To detect a direct transition from a haemorrhagic corpus luteum to an endometriotic cyst by serial transvaginal ultrasonographic scans. Prospective observational study. An academic tertiary care and referral centre for women with endometriosis. One hundred and nine women younger than 40 years, with regular menstrual cycles, undergoing first-line surgery for endometriomas, and not wanting postoperative oral contraception. Three-monthly transvaginal ultrasonography during the luteal phase for 2 years after surgery. Sonographic identification of progression from a haemorrhagic corpus luteum to a recurrent endometriotic cyst. A haemorrhagic corpus luteum was identified in 13 women. Serial ultrasonographic scans demonstrated transition to an endometriotic cyst in 11 (85%) instances and resorption in two. A unilateral endometriotic cyst without previous detection of a cystic corpus luteum was observed in 14 women. Bleeding from a corpus luteum appears to be a critical event in the development of endometriomas.

Conventional IVF performs similarly in women with and without endometriosis

Paola Vigano, Marco Reschini, Marta Ciaffaglione, Veronica Cuce, Maira Casalechi, Laura Benaglia, Paolo Vercellini, Edgardo Somigliana

Summary: This study aimed to compare the factors underlying infertility in women with endometriosis. The results showed that there were no significant differences in fertilization rate, embryo quality, and clinical pregnancy rate between women with and without endometriosis. Therefore, conventional IVF can still be used in endometriosis patients unless there is a male factor involved.

A Case-Control Study on the Effects of Plasticizers Exposure on Male Fertility

Lidia Caporossi, Paola Vigano, Enrico Paci, Silvia Capanna, Alessandra Alteri, Daniela Pigini, Giovanna Tranfo, Bruno Papaleo

Summary: Male infertility is a significant public health concern, and the potential link between exposure to plasticizers like phthalates and bisphenol A and this condition is a topic of debate. A study was conducted with 155 infertility cases and 211 controls to investigate this issue. The results showed higher urinary concentrations of phthalates in cases compared to controls, but no significant difference in BPA levels. However, no clear quantitative correlation between specific plasticizers and sperm parameters was identified, calling for further research on the risks associated with exposure to these chemicals.

ART and the forgotten siblings: a call for research

Edgardo Somigliana, Fabio Parazzini, Alice Goisis, Giovanna Esposito, Letizia Li Piani, Francesca Filippi, Paola Vigano

Summary: The article calls for more research on infertile couples to understand their intended number of children and the rate of return after the first IVF treatment. This could potentially impact clinical practice by improving chances of conceiving a second child and identifying barriers to non-return or a gap between intended and realized number of children. However, the current evidence is insufficient to recommend any interventions.

Summary: This study examined the impact of HPV status on the prognosis of early stage cervical glandular lesions. The results showed that HPV status did not affect the recurrence rate, but most adenocarcinomas were positive for high-risk HPV.

Probability of second live birth after first natural and medically assisted reproduction-mediated live birth: A historical cohort study

Summary: Limited evidence exists on the role of medically assisted reproduction (MAR) in achieving desired family size. This study aimed to assess the probability and mode of conception of a second live birth based on the mode of conception of the first child. The findings showed that women who used MAR for their first child were more likely to use MAR again for a second child.

Questionnaires for the Assessment of Central Sensitization in Endometriosis: What Is the Available Evidence? A Systematic Review with a Narrative Synthesis

Giulia Emily Cetera, Camilla Erminia Maria Merli, Giussy Barbara, Carlotta Caia, Paolo Vercellini

Summary: This review aims to identify central sensitization questionnaires used in clinical endometriosis studies and analyze the strengths and weaknesses of each questionnaire. The Central Sensitization Inventory (CSI) is the most frequently used questionnaire, validated in patients with endometriosis. However, there is ongoing debate about the construct validity of these questionnaires and the need for a gold standard diagnostic tool for central sensitization.

Non-response to first-line hormonal treatment for symptomatic endometriosis: overcoming tunnel vision. A narrative review

Giulia Emily Cetera, Camilla Erminia Maria Merli, Federica Facchin, Paola Vigano, Elisa Pesce, Francesca Caprara, Paolo Vercellini

Summary: Approximately 25-33% of women with endometriosis do not have an adequate response to first-line hormonal treatment for resolution of painful symptoms. The concept of progesterone resistance has been proposed but is controversial. A broader perspective of endometriosis is emerging, questioning whether the lack of response is due to altered signaling pathways or other pain contributors.

Summary: Low ovarian reserve, indicated by anti-Mullerian hormone (AMH) or antral follicle count (AFC), does not increase the risk of miscarriage in young women conceiving with ART.

Mitigating the economic burden of GnRH agonist therapy for progestogen-resistant endometriosis: why not?

Summary: This study aimed to investigate a cost-saving measure for the treatment of endometriosis by using a non-standard dosing regimen of a 3.75 mg depot formulation of triptorelin. The extended-interval dosing regimen, with injections every 6 weeks instead of every 4 weeks, reduced the cost by one-third without compromising pain relief.

Summary: Repetitive ovulation and menstruation provided an advantage in the Palaeolithic era, but societal changes have created a mismatch between genetic adaptation and environment, leading to an increased risk of estrogen-related diseases.

Proposal for targeted, neo-evolutionary-oriented secondary prevention of early-onset endometriosis and adenomyosis. Part II: medical interventions

Paolo Vercellini, Veronica Bandini, Paola Vigano, Deborah Ambruoso, Giulia Emily Cetera, Edgardo Somigliana

Summary: According to epidemiological data, the incidence of endometriosis increases rapidly around the age of 25. The delay in diagnosis is usually between 5 and 8 years in adult women and over 10 years in adolescents. Ovulation and menstruation, which are inflammatory events, may contribute to the early development of endometriosis and adenomyosis. Therefore, secondary prevention targeting severely symptomatic adolescents is crucial for reducing suffering, limiting lesion progression, and preserving future reproductive potential.
