Summary: This study identified three genome-wide significant loci for ADHD comorbid with DBDs, with one locus on chromosome 11 found to be a strong risk locus across European and Chinese ancestries. Summary: Psychological therapies are the first-line treatment for childhood disruptive behavior disorders (DBDs), but many patients require adjunctive pharmacotherapy. This paper summarizes the findings of existing systematic reviews and updates them with recent clinical trials on the use of antipsychotics in children and adolescents with DBDs. The data suggests that risperidone appears to be effective in the short-term management of DBDs, but all available antipsychotics are associated with metabolic adverse effects.

Summary: Conduct disorder (CD) involves aggressive and antisocial behavior, and in male youths, it is associated with blunted cortisol stress response. However, there is limited knowledge about cortisol stress responsivity in female youths with CD and other neuroendocrine responses in both sexes. This study investigated the neuroendocrine responses, including cortisol, testosterone, oxytocin, and psychological stress, in 130 pubertal youths with CD and 160 healthy controls (HCs). The results revealed that both female and male youths with CD showed blunted stress response compared to the HCs. These findings highlight the importance of investigating the biological mechanisms and personalized interventions for CD.

Summary: Iatrogenic adrenal insufficiency due to long-term use of exogenous glucocorticoids is common in rheumatoid arthritis patients, with a proportion needing small doses of glucocorticoids for clinical remission. Inadequate production of endogenous cortisol relative to enhanced clinical needs associated with the systemic inflammatory response may operate in RA, with the underlying mechanisms still unknown. Further research is needed on the pathogenetic role of compromised adrenal reserve in RA.

Best practices for evidence-based assessment of these disorders involve multiple means of information gathering and multiple informants.

Summary: This study evaluates the clinical validity of the Conduct and Oppositional Defiant Disorder Scales (CODDS) as a five-minute instrument for assessing oppositional defiant disorder (ODD) and conduct disorder (CD). The results show that CODDS has good sensitivity and specificity in predicting clinical diagnoses of ODD and CD. It also provides additional value in predicting future diagnoses and social/school functioning.

Summary: This study systematically reviewed randomized clinical trials of various psychotropic medications for the treatment of disruptive behavior (DB) in youths, and used network meta-analysis to compare their relative efficacy. The results showed that second-generation antipsychotics, stimulants, and non-stimulant ADHD medications were more effective than placebo for reducing DBs, with second-generation antipsychotics being the most efficacious. The study provides valuable information on the relative efficacy of different psychotropic medications for DB treatment and insight into potential neurobiological mechanisms underlying these symptoms.

Sex-hormone secreting adrenal tumors may be an under-recognized syndrome that can contribute to these behavioral issues. A detailed physical examination and consideration of endocrine disturbances can provide more accurate diagnosis and treatment for cats undergoing evaluation for inappropriate urination or aggression.

Summary: This study examined the clinical characteristics and familiality of comorbid Bipolar Disorder (BP) and Conduct Disorder (CD) in children. The results showed that children with comorbid BP + CD had more severe behavioral, social, and neurocognitive problems compared to those with BP alone. Moreover, first-degree relatives of children with BP + CD also had higher rates of behavioral problems and smoking.

Summary: This study investigated cortisol and testosterone levels in individuals with Borderline Personality Disorder (BPD) and examined changes in hormones following psychological treatment. The results showed that individuals with BPD had significantly higher testosterone levels than controls, with female patients having double the testosterone levels of female controls. BPD symptoms were associated with hormone levels. Treatment significantly reduced BPD symptoms, but hormone levels did not change.

Summary: This study investigated potential biomarkers for disruptive behavior disorder (DBD) by measuring psychophysiological measures of arousal, including heart rate, skin conductance, and electroencephalogram (EEG). The findings suggest that EEG measurements may be more sensitive in detecting group differences and that higher delta and lower beta activity may represent an index of a proactive subtype of aggression.
