UK cohort

A short screening tool identifying systemic barriers to distress screening in cancer care

Summary: International guidelines recommend screening for cancer patients’ distress, but rates are insufficient. This study developed a survey to assess systemic barriers to distress screening. Through a three-step approach, an initial survey with 53 questions was reduced to a final set of 14 validated questions using qualitative content analysis and factor analysis. Major barriers identified include timing of screening, lack of capacity, patient and staff overload, and refusal of patients to be referred to support services. The validated questions enable quick identification and improvement of screening programs.

Summary: This study found that the activation of tissue-nonspecific alkaline phosphatase (TNAP) precedes and predicts calcification in atherosclerotic plaques, and TNAP inhibition prevents plaque calcification and protects arteries from atherosclerosis. The study also discovered that TNAP inhibition reduces blood levels of cholesterol and triglycerides, and ameliorates plaque calcification, thereby protecting arteries.

Summary: Through a questionnaire, we assessed genitourinary cancer patients' perceptions of COVID-19 vaccines and identified potential barriers and influencers in the decision-making process. Despite some concerns, a majority of patients with genitourinary cancers were willing to receive the vaccine. Various strategies could be implemented to improve patient education on the benefits of vaccination in the context of cancer and beyond.

Impact of updated trial data on the cost-effectiveness of percutaneous mitral repair

Summary: When updated clinical trial data becomes available, reassessing the cost-effectiveness of technologies may modify estimates and influence decision-making. In this study, the impact of updated trial outcomes on the cost-effectiveness of percutaneous mitral repair (PR) for secondary mitral regurgitation was investigated. The updated trial data showed an increase in mortality in the intervention arm between two and three years follow-up, leading to a higher incremental cost-effectiveness ratio compared to previous estimates.

Elaboration of a French version of the Duke Activity Status Index questionnaire and performance to predict functional capacity

Summary: The study aimed to create a French version of the DASI questionnaire and assess its diagnostic performance in predicting functional capacity. The results showed a significant positive relationship between FDASI and VO2max. A FDASI score of 36 can be used as a reliable threshold to identify patients with a VO2max < 14 ml.kg(-1).min(-1) and can replace the self-reported inability to climb two flights of stairs.

Summary: Primary Carnitine Deficiency (PCD) will be included in the expansion of the French newborn screening program in 2023. Screening for PCD is complex and has a wide clinical spectrum. Some countries struggle with high false positive rates and have even removed PCD from their screening programs. To understand the risks and benefits, we reviewed the literature and identified hurdles and current practices in PCD newborn screening worldwide. In addition, we address the optimized screening algorithm for the implementation of PCD screening in France.

Hemodynamic Effects of Awake Prone Positioning With COVID-19 Acute Respiratory Failure

Summary: This study investigated the hemodynamic effects of awake prone positioning in non-ventilated subjects with COVID-19 acute respiratory failure. The results showed a significant increase in cardiac index and improvement in right ventricular systolic function during prone positioning, with no significant difference in PaO2/FIO2 and breathing frequency.

Summary: Pulmonary hypertension (PH) is a chronic disorder of the pulmonary circulation that often occurs with other respiratory diseases, and it may lead to right ventricular failure. This study validated an animal model of combined pulmonary fibrosis (PF) and PH, and examined its effects on lung and vascular remodeling as well as right ventricular structure and function. The results showed successful replication of the human disease features in this rat model.

Skin biopsy in adult patients with meningococcal purpura fulminans: a multicenter retrospective cohort study

Summary: This study aimed to assess the sensitivity of skin biopsy in adult patients with meningococcal purpura fulminans. The results showed that skin biopsy combined with conventional culture and meningococcal PCR had a global sensitivity of 88%, and should be considered even after the initiation of antimicrobial treatment.
