Summary: This study evaluated the relationship between MAD severity and myocardial interstitial fibrosis in MVP patients, as well as the association between interstitial fibrosis severity and ventricular arrhythmic events. Results showed that ECVsyn, a marker of interstitial fibrosis, was elevated in MVP-MAD patients even without LGE, and was correlated with MAD length and increased risk of out-of-hospital cardiac arrest. ECV assessment may provide additional value in arrhythmic risk stratification beyond LGE evaluation in MVP patients.
