Metabolic syndrome epidemic among Korean adults: Korean survey of Cardiometabolic Syndrome (2018)

Background and aims: Little information exists on the prevalence of metabolic syndrome (MetS) in Korea since 2007. We aimed to provide up-to-date estimates of the prevalence of MetS and its trend in the general adult population in Korea. Methods: We compared the prevalence and pattern of MetS among participants in the Korean National Health and Nutrition Examination Surveys (KNHANES) IV (2007-2009), V (2010-2012), and VI (2013-2015), aged >= 19 years. Data from the 2005 census of the Korean population were presented according to age standardization. Results: The overall age-standardized prevalence of MetS in 2013-2015 was 20.3% (95% confidence interval [CI], 19.6%-21%). Since 2007, the overall prevalence of MetS has remained stable, whereas the prevalences among men and women, respectively, have increased and decreased slightly. By contrast, the prevalence of MetS among men aged 19-49 years has shown an increasing tendency since 2007. Moreover, nearly 40% of women aged >= 60 years had MetS in 2013-2015. Among the five components of MetS, only elevated fasting glucose level has shown an increasing trend since 2007 in both men and women. As the family income and educational level decreased, the prevalence of MetS increased. Conclusions: The overall prevalence of MetS has remained stable since 2007. However, the prevalence of MetS was higher in middle-aged men and women aged >= 60 years. Considering the close association between MetS and socioeconomic status, age-and sex-specific strategies should be developed at the national level for the treatment and prevention of MetS in Korea.

Adiponectin and cardiometabolic trait and mortality: where do we go?

Summary: Adiponectin, a cytokine derived from adipocytes, has cardioprotective effects. While early and epidemiologic studies did not find a relationship between adiponectin and coronary artery disease, larger-scale studies reported a paradoxical increase in all-cause and cardiovascular mortality with higher adiponectin levels. The underlying mechanisms behind this paradox are still unclear. Differentiating between different subtypes of heart failure may help understand the adiponectin-mortality relationship. Mendelian randomization studies should be interpreted with caution due to potential confounding factors.

Association of Dipstick Proteinuria with Long-Term Mortality among Patients with Hypertensive Crisis in the Emergency Department

Summary: Proteinuria in hypertensive crisis is associated with all-cause mortality, with even trace proteinuria increasing the risk of mortality. Dipstick urine test could be a simple and useful method for assessing mortality risk in these patients.

Association between Age of Onset of Hypertension and Incident Atrial Fibrillation

This state-of-the-art review discusses the unique challenges in establishing cardio-oncology in Asia, including socio-economic disparity, ethnical and racial diversity, and limited training opportunities. It also provides strategies to overcome these barriers and help advance the field of cardio-oncology in Asia.

Adherence and clinical outcomes for twice-daily versus once-daily dosing of non-vitamin K antagonist oral anticoagulants in patients with atrial fibrillation: Is dosing frequency important?

Summary: This study found that twice-daily dosing of non-vitamin K antagonist oral anticoagulants (NOACs) may lead to reduced drug adherence and worsen clinical outcomes in patients with atrial fibrillation (AF). The adherence to each NOAC and the subsequent clinical outcomes were compared between different dosing regimens. The results showed that adherence was high and similar between once- and twice-daily dosing regimens, but patients with low adherence had poorer clinical outcomes regardless of the dosing frequency.

Association between D-dimer and long-term mortality in patients with acute severe hypertension visiting the emergency department

Summary: This study investigated the association between plasma D-dimer levels and long-term mortality in patients with acute severe hypertension. The results showed that patients with higher D-dimer levels had a significantly increased risk of all-cause mortality within three years.

The Use of Phentermine for Obesity in Psychiatric Patients With Antipsychotics

A Case of Abnormal Liver Function Tests in a Patient Receiving Total Parenteral Nutrition

Summary: Hepatic dysfunction is common in TPN patients and can be caused by steatosis, cholestasis, and cholecystitis. Regular assessments are necessary, even for stable patients on long-term TPN. It is important to establish a differential diagnosis and investigate other potential causes of elevated liver enzymes. We present a case of a malnourished patient on TPN who had significantly elevated liver enzymes due to concurrent use of traditional Chinese herbal medications. Liver enzymes returned to normal levels after discontinuing the herbal medications.

Comparison of Newly Proposed LDL-Cholesterol Estimation Equations

Summary: This study aimed to investigate the association between cardiovascular events and elevated on-treatment diastolic blood pressure at different levels in individuals who achieved systolic blood pressure targets. The results showed that elevated on-treatment diastolic blood pressure according to the JNC7 definition was associated with a higher risk of major cardiovascular events, while elevated diastolic blood pressure according to the 2017 ACC/AHA definition was only associated with a higher risk of stroke.
