Simultaneous determination of serum concentrations of levodopa, dopamine, 3-O-methyldopa and α-methyldopa by HPLC

Levodopa is the medication of choice for Parkinson's disease. The biological complexity of levodopa and of its main derivatives makes their determination important in the clinical field. The aim of this study was to develop an HPLC method for the simultaneous determination of serum concentrations of levodopa, dopamine, 3-O-methyldopa and a-methyldopa. We compared UV and fluorimetric detection of native and derivatised compounds. Though less sensitive than other methods, UV detection is important to exclude naturally fluorescent, interfering substances. Fluorimetric detection of derivatised compounds is more sensitive than UV detection. Since 3-O-methyldopa does not react with the derivatising agent 1,2-diphenylethylenediamine, it cannot be detected. For simultaneous determination of the four compounds after pharmacological treatment of patients we therefore advise fluorimetric detection of the native compound. Summary: This study aimed to investigate the impact of variation in analytical specificity among different anti-tTG methods on the detection of CD in cases with anti-tTG positive/EMA negative results. The findings suggest that CD diagnosis is more likely related to the positivity of multiple anti-tTG IgA assays, rather than the levels of anti-tTG IgA antibodies. Results that are anti-tTG positive/EMA negative should be approached with caution, and laboratory comments should be provided, recommending referral to a CD reference center for further evaluation.

Summary: The association between celiac disease (CD) and selective immunoglobulin A deficiency (SIgAD) has been known for over fifty years, but diagnosing and monitoring patients with both conditions remain challenging. The 2012 ESPGHAN guidelines provided criteria for diagnosing CD in SIgAD patients, but issues such as selecting patients for specific IgG antibody testing and choosing reliable tests for diagnosis and follow-up still need to be addressed.

Summary: This study aimed to investigate the prevalence and disease stage of autoimmune diseases in patients with ATD, identifying various organ-specific autoantibodies. The results showed that PCA and GADA were the most common autoantibodies, with all autoimmune diseases except celiac disease mainly at the potential stage. Sex, ATD type, smoking habit, and coexistence of other autoimmune diseases were associated with susceptibility to develop chronic atrophic gastritis or autoimmune diabetes mellitus.

Summary: This study shows that current technologies for detecting anti-double-stranded DNA antibodies are not fully comparable, especially in terms of their correlation with the Crithidia luciliae immunofluorescence test (CLIFT). The positioning of these methods in the diagnostic algorithm for systemic lupus erythematosus (SLE) is influenced by their different correlation with CLIFT.

Prevalence of myositis specific and associated antibodies in a cohort of patients affected by idiopathic NSIP and no hint of inflammatory myopathies

Summary: The study aimed to assess the prevalence of myositis-specific and myositis-associated antibodies (MSA and MAA) in patients previously diagnosed with non-specific interstitial pneumonia (NSIP) and without symptoms of idiopathic inflammatory myopathies (IIM). The results showed that 37.5% of the patients displayed significant MSA and/or MAA positivity, and two seropositive patients showed a significant impairment of FVC. A cautious assessment of NSIP patients, especially those with progressive and non-responsive disease, should include an evaluation of MSA and MAA.

Summary: This study assessed the diagnostic accuracy of anti-cN1A antibodies in patients with suspected IIM and evaluated the agreement between line immunoassay (LIA) and enzyme-linked immunoassay (ELISA). The results showed excellent concordance between LIA and ELISA for detecting anti-cN1A antibodies, suggesting that LIA could be a rapid and useful adjunct, and it could even replace ELISA for cN1A assay.

Summary: Autoantibodies against-phospholipase A2 receptor (PLA2R) are specific markers of idiopathic membranous nephropathy (iMN), and commercial ELISA tests can effectively detect these antibodies with high sensitivity and specificity. The optimal cut-off value for positive detection was found to be 2.7 RU/ml, providing the best overall efficiency for the test.

Flavonoids and ischemic stroke-induced neuroinflammation: Focus on the glial cells

Summary: Ischemic stroke is a common and serious disease, and neuroinflammation plays a crucial role in its progression. Microglia, astrocytes, and infiltrating immune cells are involved in the complicated neuroinflammation cascade, releasing different molecules that affect inflammation. Flavonoids, plant-specific compounds, have shown protective effects against cerebral ischemia injury by modulating the inflammatory responses.
