Cardiovascular function in healthy Himalayan high-altitude dwellers

Background: Residents of the Himalayan valleys uniquely adapted to their hypoxic environment in terms of pulmonary vasculature, but their systemic vascular function is still largely unexplored. The aim of the study was to investigate vascular function and structure in rural Sherpa population, permanently living at high altitude in Nepal (HA), in comparison with control Caucasian subjects (C) living at sea level. Methods and results: 95 HA and 64 C were enrolled. Cardiac ultrasound, flow-mediated dilation (FMD) of the brachial artery, carotid geometry and stiffness, and aortic pulse wave velocity (PWV) were performed. The same protocol was repeated in 11 HA with reduced FMD, after 1-h 100% O-2 administration. HA presented lower FMD (5.18 +/- 3.10 vs. 6.44 +/- 2.91%, p = 0.02) and hyperemic velocity than C (0.61 +/- 0.24 vs. 0.75 +/- 0.28 m/s, p = 0.008), while systolic pulmonary pressure was higher (29.4 +/- 5.5 vs. 23.6 +/- 4.8 mmHg, p < 0.0001). In multiple regression analysis performed in HA, hyperemic velocity remained an independent predictor of FMD, after adjustment for baseline brachial artery diameter, room temperature and pulse pressure, explaining 8.7% of its variance. On the contrary, in C brachial artery diameter remained the only independent predictor of FMD, after adjustment for confounders. HA presented also lower carotid IMT than C (0.509 +/- 0.121 vs. 0.576 +/- 0.122 mm, p < 0.0001), higher diameter (6.98 +/- 1.07 vs. 6.81 +/- 0.85 mm, p = 0.004 adjusted for body surface area) and circumferential wall stress (67.6 +/- 13.1 vs. 56.4 +/- 16.0 kPa, p < 0.0001), while PWV was similar. O-2 administration did not modify vascular variables. Conclusions: HA exhibit reduced NO-mediated dilation in the brachial artery, which is associated to reduced hyperemic response, indicating microcirculatory dysfunction. A peculiar carotid phenotype, characterized by reduced IMT and enlarged diameter, was also found. (C) 2014 Elsevier Ireland Ltd. All rights reserved.

Its exact cause is still poorly understood, but it is likely to involve a combination of genetic and environmental factors, leading to arterial fibrosis, cellular hyperplasia, and distortion of the arterial wall architecture. FMD can affect various arteries throughout the body, not just the renal and cerebrovascular arteries.

Summary: This study presents technical outlooks on computerized carotid intima-media thickness (CIMT) measurements and provides considerations for the development and comparison of these methods. A multi-center database of 500 images was collected and three manual segmentations and seven computerized methods were employed to measure the CIMT. The computerized methods showed comparable results with previous studies and demonstrated promising segmentation outcomes. The entire database has been made publicly available for further research and comparison.

Speckle tracking echocardiography in heart failure development and progression in patients with apneas

Summary: Obstructive and central apneas have negative impacts on cardiac structure and function, with obstructive apneas promoting progressive cardiac alterations leading to heart failure. Early identification of these effects and detection of initial cardiac dysfunction in patients with apneas are becoming increasingly relevant. Speckle tracking echocardiography is recognized as an effective method for early detection of cardiac dysfunction, but more research is needed on apnea-related alterations in heart failure progression.

Summary: Repeat head CT in patients on DOACs with MTBI after an initial negative CT scan is a common clinical practice. In this study, researchers assessed the incidence of delayed ICH and its association with clinical and risk factors presented on patient arrival in the ED. The results showed that delayed ICH is an uncommon event at the 24-hour control CT and does not affect patient outcome.

A causal learning framework for the analysis and interpretation of COVID-19 clinical data

Summary: We propose a clinical data analysis workflow based on Bayesian Structure Learning (BSL). This workflow incorporates prior medical knowledge into the learning process and provides explainable results in the form of causal connections among analyzed features. Evaluation on a COVID-19 dataset shows that the proposed framework gives a schematic overview of the multi-factorial processes contributing to the outcome and rediscovers established cause-effect relationships. Furthermore, the approach yields a highly interpretable tool that accurately predicts the outcome using a small number of features.

Decision tree analysis for assessing the risk of post-traumatic haemorrhage after mild traumatic brain injury in patients on oral anticoagulant therapy

Summary: This study showed that post-traumatic intracranial hemorrhage (ICH) risk in patients with mild traumatic brain injury (MTBI) on oral anticoagulant therapy (OAT) is associated with clinical risk factors such as post-traumatic amnesia, post-traumatic transitory loss of consciousness, greater trauma dynamic, GCS less than 15, and evidence of trauma above the clavicles. The absence of concussion and neurological alteration at admission significantly reduces the likelihood of ICH. The use of a machine-learning-based CART model can help stratify patients and guide decision-making in this population.

Arterial pulse wave modeling and analysis for vascular-age studies: a review from VascAgeNet

Summary: Arterial pulse waves (PWs), such as blood pressure and photoplethysmogram (PPG) signals, are rich sources of information for assessing vascular age and identifying individuals at elevated cardiovascular risk. This review explores the possibilities and limitations of reduced-order biophysical models and theoretical and empirical methods for analyzing PW signals and extracting clinically relevant information. Detailed mathematical derivations are provided, demonstrating the relationships between these models and methods. Suggestions for future research are also outlined to fully utilize the potential of modeling and analyzing PW signals for accurate vascular age assessment.

Vascular function in hypertension: does gender dimension matter?

Summary: Blood pressure and vascular ageing trajectories differ between men and women due to sex-related and gender-related factors. Endogenous 17β-estradiol plays a protective role in women's vascular aging, but gender-related factors such as psychological stress and low socioeconomic status have a greater impact on vascular aging in women. On the other hand, men are more susceptible to the adverse effects of alcohol consumption and social deprivation. Unhealthy diet habits and smoking have a disproportional negative effect on women's vascular health. Therefore, future research and prevention strategies should take into account the complex role of gender-related factors.
