The incidence of vertebral fragility fractures and deformity increase steeply with age. Every sixth woman and every twelfth man will sustain a symptomatic vertebral fracture. Vertebral fractures result in pain, functional disability and decreased quality of life, which may last for several years, and may also affect mortality. The patient with an acute fracture should be examined with radiology for diagnosis. In case of a low-energy fracture, osteoporosis should be suspected and investigated. If the pain management fails, vertebroplasty or kyphoplasty could be considered. Braces may be used, but evidence for its effect is lacking. In the rare event of neurological compromise, or unstable fractures, surgical treatment should be considered. After vertebral fragility fractures, the risk for new fractures is high and secondary preventive measures advocated. The best evidence for secondary prevention is currently on medical treatment of osteoporosis. (C) 2014 Elsevier Ltd. All rights reserved.

Effectiveness of laminectomy with fusion and laminectomy alone in degenerative cervical myelopathy

Idiopathic scoliosis: a systematic review and meta-analysis of heritability

Summary: Twin and family studies indicate a hereditary component in idiopathic scoliosis, but study heterogeneity is large, and the degree of the heritability is uncertain. Nevertheless, known genetic variants associated with idiopathic scoliosis can still only explain a minor part of heritability.

Serum metal ion levels following spinal deformity surgery: a case-control study of 182 individuals

Summary: This study evaluates the levels of chromium, cobalt, and titanium after fusion surgery for scoliosis. It finds that the use of stainless steel and titanium implants in spinal fusion surgery is associated with elevated metal ion concentrations several years after surgery.

Postoperative outcomes of pedicle screw instrumentation for adolescent idiopathic scoliosis with and without a subfascial wound drain: a multicentre randomized controlled trial

Summary: This study aimed to assess if omitting the subfascial drain after correcting adolescent idiopathic scoliosis (AIS) would yield comparable results to using a drain in terms of blood loss, hemoglobin level, and opioid consumption. The results showed that omitting the drain did not lead to increased postoperative hemoglobin levels, but patients without the drain required 30% more opioids during the first 48 hours.

Validity of the short musculoskeletal function assessment questionnaire in patients with a spine fracture

Summary: The aim of this study was to validate the use of the Short Musculoskeletal Function Assessment (SMFA) questionnaire in patients with a spine fracture. The results showed that the SMFA indices are highly correlated with ODI in patients with a spine fracture. Therefore, the SMFA indices can be used as an alternative to assess the outcome in patients with spine fractures.

Evolution of patient-reported outcome measures, 1, 2, and 5 years after surgery for subaxial cervical spine fractures, a nation-wide registry study

Summary: This study assesses the long-term effects of surgical fixation for cervical spine fractures by examining patient-reported outcome measures (PROMs) at 1, 2, and 5 years postoperatively. The results showed that PROMs remained stable beyond 1 year and the 1-year PROMs accurately predicted the outcomes at 2 and 5 years.
